Using a binding assay for sex hormone binding globulin, a Japanese group showed that they could select breast cancer patients more likely to be responsive to endocrine therapy, i.e. oestrogen receptor positive patients, by an increase in oestrogen binding capacity of the protein. We directly measured the levels of this protein using a specific antiserum to it, and the levels were not different from normal in any group of breast cancer patients whether or not oestrogen receptors were present. Both methods of measurement can detect a slight increase after a weak oestrogenic stimulus, indicating that they are as sensitive as each other.

Suprascapular nerve injuries at the spinoglenoid notch are uncommon. The true incidence of this lesion is unknown; however, it appears to be more common in athletes who participate in sports involving overhead activities. When a patient is being evaluated for posterior shoulder pain and infraspinatus muscle weakness, electrodiagnostic studies are an essential part of the evaluation. Electromyography will identify an injury to the suprascapular nerve as well as assist in localizing the site of injury. In addition, imaging studies are also indicated to help exclude other diagnoses that can mimic a suprascapular nerve injury. The initial management should consist of cessation of the aggravating activity along with an organized shoulder rehabilitation program. If the patient fails to improve with 6 months to 1 year of nonoperative management, surgical exploration of the suprascapular nerve should be considered. Release of the spinoglenoid ligament with resultant suprascapular nerve decompression may result in relief of pain and a return of normal shoulder function.

1. Irritability may be caused by psychologic factors, physiologic factors, external factors, or a combination of factors. 2. If the nurse reacts to another individual's irritability by feeling inadequate, becoming irritable, or any other number of nontherapeutic responses, emotions may be escalated, interpersonal relationships may be irretrievably damaged, and productivity on the job may be diminished. 3. A therapeutic response to irritability can have a steadying effect and lead to a peaceful resolution; appropriate response by the nurse to facilitate positive communication, provide support, and promote effective resolution of problems can assist in pacifying tense situations when irritability becomes a problem.
Events are managed and people are led. In the people-oriented society of today, good managers must also be good leaders. A manager who ignores using techniques that focus on the needs of employees will soon find a large gap between what the leader desires and what the employees will do. An approach that communicates positive regard for others, enthusiastic role modeling, participative management, and a vision that is constantly held imminent will enable the nurse to lead.
Delegation can be an effective tool for the OR manager in completing work activities in a more timely manner. A manager with skill in delegation can make the work environment a more positive and enjoyable place in which to work, while at the same time increasing productivity.
One of the most difficult aspects of leading a discussion and trying to facilitate group problem solving is restraining domineering group members. A group leader who cannot control aggressive, talkative individuals will soon have a group where members are not only angry and frustrated, but also who will make excuses to be excluded or who will leave the group.
