HL-A markers in the Vietnamese population.

A total of 126 normal, unrelated individuals from northern central and southern Vietnam have been typed for 32 alleles of the A and B loci, including B HS. The gene, haplotype frequencies and delta values obtained are compared with those of four other Mongoloid populations. The gene frequencies were similar to those found in a Chinese (Cantonese) population.

A comparative study on ibuprofen (Brufen) and indomethacin in non-articular rheumatism.

Sixty hospital out-patients between 18 and 67 years of age entered this double-blind parallel study set up to compare ibuprofen 1200 mg daily with indomethacin 75 mg daily. Both groups of patients received one capsule three times daily. Ten patients withdrew from the trial because of side effects and a further 3 were excluded--one because of an incorrect diagnosis and 2 because of incomplete assessments--leaving the statistical analysis to be performed on 47 patients. Assessments made after one and two weeks showed significant improvement for the parameters of pain and tenderness at each examination in both treatment groups. Restriction of movement also showed improvement, though not reaching statistical significance. Although there was no statistically significant difference beteen the two drug groups the mean improvement in all three parameters was consistently better with ibuprofen than with indomethacin. The commonest side effects were related to the gastrointestinal tract. Altogether, 10 patients withdrew because of side effects--4 on ibuprofen and 6 on indomethacin. The gastric symptoms occurring with indomethacin appeared to be generally more severe and usually occurred early, within 3 or 4 days of commencing treatment, whereas patients on ibuprofen withdrew usually after 7 or 8 days.

Effect of ibuprofen on blood pressure control by propranolol and bendrofluazide.

The effect of 1600 mg/day ibuprofen in two groups of patients with hypertension controlled by either propranolol or bendrofluazide was studied in a double-blind, double-placebo, randomized crossover trial. No significant difference in blood pressure was found at the end of the crossover period in either group, suggesting that the routine co-administration of ibuprofen does not attenuate the anti-hypertensive effect of thiazide diuretics or propranolol. Significant weight gain, attributable to fluid retention, had occurred in the bendrofluazide-treatment group by the end of the drug-free washout period. No significant change in mean weight occurred in the crossover stages in either group, although substantial weight gain was noted during ibuprofen treatment in two patients given bendrofluazide and one given propranolol. Biochemical variables were unaffected by ibuprofen throughout the crossover period. This study suggests that ibuprofen may be administered routinely to patients receiving thiazides or propranolol without loss of control of the anti-hypertensive action of these drugs but it is recommended that individuals are monitored for possible weight gain or an increase in diastolic blood pressure.

Familial versus sporadic longevity and MHC markers.

We have studied the polymorphism of HLA-A, B, in 2 elderly populations (> or = 90 years) compared to a control series of 429 healthy unrelated individuals less advanced in age. The aged population issued from the CHRONOS cohort consisted of 336 centenarians without familial history of longevity, and 102 nonagenarians index cases randomly selected from families. Almost all individuals (310) were previously typed for HLA-DRB1. The increased allelic frequency of HLA DR11 was observed in familial nonagenarians (18.3%) compared to controls (10%) (p < .001) and to sporadic centenarians (11.8%). Concerning HLA Class I alleles, only rare alleles (A30, B70) remain significantly different from the controls after correction of the p value. No distortion of the Mendelian sharing of haplotypes was observed among sibling pairs of familial nonagenarians. A protective effect of the HLA-DR11 molecule itself, presenting adequately immunogenic-infectious peptides, is probable rather than genes in disequilibrium. Our study strongly supports the heterogeneity of longevity, the association of HLA-DR11 in its familial form in aged populations.

Computerized system for the selection of the most HLA-compatible kidney recipient (France-Transplant).

The RITRAN program is applied to renal transplantation and operates within the framework of France Transplant. It makes it possible to choose the most compatible kidney recipient within the ABO blood group system and within the major histocompatibility system in man (HLA). The file presently comprises the data stored on tape concerning 1800 dialysis patients who are awaiting grafts. It is managed by UNIVAC 1108 computer. More than 25 medical-surgical transplant teams from France, Switzerland, Holland and Spain are connected with the computer. It was five years ago that the first interrogation system began operating in real time 20 hours out of 24, using a telex line, and it continues to provide complete satisfaction. The updating of the data is done by the RITREC system, also using a telex machine. An adaptation of the programs applied for choosing blood and platelet donors is in the process of being realized.
