Hyperdense renal masses: a computed tomographic dilemma.

High-density renal masses are atypical lesions that represent a diagnostic dilemma for computed tomography (CT). The differential diagnosis includes mainly complicated benign cysts and malignant neoplasms. This report analyzes the CT findings in 17 cases of discrete, sharply marginated masses that initially exceeded the density of the uninvolved renal parenchyma, averaging 50 H or more. The subsequently proven pathologic entities responsible for these high attenuation values included primary malignancies in six patients, one of whom also had multiple retention cysts containing highly proteinaceous, gelatinous material; a hemorrhagic cysts with an associated desmoplastic, fibrotic reaction in one patient; and single or multiple hemorrhagic cysts in four patients. No histologic proof was obtained in six patients, three with neoplastic disease and one mentally retarded youth who underwent renal aspiration without sufficient material obtained for analysis. Two patients with presumed hemorrhagic cysts have had serial CT scans without interval change in 8-12 months. An approach to the therapeutic management of affected patients is described.

The diagnosis of puerperal ovarian vein thrombophlebitis by computed axial tomography scan.

The diagnosis of puerperal ovarian vein thrombophlebitis by computed axial tomography (CAT) scan is reported. The characteristic finding, ie, a soft tissue mass with a diameter of 2 cm and length of 14 cm, extending from the uterine cornua to the level of the renal vein, can be seen by CAT scan. The near total resolution of the mass on follow-up CAT scan confirmed successful treatment with antibiotics and heparin. Observation of these findings will allow future cases to be definitively diagnosed and treated without invasive procedures.

Detection of open spina bifida by the lemon sign: pathologic correlation.

This study was undertaken to evaluate the association of medial displacement of the frontal bones of the fetal calvarium, the lemon sign, with open spina bifida (OSB) and to assess other pathologic findings associated with this change. During the past 8 years, 8 cases of OSB were seen in our institution. The lemon sign was retrospectively identified in 6/6 cases of OSB scanned before 22 weeks gestation, while it was not seen in 3/3 cases examined after 26 weeks. One case was evaluated with serial studies before and after 22 weeks. Scalloping of the frontal bones was associated with a wide variety of pathological findings. This sign may prove to be an important marker for OSB.

CT demonstration of pulmonary effects of tangential beam radiation.

Breast cancer is sometimes treated with an excisional biopsy and a radiation portal limited to the breast and the adjacent chest wall, especially in patients with negative lymph node dissections. The beam passes through this portion of the chest wall tangentially. Such radiation can result in changes in the included lung parenchyma that are pleural-based and sharply demarcated from the normal lung on CT lung windows. In our experience CT lung windows were more sensitive than chest radiography in showing these changes. Such changes must be recognized and differentiated from pleural metastases. Computed tomography also resulted in a more specific diagnosis than chest radiography because it better localized the abnormality to the radiation portal.

CT diagnosis of villous adenoma.

Villous adenoma may have a specific appearance on barium enema examination but specific CT findings of villous adenoma have not been previously described. Two recently encountered cases of villous adenoma in the rectum had a distinctive pattern on CT. Suggested criteria for CT diagnosis of villous adenoma include a homogeneous water density component of less than 10 HU, occupying more than half the lesion and eccentric on the luminal side of the mass. This low density component interfaces with the majority of the luminal surface of the lesion. No air-fluid level is seen and the lesion should not have a round cystic configuration. A description of these findings is illustrated and discussed.

MR imaging of fetal brain.

Magnetic resonance (MR) imaging was used to evaluate normal fetal intracranial anatomy in axial, coronal, and sagittal planes. The T1 and T2 weighted images (WI) of aborted fetuses of varying gestational ages were correlated with anatomic sections. In the premature fetus three distinct intensity zones were seen on MR that were not visualized on gross specimens. Unmyelinated white matter displays low intensity on T1 W1 and high intensity on T2 W1. Maturational changes of the brain were observed with advancing fetal age.

Developmental models of social cognition in assessing the role of family stress in relatives' predictions following traumatic brain injury.

The present study extended the utilization of developmental models of social cognition to the investigation of stress and relatives' perception of traumatic brain injury (TBI) survivors. Structured interviews were conducted with 21 TBI survivors utilizing interpersonal negotiation strategies (INS) and self understanding (self) in the framework of Selman's model of social perspective-taking and Damon and Hart's multidimensional model of self understanding. A relative group composed of 21 participants was interviewed and their predictions of the responses of the TBI survivors to the action domain of the INS stories were obtained. The relative participant group was also administered the Beck Anxiety and Depression Inventories and Leeds Scales of Depression and Anxiety. The relationships among relative groups' predictive scores, INS and self domains and stress levels were analysed. The result of the survivor groups responses on INS and self were in agreement with the previous findings that TBI survivors respond at psychosocially immature levels. Comparison of INS action scores obtained by the survivor group and predicted by relative group were within one developmental level of each other in 87.4% of the cases. Fifty-two per cent of the relative group scored in mild to moderate or greater depression and 48% scored in the mild to moderate range of anxiety on the Beck scales. Person correlation coefficients indicated significant negative correlations between Beck scores and predictive INS scores. ANOVA indicated significantly higher Beck depression scores in relatives of TBI survivors living in residential facilities than relatives of TBI survivors in an outpatient treatment programme. The study supports the view that developmental social cognition methods appear to advance our understanding of psychosocial adjustments and relatives' perceptions of social cognition in TBI survivors.
