Community Treatment Teams: an alternative to state hospital.

This article describes the implementation of Community Treatment Teams as a strategy to provide services to patients discharged from a state psychiatric hospital. The closing of Philadelphia State Hospital was the impetus for the development of an alternative long term care treatment system for the seriously mentally ill which was based on a variation of the community treatment team model. This article provides a description of the teams, their functioning, structure, and the impact they have had on the service system.

Evaluation of intensive case management for seriously and persistently mentally ill persons.

Case management and continuity of care have been an integral part of the community mental health movement since its inception in the United States in 1963. The application of these concepts in treating the seriously and persistently mentally ill has, however, undergone continuous change. The most recent application is called intensive case management. In this article, preliminary findings from a 3-year evaluation of intensive case management programs in two Pennsylvania community mental health agencies are discussed. Results show only limited improvement in client level of functioning, but substantial desired change occurred in treatment utilization patterns and reduced cost of care.

Accreditation, certification, and the quality of care in state hospitals.

Data on 216 state psychiatric hospitals were analyzed to determine whether accreditation by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or certification by the Health Care Financing Administration (HCFA) were related to seven hospital characteristics generally accepted as reflecting quality of care. The characteristics were average cost per patient, per diem bed cost, total staff hours per patient, clinical staff hours per patient, percent of staff hours provided by medical staff, bed turnover, and percent of beds occupied. While a majority of the hospitals had either JCAHO accreditation, HCFA certification, or both, analysis revealed a weak relationship between accreditation or certification status and the indicators of quality of care. Accredited or certified hospitals were, however, more likely to have higher values on specific indicators than hospitals without accreditation.

Increased use of psychiatric emergency services by young chronic mentally ill patients.

As part of an investigation of the causes of increased use of Philadelphia's seven psychiatric emergency services, staff of the services were asked to identify the problems and changing conditions facing them. Difficulty securing inpatient beds for public patients and heavy use of services by a small number of patients fitting the description of young adult chronic patients were mentioned by staff at most of the services as among the most important problems or changes. The staffs' perceptions were confirmed in the second part of the study, which analyzed emergency service data for fiscal years 1979 through 1985. The data analyses also indicated that the heavy users continued to make extensive use of emergency services for several years following their first admission, causing their total number of visits to climb from 3,048 in 1980 to 14,839 in 1985. The authors suggest immediate strategies for responding to the emergence of these young, very disturbed patients.
