Affinity-purified antibodies of defined specificity for use in a solid-phase microplate radioimmunoassay of human Tamm-Horsfall glycoprotein in urine.

Rabbit antibodies to human Tamm-Horsfall glycoprotein (prepared by salt precipitation from normal urine) were purified by affinity chromatography using columns containing Tamm-Horsfall glycoprotein linked to CNBr-activated Sepharose 4B. The specificity of these antibodies was determined by analysis of their binding characteristics on Western blots of Tamm-Horsfall protein from sodium dodecyl sulphate/polyacrylamide gradient gels and comparison with the reactivity of monoclonal antibodies to this glycoprotein. Optimal conditions of adsorption to poly(vinyl chloride) microtitre plates were established such that these purified antibodies could be used in a solid-phase radioimmunoassay for the determination of urinary Tamm-Horsfall-glycoprotein concentration. The specificity of the immunoassay was confirmed by competitive inhibition of the urinary Tamm-Horsfall glycoprotein by purified freeze-dried material in solution. A standard curve obtained with this material showed the radioimmunoassay to have a sensitivity of at least 5 ng/ml, with linearity between 30 and 600 ng/ml. The mean coefficient of variation over the linear section of the curve was 11.3 +/- 2.2% (n = 13). The effects of dialysis and freezing of urine samples before determination of Tamm-Horsfall-glycoprotein concentrations were investigated and the mean 24 h urinary excretion rate in 60 normal donors was shown to be 84.9 +/- 44.1 mg.

Penetration of ceftazidime into the human prostate gland following intravenous injection.

The concentration of ceftazidime in human prostatic tissue obtained by transurethral resection was measured by microbiological assay in 24 patients after the intravenous administration of one 2 g dose. The average concentration of ceftazidime in the tissue was 10.1 micrograms/g 70 min after injection and 6.1 micrograms/g 140 min after injection; these levels were each approximately 14% of the corresponding serum levels. The concentration of ceftazidime achieved in the prostatic tissue was greater than the minimum inhibitory concentrations of ceftazidime for Gram-negative pathogens.

Serial prostatic histology. A valid marker of response to hormone treatment.

Serial transrectal needle biopsies were taken from 146 patients (657 specimens) with advanced prostatic cancer (T3/T4, M1/MO) admitted to a trial of two forms of oestrogen therapy. Histological grading by Gleason and Mostofi techniques on the initial biopsy showed no correlation either with extent of disease or its eventual outcome on treatment. Change in grade was dissociated from the response of bone metastases to hormone treatment and did not appear to influence survival. However, the prognosis of 16 patients showing clearance of tumour from their serial biopsies was generally good.

Bone imaging and serum phosphatases in prostatic carcinoma.

One hundred and twenty-seven patients with locally advanced prostatic cancer were evaluated for the presence and progress of bone metastases before and during hormonal therapy, by serial radionuclide imaging and frequent measurement of plasma acid (tartrate-labile) and alkaline phosphatase. For comparison, serial changes in imaging and phosphatases were classified in each patient into one of six groups. Of 71 patients with negative imaging before treatment, 82% had normal alkaline phosphatase levels and 83% had normal acid phosphatase levels. Of 56 patients with bone metastases at presentation, false negative alkaline and acid phosphatase levels were noted in 18% and 36% respectively, though a few patients eventually developed abnormal levels. Serial plasma biochemistry and particularly alkaline phosphatase showed a response to treatment which was not always obvious on imaging. An assessment of the hepatic component of alkaline phosphatase by reference to plasma gamma glutamyl transpeptidase and isoenzyme electrophoresis was helpful in the evaluation of a false positive result but unnecessary where imaging was positive and phosphatase elevated. It is concluded that serial alkaline phosphatase estimation is essential in the follow-up of patients with prostatic cancer and bone metastases, and probably renders serial imaging studies superfluous once the presence of skeletal metastases has been proven. By comparison, acid phosphatase is a much less effective marker.

The conus demyelination syndrome in multiple sclerosis.

Bowel, bladder and sexual dysfunction are common in multiple sclerosis and are generally attributed to the widespread nature of the involvement of the neuroaxis by the demyelinating plaques. Recently we encountered a specific subset of patients within this group who had characteristic clinical complaints of hesitancy, straining and incomplete voiding, perineal hypesthesia on examination, areflexia by cystometry and colonometry and electrophysiological parameters suggesting involvement of the conus medullaris. This heretofore postulated but undocumented mechanism of neurovisceral dysfunction in multiple sclerosis is detailed and discussed in 2 patients in this report.

Ceftazidime: a new approach in the treatment of moderate and severe infections.

Ninety patients with serious infections, including 61 with septicaemia, pneumonia, peritonitis or meningitis, were treated with ceftazidime. Of these patients, 85.6% were clinically cured (73.3%) or improved (12.2%) by the antibiotic. In this study, 57.7% had infections due to Escherichia coli (24.7%), Klebsiella sp. (14.5%) and Pseudomonas sp. (18.5%). Two children with cystic fibrosis and Pseudomonas pneumonia and an adult with Legionella pneumonia responded well to ceftazidime treatment. Seventy patients had fever before treatment and most of them became apyrexial in less than 2 to 3 days. Ceftazidime was given either intramuscularly (42 patients) or intravenously (48 patients), in a dose of 1 g tds in 71 patients or 2 g tds in severe infections in 11 patients, or reduced to suit the renal function (7 patients) or in paediatric doses (2 children). Blood ceftazidime levels were measured in eight patients with normal renal function. The average level one hour post dosing was 45.2 mg/l and the average trough level was 8.1 mg/l. Six patients were suffering from variable degrees of renal insufficiency (serum creatinine 149 to 668 mmol/l). Their average blood level 1 h post-dosing was 68.8 mg/l. In a patient with meningitis, the CSF level was 2.4 mg/l 2 h after a 1 g dose. These levels are several times the ceftazidime MIC values for most clinical bacterial isolates. Ceftazidime is a valuable and safe alternative to aminoglycoside therapy.

The choice of weapons: a study of methods of suicide by sex, race, and region.

The relationship of sex, race and region to methods of suicide are used to test the validity of two theories which attempt to explain the choice of methods among suicide victims. The two theories, lethality of intent and differential socialization with firearms, are briefly discussed. Data are collected from official death certificates in five cities during a three year period, 1969 through 1971. The findings provide little support for the lethality of intent theory. Although the differential socialization theory receives partial support, one can argue for a broader sociocultural perspective on methods of suicide which includes location in the social structure as well as regional socialization.

Isochromosome for the long arm of the Y in an infertile male.

A 37-year-old man investigated for infertility had bilateral atrophic testes. Cytogenetic investigations revealed a chromosome complement of 45,XO/46,Xi(Yq)/46,XY. Mechanisms for the origin of the i(Yq) are considered, and the relation of his chromosome constitution to his infertility and hypogonadism are discussed.

Pion cancer therapy: positron activity as an indicator of depth-dose.

Negative pions are useful for radiation therapy of localized carcinoma because of their large end-of-range energy deposition upon capture in nuclei and the potentially accurate spatial localization of the radiation dose. Accurate determination of the depth of penetration of the pion beam in vivo can be accomplished by counting the back-to-back annihilation gamma rays resulting from positron activity induced by the stopped pions.

Patient preference for self-collected cultures for group B streptococcus in pregnancy.

To determine pregnant women's preference for self-culture technique, 251 women between 24 and 42 weeks' gestation were interviewed after performing self-collected cultures (vaginal and rectal) for group B streptococcus. Patient receptiveness to self-culture, the ability to perform self-culture, and the desire for choice in the future were derived using the Patient Preference Tool. The majority of women (77%, n = 194) gave positive descriptions of self-culture technique, and the majority of women preferred self-culture technique over nurse-collected sampling (57%, n = 142). Seventy-nine percent (n = 197) stated their desire to have a choice about self-culture in the future when similar testing was needed, and 89% (n = 224) believed that other women would also like this choice. Additionally, patient samples were highly correlated with nurse-collected samples for accuracy of culture results. This study provides data supporting that women desire active participation in their care.
