Hemorrhage from myocardial revascularization.

Four groups of 12 dogs each had ligation of the left anterior descending coronary artery (LAD) with subsequent release of the ligature and confirmed reperfusion. After 24 hours of reperfusion, the hearts were removed. sliced at 1 cm intervals along the transverse axis, and stained with triphenyltetrazolium chloride. Measurements of the infarct size as a percentage of the left ventricular area (I) and of hemorrhage as a percentage of infarct size (H) were made. The duration of ligation was 3 hours in Group 1, 6 hours in Group II, 18 hours in Group III, and 30 hours in Group IV. No significant difference in infarct size was found among the groups. Percent hemorrhage was 25.2% of infarct area in Group I, 28.3% in Group II, 18.1% in Group II, and 0.7% in Group IV. If reperfusion hemorrhage into an acute myocardial infarct is deleterious, these data suggest than danger to be decreased at 18 hours and absent at 30 hours after acute coronary occlusion.

Studies on the role of sulfhydryls in the myosin ATPase. Characterization of the site of modification by the bifunctional sulfhydryl reagent p-N,N'-phenylenedimaleimide.

Myosin has been modified with near stoichiometric amounts of the bifunctional reagent [14C]p-N,N'-phenylenedimaleimide (pPDM) in the presence of MgADP under conditions which abolish its ATPase activity. Subsequent carboxymethylation and CNBr cleavage results in the 14C label being associated with a single polypeptide of Mr approximately 10,000. Amino acid composition and partial sequence analysis of this peptide showed that it corresponded to the peptide containing -SH1 and -SH2 sequenced by Elzinga and Collins (Elzinga, M., and Collins, J.H. (1977) Proc. Natl. Acad. Sci. U.S.A. 74, 4281-4284) and to the peptide labeled at -SH1 or -SH2 by N-ethylmaleimide by Kunz et al. (Kunz, P.A., Walser, J.T., Watterson, J.G., and Schaub, M.C. (1977) FEBS Lett. 83, 137-140). These data indicating that pPDM does label the -SH1- and -SH2-containing region in myosin by covalently bridging them and shows that in the presence of MgADP these thiols can approach to within 12 to 14 A.

Dietary intakes, resting metabolic rates, and body composition in benign and malignant gastrointestinal disease.

Dietary protein and energy intakes were assessed in 42 patients with cancer and 24 with benign conditions of the gastrointestinal tract. The relations of dietary intake to body composition was examined. Resulting metabolic rate was measured in 51 patients. No significant differences in dietary intake or metabolic rate were found between patients with cancer and those with benign disease. There were significant positive correlations between protein and energy intakes and the ratio of total body potassium to total body water in patients with benign disease but not in those with cancer. Weight loss was probably due to inadequate food intake, the main defect being energy deficiency, since protein intake was usually well maintained. Supplementing with energy the voluntary ingested diet of patients with cancer would probably prevent weight loss in most cases.

Psychophysiological differentiation between psychopathic and schizophrenic abnormal offenders.

This paper is a follow-up of research by Schalling (1978), Mednick (1975), Mednick & Hutchings (1978), Venables (1975) and a pilot study on the psychophysiological response characteristics of maximum security patients, Hinton & O'Neill (1976, 1978). In line with hypotheses, patients rated as 'disoriented' by nurses and diagnosed 'schizophrenic', tended to have increased spontaneous electrodermal fluctuations and reduced orienting response recovery time relative to non-'disoriented' and diagnosed 'psychopaths'. Also low rate spontaneous fluctuation in skin resistance plus long skin resistance ORt/2 differentiated 'high public risk' psychopaths from 'low risk' domestic offenders.

Infusion cholecystography in the diagnosis of acute cholecystitis.

The use of infusion cholecystography as an aid in the diagnosis of acute cholecystitis was investigated in 21 patients. Seventeen of 18 patients (94 per cent) with positive cholecyst-tomograms who underwent laparotomy had confirmation of acutely inflamed gallbladders both macroscopically and histologically. These findings suggest that infusion cholecystography can make a significant contribution in reducing the incidence of misdiagnosis in acute cholecystitis, and that the investigation should be part of the management of patients in whom early surgery is planned.

Gastric reduction for morbid obesity: technique and indications.

The results of gastric reduction for obesity in 12 patients are described. Emphasis is laid on preoperative weight loss, using jaw wiring in 9 patients. The successful outcome of the operation was found to depend on the formation of a small gastric pouch and a narrow gastro-jejunal anastomosis. A constant-sized pouch was achieved by forming the pouch around a 100-ml silicone balloon devised for the purpose. Postoperative results were good in 7 patients, partial success was achieved in 3 patients and there were 2 failures. It is concluded that the procedure will produce successful results provided that a small proximal pouch with a narrow outlet is fashioned in motivated and psychologically well-balanced patients.

Detection of antibody-forming cells and humoral antibody to horseradish peroxidase.

Two new simple methods for detecting antibody-forming cells by hemolytic plaque assay and hemagglutinating antibody to horseradish peroxidase have been developed in mice. Both techniques utilize as target, sheep erythrocytes coupled directly with horseradish peroxidase. These assays are sensitive, antigen-specific and are useful to quantitate both direct and indirect antibody-forming cells and humoral antibodies.

Home parenteral nutrition.

One hundred and six patients were placed on a home parenteral nutrition program because of severe gastrointestinal tract lesions. In 41, sufficient improvement allowed the resumption of oral alimentation. Forty-eight remain on the program. Seventeen, including ten with malignant disease, died from causes not related to home parenteral nutrition. All patients achieved and maintained the appropriate weight for age and body build. In the pediatric patients, normal or accelerated linear growth occurred. Complications included sepsis, 18 episodes in 12 patients; local infection of the catheter, 14 in six; catheter thrombosis, six in five; ketoacidosis, one in one; contaminated solutions, one in one, and essential fatty acid deficiency, one in one. Home parenteral nutrition is a relatively safe and effective alternative for long term nutritional support.

The relationship of personality style to abstract thinking deficits in schizophrenia.

Hypothesized that the correlation between Inability to Abstract and Autism measures of schizophrenics would be larger after the effects of relevant personality variables were partialed out than before. The multiple correlations of these deficit measures with 15 of the Sixteen Personality Factor scales and a measure of verbosity were determined in a sample of 100 schizophrenics. The correlations between Inability to Abstract and Autism before and after those scales that contributed significantly to the Rs had been partialed out also were calculated. Neither was significant. The results offer no support for the view that observed differences between schizophrenic subgroups in inability to abstract and autism reflect variations in personality style rather than cognitive factors.

Quantitative assessment of the quality of medical care provided in five family practices before and after attachment of a family practice nurse.

The standards of patient care were maintained in five urban medical practices after the introduction of family practice nurses. Evaluations were achieved before and after their appointment by the indicator condition method. Minimal explicit criteria for the management of patients with 12 indicator conditions and by the use of 14 drugs were approved by an ad hoc peer group of community physicians. These cirteria were applied to the five practices by the use of a single-blind design and the abstraction of unaltered medical records. A standardized score for each practic e permitted comparison of scores for the management of indicator conditions and for the clinical use of drugs before and after attachment of the family practice nurses. For each of the indicator conditions and the drugs assessed in the five practices similar levels of adequacy were observed in the two study periods. These explicit (objective) audit resutls agreed with the implicit (subjective) assessments of the family practice nurses by their physician colleagues.

Effect of nucleotide binding on the proximity of the essential sulfhydryl groups of myosin. Chemical probing of movement of residues during conformational transitions.

The reaction of myosin with three bifunctional sulfhydryl reagents of differing cross-linking span is reported. In the absence of nucleotide only p-N,N'-phenylenedimaleimide with a cross-linking span of 12-14 A can bridge between the two essential sulfhydryls of myosin. The other two reagents, 2,4-dinitro-1,5-difluorobenzene and 4,4'-difluoro-3,3'-dinitrodiphenyl sulfone with cross-linking spans of 3-5 and 7-10 A, respectively, react under identical conditions with the SH1 sulfhydryl but do not bridge to the SH2 group. In the presence of MgADP, both p-N,N'-phenylenedimaleimide and 4,4'-difluoro-3,3'-dinitrodiphenyl sulfone bridge across the SH1 and SH2 groups indicating a closer proximity of these two sulfhydryls in the presence of bound nucleotide. These results are discussed in relation to the conformational change induced in myosin by binding of the nucleotide.

Reactivity of essential thiols of myosin. Chemical probes of the activated state.

14C-Labeled fluorodinitrobenzene and N-ethylmaleimide have been used as chemical probes of the conformational states of myosin induced by the binding of MgADP and MgATP. The results indicate that in the high-energy conformation, MMgADP-Pi, the essential thiols are protected from modification but their diminished reactivity does not result from depletion of the reagent by reaction at nonessential thiols. The binding of MgADP to myosin exposes the essential thiols as reflected by an increased rate of their modification. The influence of the divalent cations Mg2+ and Ca2+ on the conformation of the M species has also been investigated. By monitoring the incorporation of fluorodinitrobenzene, the conformations of the M state in the presence of these cations can be clearly discerned.
