Investigating the MESA (multipoint electrotactile speech aid): the transmission of connected discourse.

Three normal-hearing young adults were tested in their reception of connected discourse materials under two receiving conditions; visual-reception alone (lipreading) and visual reception in conjunction with an electrotactile speech aid (MESA). Subjects were artificially deafened with earplugs and white noise; all stimuli were delivered live-voice using a special tracking procedure. Analysis of the data suggests that after an initial period of learning, combined visual and electrotactile receptive performance exceeds lipreading-alone performance. After extensive learning however, performance in lipreading alone or MESA plus lipreading is practically equivalent.

[Severe mechanical haemolysis after correction of a partial atrioventricular canal (author's transl)].

Following correction of a partial atrioventricular canal and closure of the atrial septum defect with a Dacron patch a severe hemolytic anemia developed caused by moderate mitral insufficiency. The clinical postoperative course and the successful treatment by reoperation is described in detail. The Dacron patch was excised, the mitral cleft was closed by four single sutures, and the septal defect was covered with a pericardial patch. In the literature ten other communications were gathered reporting hemolytic anemias following correction of endocardial cushion defects with Dacron, Teflon, or Ivalon patches. Three of six reoperated patients died postoperatively. A woman died after correction with renal failure caused by severe hemolysis. In four patients hemolytic anemia was compensated and there was no need for reoperation. In consideration of our own experience and those reported in the literature we recommend pericardium instead of synthetics for closure of atrial septum defects of Foramen-primum-type.

[Risk of psychotropic drug potentiation by dipropylacetamide (16 cases)].

We report 16 cases in which there is a dangerous potentialization of psychotropic drugs with dipropylacetamide, and compare them with 19 cases possible to control, that dipropylacetamide used alone, or in another association, was well tolerated. We pointed out the importance of a depressive evolution, and of dangerous associations, mainly with tricyclic compounds, lorazepam and 5-hydroxytryptamine. Clinical signs are described; they consist in a fast evolutive syndrome, which is the same that "syndrome malin of psychotrops" with neurovegetative signs and central fever, or ingravescent wakefulness disorders and spasmodic cerebellar signs. The importance for the diagnosis and the prognosis of E.E.G. data is pointed out; plasma levels determination does not seem to give important data. Drugs interactions studies are very important in the clinical evaluation of psychotrops. Several drugs may interact with dipropylacetamide. This fact sustains some hypothesises about the action site of the drug.

[Psychological treatment of transsexualism and sexual identity disorders some recent attempts].

Psychological treatments of transsexualism and sex gender Dysphoria syndroms. Recent data. There are generally two types of patients requesting sex change operations: one type is the classical but rare transsexual, the others belong to the larger more vague group of sex gender Dysphoria. These syndroms have been felt to be unresponsive to psychotherapy. We review here some recent attempts of psychological treatments which claim to improve satisfaction with biological sex gender identity and result in renonciation of the request of sex reassignment surgery (S.R.S.). These psychotherapies use various methods: Behavior therapy, psychoanalytically-oriented psychotherapy, religious exorcism and conversion. The reports reviewed suggest that sex gender identity may be more malleable than previously thought in transsexual syndrom, at least in relatively youthful subjects. Cases of recent onset of the syndrom which result from a crisis cases underline the necessity for careful evaluation and attempted psychological treatment before undertaking irreversible surgical procedures.

[Disinhibition and its risks].

The authors point out clinically the risks of desinhibition in psychiatry. The classical risk of suicide by suppression of inhibition with melancholic patients is well known and prevented. The main risk for neurotic patients is that of trying to get desinhibition by means of toxicomanic behavior essentially though alcoholism. As far as psychoses are concerned, the risk seems to be mainly that the psychiatrist might misunderstand the concept of desinhibition and make a wrong utilization of desinhibitory treatments. At last, the "institutional" desinhibition constitutes a misunderstanding for psychopathic personnalities.

Vacuolated lymphocytes in type II glycogenosis--a diagnostic approach?

Using electron microscopy, glycogen-filled lysosomes were found in peripheral lymphocytes in 5 cases of the infantile form of glycogenosis type II. In two infants whose blood smears were available, the ultrastructural demonstration of this pathognomonic storage corresponded to well-delineated vacuoles detected by routine light microscopy. Detection of such vacuoles in peripheral lymphocytes by light microscopy and demonstration of glycogen-filled lysosomes by electron microscopy could be a simple and harmless tool for diagnosing the classical form of type II glycogenosis.
