Multihospital systems: implications for physicians.

This overview of the movement toward multihospital systems and shared services arrangements and its implications for physicians includes a description of the major types of arrangements formed so far, an assessment of their performance, a summary of responses to the new systems, a discussion of their advantages and disadvantages, and a brief outlook on the future of such systems.

How to manage adult respiratory distress syndrome.

The key clinical features of adult respiratory distress syndrome are increasing dyspnea, tachypnea, and work of breathing; diffuse pulmonary infiltrations on chest radiographs; severe hypoxemia; and absence of a classic diagnosis. Adequate tissue oxygenation is the cornerstone of therapy. Therapeutic modalities include mechanical ventilation, fluid restriction, diuretics, and cardiotonic and vasopressor agents.

Ultrasonographic measurement of fetal growth parameters for estimation of gestational age and fetal weight.

The present study was undertaken to establish accurate, convenient, simple and rapid ultrasonographic measurements of fetal parameters and their correlation with gestational age and fetal weight. Relationship with gestational age was studied in 60 fetuses from 348 measurements of the biparietal diameter (BPD) and was also correlated with eight other parameters. There is no doubt that the BPD is the most reliable parameter for estimation of gestational age. In 34 infants, 306 measurements of nine fetal parameters taken within four days before delivery were correlated with their birth weights. Abdominal area gave the highest correlation, compared with other parameters or their combinations. Since combined measurements of several parameters gave only a slightly higher multiple correlation (R = 0.89) with fetal weight than the measurement of abdominal area alone (r = 0.85), their use is not justified as a routine procedure.

Studies on the nature of protease-induced growth stimulation in normal and transformed BHK cells.

In the presence of growth-limiting serum concentrations trypsin displays mitogenic activity on actively-growing but not quiescent BHK cells. These results suggest that BHK cells arrested in G1 (G0) are not sensitive to protease-induced growth stimulation. Previous work strongly suggested that the trypsin active-site is not directly involved in its mitogenic activity on BHK cells. Additional studies on denatured trypsin fragments further indicate that the molecular conformation and size of native trypsin may not be absolutely required for mitogenic activity. Cellular multiplication induced by the addition of fresh serum to quiescent BHK cultures is not inhibited by high concentrations of soybean trypsin inhibitor. Similar to our previous findings with trypsin, it has been further observed that plasmin is not sufficient to initiate the growth of BHK cells in soft agar. Trypsin also fails to enhance the growth of a thermosensitive polyoma-transformed BHK line in soft agar at the restrictive temperature. Finally, the growth of transformed BHK cells in soft agar does not display a requirement for plasminogen and is not inhibited by soybean trypsin inhibitor. These studies argue against the involvement of plasmin or other exogenous trypsin-like enzymes in the growth and transformation of BHK cells.

Second primaries in cases of cancer of the larynx.

The incidence of second primaries in 1,600 patients treated with radiotherapy for cancer of the larynx is reviewed. Carcinoma of the lung is the most frequent second primary with a significantly higher incidence in supraglottic laryngeal cancer. Because the follow-up of this irradiated population is limited it is not yet possible to estimate the risks of radiation-induced cancer in the entire life span following radiation treatment of cancer of the larynx.

The effect on the bladder pressure of sudden entry of fluid into the posterior urethra.

The posterior urethra was stimulated by the rapid injection of saline against the urethral wall. The effect was studied in 50 female patients attending a urodynamic clinic. The injection was made via a double lumen catheter, and bladder, urethral and rectal pressures were observed during and after the ingress of fluid. The results indicate that sudden entry of fluid into the posterior urethra does not initiate a detrusor contraction even in those patients in whom the injection of fluid is painful. The relevance of these results in relation to the unstable bladder and urodynamic tests is discussed.

Plaque assay of nuclear polyhedrosis viruses in cell culture.

The nuclear polyhedrosis virus of Autographa californica has been titrated in Spodoptera frugiperda cells by the plaque method, using a solid overlay which does not require either the use of modified culture medium or expensive purified agarose or the addition of culture medium as a liquid layer above the solid agarose. This assay is more sensitive than that using a viscous methyl cellulose overlay but less sensitive than the end-point dilution technique. Neither Trichoplusia ni nor Bombyx mori cells were satisfactory as indicators for the assay as described, since they failed to form a stable monolayer. Manduca sexta cells could be utilized for assay of A. californica nuclear polyhedrosis virus, but the sensitivity was lower than with S. frugiperda cells.
