Ultrasound contributions to the management of the severely isoimmunized fetus.

Twenty-four out of 81 fetuses affected by anti-D isoimmunization underwent ultrasonic guided intrauterine transfusions (2.8 I. U. T.s per fetus). The absolute value and trend of delta OD 450 micron value was correlated with the severity of fetal condition as evaluated by ultrasonography following simple semiquantitative grading of ascites (mild, moderate, severe) and diagnosis of hydrops. The evaluation of disease was monitored in this way during transfusion therapy. Transfusion procedures have been ultrasonically guided. When ascites was present a few milliliters of isolytic solution allowed the bubbling effect to be observed. In the case of no ascites we confirmed the needle positioning by a cineradiographic sequence lasting a few seconds. Fetal transfusions were repeated every 10 to 15 days and the amount of packed red cells to be injected was determined according to the week of gestation. Post-transfusion monitoring included ultrasonic reevaluation of fetal parameters and non-stress testing. All fetuses were delivered via cesarean section before the 35th week of gestation. In no case was treatment started after the 31st week. Seventeen fetuses were transfused before the 26th week (71%). In 13 fetuses transfusions were started before ascites had appeared. Only 5 fetuses worsened and the 3 which became hydropic eventually died. The delta OD-450 value of these 5 cases before therapy had already indicated that they were more severely affected. Survival rate in this group was 69%. Eleven fetuses showed different degrees of ascites or hydrops at the time of the first transfusion.(ABSTRACT TRUNCATED AT 250 WORDS)

Correlation between fetal and maternal serum bile acid concentrations.

Serum concentrations of different bile acids (BA) were determined by radioimmunoassay in 56 human fetuses and mothers. Serum was obtained immediately after legal abortion, performed between the 14th and the 21st wk of gestation. Conjugated cholic (CCA) and chenodeoxycholic acid (CCDCA) concentrations were determined in 33 cases, conjugated lithocholic (CLCA) and deoxycholic acid (CDCA) in 20, and sulfolithocholyglycine (SLCG) in 15. In fetal blood, mean concentrations of CCA (0.80 +/- 0.40 mumol/liter), CCDCA (4.50 +/- 2.70 mumol/liter), and CLCA (1.70 +/- 1.04 mumol/liter) were significantly higher than those in the mother (CCA 0.34 +/- 0.17 mumol/liter; CCDCA 0.79 +/- 0.34 mumol/liter; CLCA: 0.70 +/- 0.30 mumol/liter; p less than 0.001); fetal serum levels of CDCA (0.46 +/- 0.32 mumol/liter) and SLCG (0.15 +/- 0.09 mumol/liter) were lower than in the mothers (CDCA 1.20 +/- 0.80 mumol/liter, p less than 0.001; SLCG 0.40 +/- 0.30 mumol/liter, p less than 0.01). There was no correlation between levels of BA and gestational age. Serum total protein and albumin concentrations were both reduced in 10 fetuses as compared with the mothers. These data support the concept of a state of physiologic cholestasis during development and suggest that placental transfer of primary BA occurs mostly in the fetal to maternal direction. This transfer could be facilitated by the reduced fetal plasma albumin concentration, since BA in free solution diffuse more easily through the placenta. There is evidence of lithocholic acid synthesis in the fetal liver, while deoxycholic acid appears to be mostly of maternal origin. Finally, sulfation of BA is poorly developed at this age of gestation.

Detection of the common acute lymphoblastic leukaemia antigen (CALLA) on B cells from human fetal tissues. A multiple phenotypic characterization.

Fetal bone marrow liver and spleen of gestational age 15-20 weeks contain CALLA+, HLA-DR+ lymphoid cells. We show that a proportion of them expresses surface membrane immunoglobulins (SIg) as well as B cell differentiation antigens. A multiple phenotypic analysis reveals that CALLA+ fetal B cells are: HLA-DR+, SIg+, FMC8+, BA1+, Y29.55+ or Y29.55-, B2+, TdT-. Tissue specific phenotypic differences concern the expression of B7 and HLA-DC on spleen but not on bone marrow B cells. This study indicates that the distribution of the CALL antigen across the B cell committed lineage is much wider in fetal than neonatal life since CALLA+B cells have not yet been detected in bone marrow and peripheral blood of normal infants and adults. In addition, the interpretation of our phenotypic data suggest that fetal bone marrow B cells are more immature than those present in the spleen, thus, further supporting the evidence that the bone marrow is the organ of B cell lymphopoiesis.

Cadmium concentrations in blood and urine of pregnant women at delivery and their offspring.

The levels of cadmium in blood (CdB) and in urine (CdU) were measured in 40 women at delivery and in their offspring. The women were not occupationally exposed to cadmium. The CdB levels in the pregnant women were significantly lower than in a control group consisting of 40 subjects of the same sex and age living in the same area. The difference can probably be ascribed to the physiological hemodilution that takes place in pregnancy. The CdU levels in the two groups were identical, which suggests that no mobilization of the metal from tissue deposits occurs during pregnancy. The presence of cadmium in the blood and urine of the newborn demonstrates that cadmium crosses the placental barrier. There was a rather high correlation between the CdU levels of mothers and offspring. Since only traces of cadmium are present in tissues at birth, it is speculated that the CdU levels in the newborn are influenced by the tissue deposits of cadmium in the mother. Further research is required to ascertain whether, in the case of women occupationally exposed to cadmium or living in cadmium polluted areas, transfer of the metal across the placenta can produce toxic effects in the foetus.

Does the guanine nucleotide regulatory protein Ni mediate progesterone inhibition of Xenopus oocyte adenylate cyclase?

In Xenopus laevis oocytes progesterone is able to inhibit directly the plasma membrane adenylate cyclase activity and induce reinitiation of meiotic maturation. To determine whether progesterone inhibition is mediated by the inhibitory guanine nucleotide-binding regulatory component of adenylate cyclase, Ni, the effect of the Bordetella pertussis toxin (IAP) and limited proteolysis on progesterone action in oocytes was investigated. Treatment of oocyte membranes with islet activating protein (IAP) in the presence of [32P]NAD led to incorporation of radiolabel into a 41 000-dalton membrane protein. However, exposure of isolated oocytes to 100 ng/ml IAP for up to 24 h, or oocyte membranes with concentrations of toxin as high as 100 micrograms/ml, had no effect on either progesterone inhibition of adenylate cyclase or induction of maturation. Similarly, limited alpha-chymotrypsin proteolysis of oocyte membranes failed to modify progesterone-induced inhibition of adenylate cyclase. In contrast, inhibition of human platelet adenylate cyclase by epinephrine, acting via a GTP-dependent, alpha 2-adrenergic receptor-mediated pathway, is almost completely abolished by both IAP treatment and limited proteolysis of platelet membranes. These data indicate that unlike attenuation of platelet enzyme activity, the inhibition of adenylate cyclase in oocyte membranes by progesterone does not occur via a classical Ni-mediated pathway.

Phenotype of the terminal transferase-positive cells in human foetal liver and bone-marrow: analysis with monoclonal antibodies.

Liver and bone-marrow of 20 human foetuses between 15th and 20th week of gestational age were examined. Mononuclear cells were labelled with murine monoclonal antibodies to reveal surface antigens and with a rabbit antiserum to TdT for the detection of the nuclear enzyme, using a double colour indirect immunofluorescence technique. The results have revealed that TdT+ cells express the phenotype of B cell precursors HLA-DR+, CALLA+, FMC8+, LEU-1-, LEU-5-, LEU-9-. It suggests that foetal liver and bone-marrow have an active function in B-lymphopoiesis in humans. The involvement of TdT in the process of B-cell immunological acquisition is uncertain because only humans and Xenopus embryos, among the species examined, express TdT outside the thymus during ontogenesis.

Terminal deoxynucleotidyl transferase (TdT) in human foetuses. An immunofluorescent and biochemical study.

The liver, bone marrow and cord blood of 30 human foetuses between the 15th and 21st weeks of gestational age were examined. Liver suspensions were investigated both by immunofluorescent and biochemical assay; cord blood suspensions and bone marrow touches with immunofluorescent assay. The results indicate the existence of a few TdT+ cells in the liver, cord blood and bone marrow of human foetuses in the ages studied. The peaks for each organ were: 15 +/- 1.60 (mean +/- SD) TdT+ cells in the liver, 12 +/- 2 in the bone marrow and 18 +/- 2.83 in the cord blood. In all the organs TdT+ cells decrease in the later weeks. In the liver, the results of the immunofluorescent assay were confirmed by the biochemical activity.

[Comparative effects of an extract of Anguilla Stannius corpuscles and of an active fragment of human parathyroid hormone (1-34 hPTH) on an anuran batracian, Xenopus laevis].

Parathyrin of the corpuscles of Stannius (PCS), glands which are restricted to Holostei and Teleostei, resembles mammalian parathyroid hormone secreted by the parathyroids. These glands made their first phylogenic appearance with Amphibians, whereas the Stannius corpuscles disappeared. We have compared, in the clawed toad, Xenopus laevis, the effects of injections of extracts of corpuscles of Stannius and of the active synthetic fragment of parathyrin (1-34 hPTH). Both hormones elicited hypocalcemia and hypophosphatemia within a 4 days treatment. These results are interpreted according to the hypothesis that in the most primitive Amphibians there are receptors leading to hypocalcemic effect co-existing with the appearance of "new" glands which secrete a hypercalcemic hormone in mammals.

[Demonstration of maturation promoting factor activity in Saccharomyces cerevisiae].

Microinjection of a crude yeast extract into fully grown immature Xenopus laevis oocytes induces the reinitiation of meiotic maturation as does Xenopus laevis oocyte MPF (Maturation Promoting Factor) by itself. Using several strains of yeast exhibiting temperature sensitive cell cycle block (cdc), it was observed that yeast extract exhibits MPF activity only when obtained during the time interval from the G2 period to the metaphase. This observation stresses eucaryotic ubiquity of MPF.

[Value of the use of tissue adhesive (Braun Histoacryl) and its importance as a safety measure in difficult anastomoses].

In an experimental and then clinical investigation, the intention has been to demonstrate that tissue adhesive (Histoacryl Braun) is a useful safety factor in a wide variety of difficult intestinal sutures, in both emergency surgery and in that of choice. Experiments were carried out on about a hundred rabbits which received colic anastomosis, after clean, complete section, anastomosis of the viscera, with four cardinal sutures in 000 silk and the remaining suture of a diameter of about 3 cm with adhesive. This stage having been surmounted with excellent results, work continued with the experiment in emergency surgery and surgery of choice, on the basis of the criterion of greater safety as a complement of the traditional high dehiscence risk suture technique. The clinical results obtained thus far have been encouraging enough to suggest its experiments.

[Experimental study of the use of tissue adhesives in colonic surgery].

The authors report the own experience about the experimental use of tissue adhesive in the colonic surgery, on 14 rabbits, with quite satisfactory result. Therefore they evince that this tissue adhesive, althought it can't replace the surgical suture, yet it can help the surgeon out of a hard colonic anastomosis.
