Clinicopathologic comparisons between estrogen receptor-positive and -negative gastric cancers.

We report that a modified dextran-coated charcoal (DCC) assay, including the addition of sodium molybdate and 5% DCC stripping of endogenous hormone, detected higher estrogen receptors (ERs) than those by the conventional assay. ERs in 21 gastric adenocarcinoma were determined by the modified DCC assay; 13 patients had ERs of 2.5 to 520.2 fmol/mg protein with a mean dissociation constant of 1.9 x 10(-10) M. The remaining 8 patients had no detectable amount of the receptor. There were no differences between the ER-positive and ER-negative groups in clinico-pathologic characteristics such as age, sex, tumor size, location, gross appearance, invasive depth, invasion of lymph vessel or vein of stomach wall, nodal involvement, peritoneal dissemination, liver metastasis, and curability. Histological analysis, using Japanese, Lauren's, or Ming's classifications, all revealed no differences between the ER-positive and -negative groups. The survival rate was identical for the 2 study groups. These results suggest that the presence or absence of ERs in gastric adenocarcinoma does not correlate to either biologic or clinicopathologic characteristics of this tumor; the role of ERs in human gastric cancer remains to be elucidated.

Chondrosarcoma of bone: a statistical analysis of prognostic factors.

From 1980 to 1988, 30 cases of chondrosarcoma of the bone were treated surgically. Twenty-seven cases were followed from 6 months to 8.7 years (mean, 3.2 years). In all cases, a clinicopathological review was performed and the significance of prognostic factors were analyzed. The five-year survival of total patients was 88%. The survival was related to adequacy of treatment and histologic grade of the tumor. The five-year survival of adequately treated patients was 100%, but in inadequately treated patients it was 68%. At 8.7 years, 100% of the patients with grade-1 or grade-2 tumors survived, compared to 50% of the six-month survival rate in patients with the grade-3 tumors. None of the grade-1 or grade-2 chondrosarcomas metastasized, while 75% of grade-3 chondrosarcomas metastasized. The incidence of local recurrence was influenced by adequacy of treatment. At the final follow-up, the recurrence rate in the adequately treated group was 7%, while in the inadequately treated group it was 57%.

Endothelium-dependent production of prostacyclin in human internal mammary artery.

The internal mammary artery (IMA) has become the conduit of choice in coronary artery bypass surgery because of superior long-term patency. It had been shown that IMA graft could release prostacyclin. Prostacyclin is a potent vasodilator and also can inhibit platelet aggregation. To determine the role of human IMA endothelium in production of prostacyclin, we tested the reactivity of segments of human IMA to hypoxia in vitro. Human IMAs were harvested during coronary artery bypass surgery. Prostacyclin was measured from fluid in the organ baths by radioimmunoassay of its major hydrolytic product 6-keto-prostaglandin F1 alpha. Rings (4 mm in length) of IMA, with and without endothelium, were suspended in organ baths containing physiologic salt solution. Rings were contracted with norepinephrine, and exposed to hypoxia (pO2 35 +/- 5 mmHg) for 15 minutes then reoxygenated. In segments with endothelium, hypoxia induced a transient relaxation followed by contraction. The transient relaxation was associated with a significantly increased production of 6-keto-prostaglandin-F1 alpha (from 34.1 +/- 2.7 pg/ml prehypoxia to 51.6 +/- 6.7 pg/ml during hypoxia, mean +/- SEM, p less than 0.05). This transient relaxation was blocked by indomethacin but not by NG-monomethyl-L-arginine (L-NMMA) and free radical scavengers (superoxide dismutase, catalase and deferoxamine). However, in segments without endothelium, the prehypoxia (14.7 +/- 0.9) and during hypoxia (15.5 +/- 1.4) level of 6-keto-prostaglandin F1 alpha were not increased and were significantly lower than those with endothelium. This study demonstrated that endothelium of IMA grafts could release prostacyclin either in a basal condition or upon stimulation of hypoxia. This ability possibly contributes to its long-term patency.

Protease inhibitor and defective proteolysis in cystic fibrosis.

Meconium specimens from 18 infants with cystic fibrosis (CF) had strong trypsin inhibitory activity (TIA). The same specimen, which contained increased quantities of undigested proteins, had normal concentrations of immunoreactive trypsin (IRT), but deficient trypsin catalytic activity (TCA). TIA was not detected in any specimen from non-CF infants who had high concentration of proteins comparable to that of CF infants. Subjecting meconium supernatant of CF infants to Sephadex G-75 gel filtration revealed that TCA was greatly enhanced in effluents after fractions were activated by porcine trypsin. TCA was present in the same fractions with IRT. The findings suggested that proteases were secreted into the intestinal lumen in CF infants prior to birth. Deficient proteolysis in the disease might be due to the presence of a trypsin inhibitor.

Use of the dexamethasone suppression test to detect depressive disorders of mentally retarded individuals.

The applicability of the Dexamethasone Suppression Test (DST) with 39 mentally retarded individuals was examined. Subjects were assigned to one of three groups based upon the presence of zero, one, or two (or more) primary behaviors: unprovoked aggressive/assaultive behavior, self-injurious behavior, and severely withdrawn behavior. Chi-square analysis revealed that individuals exhibiting two or more of these types of behavior were more likely to obtain a positive DST result than were individuals exhibiting only one behavior. Results support the hypothesis that retarded persons can become depressed (as defined by the DST) and may express the depression through these behaviors. Furthermore, these results suggest that the current diagnostic criteria for depression in the retarded population need to be revised.

Comparative diagnostic value of phenylalanine challenge and phenylalanine hydroxylase activity in phenylketonuria.

Serum phenylalanine (phe) concentrations during and following phe challenges and liver phenylalanine hydroxylase (PH) activity were compared in 13 phenylketonuric (PKU) patients. These patients were separated into two groups: eight patients with no detectable PH activity (PH degrees) and five patients with residual PH activity (PH-) ranging from 9 to 24% of the activity obtained in 10 non-PKU subjects. The rise in serum phe concentration during 3 days of oral loading did not differentiate the two groups. However, the difference in serum phe concentration of the PH degrees and PH- groups reached statistical significance at 24 h postloading (p less than 0.01). We concluded that combined results from multiple measurements during the oral challenge, namely serum phe concentration after termination of loading, serum phe clearance rate, post-loading phe tolerance index and urinary metabolite excretion, make a better indicator for predicting residual PH activity for the majority of PKU subjects than peak phe concentrations during phe challenge.

Fluorescent spot test for detecting carbohydrate-splitting enzymes in meconium.

I describe a fluorescent spot test for detecting the acidic glycosidases in meconium. Meconium is mixed with various 4-methylumbelliferylglycosides at the appropriate pH. In the presence of carbohydrate-splitting enzymes, 4-methylumbelliferone, a strongly fluorescent compound, is released. The reaction mixture, when spotted on chromatography paper and viewed under ultraviolet light, exhibits a bright fluorescence for specimens containing acidic glycosidases. The technique is applicable to any test that involves production of fluorescent products from non-fluorescent reactants. The procedure was applied to meconium specimens selected because of increased albumin content known to be associated with cystic fibrosis in newborns. The presence of beta-D-fucosidase in meconium was correlated to increased albumin content. Detection of beta-D-fucosidase by the present procedure may be useful as a screening test for cystic fibrosis.

Distribution of phenylalanine hydroxylase (EC 1.14.3.1) in liver and kidney of vertebrates.

The range of phenylalanine hydroxylase activity was determined by measuring the conversion of radioactive phenylalanine to tyrosine in liver and kidney of various vertebrates. Rodents (rats, mouse, gerbil, hamster and guinea pig) were found to have the highest liver phenylalanine hydroxylase activity among all animals studied. They are also the only species that possessed a significant kidney phenylalanine hydroxylase activity which was about 25% of that found in the liver of the same animal. The synthetic dimethyl-tetrahydro-pteridine, used as a cofactor for the enzyme assay in most studies, catalyzed non-enzymatic hydroxylation of phenylalanine to tyrosine. Inclusion of boiled-blank and strict control of timing between incubation and product measurement were essential precautions to minimize erroneous results from substrate contamination and non-enzymatic hydroxylation.

Pre-operative serum levels of tissue polypeptide antigen in patients with gastric cancer.

Pre-operative serum tissue polypeptide antigen (TPA) levels were measured in cases of gastric cancer from December 1987 to December 1992. All 351 cases received gastrectomies. The clinicopathological factors were analysed. The significant factors that correlated with the elevation of pre-operative serum TPA levels included tumour size (> 7 cm), Borrmann-type cancers, late stages (III and IV), lymph node metastasis, hepatic metastasis and Ming's expanding type cancers. Multivariate analysis showed that the tumour size (> 7 cm) and the presence of hepatic metastasis are significant factors. To clarify the relationships between gastric cancer per se and the pre-operative serum TPA levels, we selected cases without evidence of metastasis (n = 139). The tumour size was the only significant factor when multivariate analysis was applied. Possibilities of hepatic recurrence were found in cases with high pre-operative serum TPA levels (> 220 U/L), even radical gastrectomies were performed. A high pre-operative serum TPA level did not display a poor survival prognosis, if the radical gastrectomy was possible. We thus concluded that: (i) elevated pre-operative serum levels of TPA are associated with either a large size tumour (> 7 cm) or the presence of hepatic metastasis and the tumour size is the most important factor relating to the serum TPA levels; (ii) high pre-operative serum TPA levels (> 220 U/L) may serve as indicators of later hepatic recurrence; and (iii) elevated serum TPA levels were not indicators of survival prognosis.

Poorer prognosis in young patients with gastric cancer?

BACKGROUND/AIMS: Although the relationship between prognosis and age of patients with gastric cancer is controversial, a poorer prognosis in young patients has been suggested by most investigators. To further examine the hypothesis, a retrospective study was undertaken to analyze a large series of patients with gastric cancer in Taiwan. METHODOLOGY: A total of 1,642 consecutive patients diagnosed with gastric cancer and receiving further management at one medical center from 1988 to 1993 were reviewed. The gender, TNM tumor stage, rate of curative resection and survival of the patients were compared in the young age group (< or = 39 years) and the old age group (> 39 years). Survival was estimated with the product-limit method and difference in survival was tested by the log-rank test. Multivariate analysis was done by the Cox proportional hazard model. RESULTS: Among the 1,642 patients, 61 patients were in the young age group and 1,581 patients were in the old age group. There was no significant difference for the 2 groups of patients in the distribution of TNM stage (stage I: 20%; II: 8%; III: 13%; IV: 59% vs. 19%, 11%, 25% and 45% respectively, in the old age group, p = 0.098) and rate of curative resection (38% vs 51% in the old age group, p = 0.059). The overall 5-year survival showed no significant difference between the 2 groups (25% vs. 29% in the old). Subgroup analyses showed that survival after curative resection and survival without curative treatment (including palliative resection and no resection) also had no difference in the 2 groups. Multivariate analysis also showed that age was not an independent factor. CONCLUSIONS: Although most reports suggested a dismal prognosis in young patients with gastric cancer, based on our findings, young patients (< or = 39 years) do not have a worse prognosis than older patients.

Comparison of the UICC/AJCC 1992 and 1997 pN categories for gastric cancer patients after surgery.

BACKGROUND/AIMS: UICC/AJCC 1997 classification changes pN category. We evaluated its prognostic impact. METHODOLOGY: A total of 710 patients who underwent a > or = D2 gastrectomy were recruited. Among them, the data of 319 patients who had involved regional lymph nodes and no evidence of distant metastases were used for comparing the 1992 and 1997 pN categories. RESULTS: For 1997 category, 201 patients (64%) were pN1, 75 (23.5%) pN2, and 43 (13.5%) pN3. For 1992 category, 143 patients (44.8%) were pN1, and 147(46.1%) pN2. 29 patients (9.1%) with lymph node metastasis to the hepatoduodenal ligament were distant metastasis. The 1997 pN category was a more powerful prognostic discriminant (relative risk: 2.086) than the 1992 category. Compared to the 1992 stage classification, the 1997 one had a skewed distribution of patients with marked shift of patients of stage IIIA (105-126 patients), IIIB (116-58 patients), and IV (100-122 patients). The survival difference between stage IIIA and IIIB for the 1997 stage classification is narrower than for 1992. CONCLUSIONS: The 1997 pN category allows for estimation of prognosis superior to the 1992 category.

Isolation, pathogenicity, and H120 protection efficacy of infectious bronchitis viruses isolated in Taiwan.

Seven isolates of infectious bronchitis (IB) virus (IBV) were isolated from two breeder farms and five broiler farms in Taiwan in 1992. The cardinal signs of disease in breeders were egg production drops and watery albumen, and those in broilers were respiratory distress and renal urate deposition or death. All diseased chickens had been vaccinated with IB vaccines (mostly H120). The viruses were isolated and identified by chicken embryo inoculation and electron microscopy. The genomes of the isolates were extracted and amplified by polymerase chain reaction; the restriction fragment length polymorphism analysis suggested that the genotypes of the present IBV isolates were different from the eight reference strains. One-day-old specific-pathogen-free chicks were inoculated with the field isolates in order to test the virulence of those isolates. Respiratory distress and depression commenced at 24 hours after inoculation. Two of the seven isolates were found to be highly virulent, causing 50% or more mortality in inoculated chicks. Vaccine protection tests showed that H120 could protect chickens against challenges with four of six field isolates.

Is gastric remnant cancer clinically different from primary gastric cancer?

BACKGROUND/AIMS: Poorer survival, uncertain etiology and a possible surgically induced carcinoma prompted the question of whether the gastric remnant cancer is a special form of gastric carcinoma. A retrospective study was done to see if gastric remnant cancer is clinically different from primary gastric cancer without consideration of its etiology. MATERIALS AND METHODS: There are 77 patient with gastric remnant cancer diagnosed in our hospital in past 15 years. Thirty-two out of 77 patients underwent gastrectomy. Another 536 patients with primary gastric cancer and 73 patients with proximal third gastric cancer were used as the controls. The clinicopathological characteristics including gender, age, TNM tumor stage, Lauren's classification, degree of tumor cell differentiation, rate of curative resection and 5-year survival rate were analyzed to see if there is significant difference among the three groups. RESULTS: Almost 90% of patients with gastric remnant cancer had a partial gastrectomy with Billroth II operation before and the median incubation interval was 24.5 years. Male predominance in gastric remnant cancer was shown in our series. However, the age distribution, TNM tumor stage, Lauren's classification, degree of tumor cell differentiation, rate of curative resection and 5-year survival after curative resection were no different among the three groups. CONCLUSIONS: There was no difference in clinical behavior of resected gastric remnant cancer from other gastric carcinoma. Since most of the tumors were detected too late to be resected, early detection is the only way to increase the resection rate and improve the survival. Annual endoscopic surveillance after gastrectomy was recommended for early detection.

Serum tumor necrosis factor in patients with gastric cancer.

We have measured serum TNF-alpha levels in 220 gastric cancer patients, 9 patients with gastric polyps or ulcers, 9 hepatitis B carriers and 85 normal controls. The results showed that no positive TNF-alpha value (> 10 pg/ml) was detected in Hepatitis B carriers and benign gastric lesions' patients and normal controls. In the cancer group, 17 out of 220 patients (7.7%) had positive-TNF-alpha values. The proportion of TNF-alpha positive was 6.8% in stage I disease, 6.5% in stage II disease, 3.7% stage III, and 12.9% stage IV. No clinicopathologic factors were related to positive TNF-alpha value. TNF-alpha value was not an independent prognostic indicator The role of TNF-alpha in gastric cancer remains obscures.
