Ventricular fibrillation in acute myocardial infarction.

Ventricular fibrillation (VF) developed in 7.42% patients with acute myocardial infarction (AMI). It was more frequent among patients with anterior myocardial infarction compared to inferior myocardial infarction. On the basis of sequence of events, cases of ventricular fibrillation (VF) were classified in to Primary (P), Secondary (S) and Agonal (A). 57.69% cases had SVF, 30.76% had AVF and only 11.53% cases had PVF. In all cases of PVF, ECG was normal prior to the development of VF and developed abruptly without any warning.

Effect of periodic antiascaris and antigiardia treatment on nutritional status of preschool children.

One hundred fifty-nine children aged 24 to 61 months with 60% ascariasis prevalence and 21.5% giardiasis prevalence in rural Guatemala were studied prospectively for 1 yr. They were divided into four groups comparable for age, sex, socioeconomic status, and past growth experience as judged by slopes of height and weight on age. Each group was randomly assigned to the following 2-monthly treatment regimens: group I, placebo, group II, piperazine, group III, metronidazole; group IV, piperazine and metronidazole. Height and weight were measured every 3 months and stools were examined for parasites every 4 months. Piperazine administration decreased the prevalence of ascariasis to 33.8% at the end of the study but growth remained unaltered. Metronidazole administration decreased the prevalence of giardiasis to 2.5% at the end of the study and was accompanied by increased growth as judged by delta weight, delta % weight for age, slope of weight on age, delta height, delta % height for age and slope of height on age. It is suggested that failure of antiascaris treatment to enhance growth in this study may be because of 1) absence of severe malnutrition in the subjects, 2) adequacy of dietary protein, 3) possible low worm load, and 4) failure to eradicate ascariasis. The findings suggest that giardiasis is associated with reduced growth in preschool children.

Diabetic status in leprosy.

The diabetic status of the local Jhansi patients (120 cases) was established before and after antileprosy treatment. Control studies were performed in normal healthy subjects (50 persons) without family history of diabetes mellitus. Random normals showed an incidence of diabetes only 2%, while leprosy patients (94 males and 26 females) had incidence of diabetic status of 14.2%. The highest incidence (19.3%) of diabetes was in lepromatous leprosy and lowest incidence (6.4%) in tuberculoid leprosy patients. Repeated studies in leprosy after treatment showed not only clinical improvement for leprosy but also disappearance of the chemical and latent diabetes mellitus and lowering of blood sugar levels in manifest diabetes mellitus. Incidentally it was noted that 'diabetic status' was worse among males (82.3%) and with advancing age. Association and improvement of diabetic status with specific treatment would tentatively suggest that Mycobacterium leprae lesions are not confined to skin alone but somehow also related to carbohydrate metabolism. A careful management of the chemical and latent diabetes may help in clinical management of leprosy too.

Complete heart block complicating acute myocardial infarction.

Complete heart block (CHB) developed in 10.3% of patients with acute myocardial infarction (MI). It was more frequent among patients with inferior myocardial infarction compared to anterior myocardial infarction, but the mortality was significantly high among patients with anterior MI who developed CHB. A new classification into primary (P) and secondary (S) CHB is suggested by the sequence of events. The incidence and mortality of SCHB was significantly high when compared to the incidence and mortality of PCHB. Wide QRS complexes, a heart rate of less than 60/min, and syncopal attacks were the grave prognostic signs. Progression from a lesser degree of AV block and regression of CHB to a lesser degree of AV Block were both observed in 25.8% cases. Circulatory failure in the form of shock, hypotension, congestive heart failure, and left ventricular failure, alone or in combination was a factor in 17 of the 22 patients who died. Four of 5 patients who underwent transvenous pacing also died.
