[Heterophil antibody negative infectious mononucleosis due to Epstein-Barr virus. A clinical, hematological and serological study of 14 cases (author's transl)].

Fourteen patients admitted to the hospital for heterophil antibody negative infectious mononucleosis (IM) were studied; the causative agent was Epstein-Barr virus, as confirmed by the presence of anti-VCA antibodies (IgG and IgM) and the absence of anti-EBNA antibodies. The patients were from 2 to 69 years old, 7 being less than 10 years old while 4 were more than 45 years old. In all cases, asthenia and adenopathy were found and a deterioration in liver functions was present whenever it was looked for; jaundice occurred in 3 patients. In 6 cases fever persisted for several weeks. Leukocytosis and a high level of mononuclear cells were present in 8 patients and atypical lymphocytes in 10. Anti-EA titers were found to be high in the severe forms of illness. Clinical and laboratory features of these 14 heterophil negative IM patients were compared with a group of 21 heterophil positive IM patients studied under the same conditions; the most striking differences between the two groups were the frequency of patients under 10 and over 45 years old in the heterophil negative group, the frequency of abnormal liver function tests and of persistent fever, and a lower frequency of atypical lymphocytes.

[Erythroblastopenia in a child with immune deficiency and hyper IgM].

The authors report the case of a boy with a history of recurrent infections, who presented with erythroblastopenia at age 5 years. An immune deficiency with hyper IgM was found. Erythroblastopenia appeared to be related to a serum inhibitor of erythropoiesis. This inhibitor was present in the serum IgM fraction and acts by blocking the erythropoietic activity of a soluble substance in the serum, which is not erythropoietin but which could be a cofactor of this hormone. A series of 5 plasma exchanges, performed in 10 days, permitted to clear the inhibitor and induced remission, still persisting after a 12 month follow-up.

[Severe bronchoalveolitis in chickenpox].

Varicella pneumonia, is commonly an easy diagnostic because of specific eruption. We report a case in an 8 years old child with moderately with-spread macular and purpuric rash. Hematologic data showed anemia and thrombocytopenia from central origin. Surgical biopsy of lung was performed with bacteriologic, parasitologic and virologic studies. The diagnostic was brought by ascension of serum complement fixation titers to varicella virus from 1/8 to 1/1024. This case is compared to those recorded in the literature.

[A case of acute reversible dysautonomia in an adolescent].

A 16 years old male presented with orthostatic hypotension with constant pulse, associated with severe constipation, decrease of sudoral secretions, weight loss and asthenia. This dysautonomia evolued during a period of 6 months and disappeared completely leaving no sequellaes. The symptoms appeared after a rubella. The authors discuss the originality of this observation among the pure pan-dysautonomia and the modes of exploration of the autonomic nervous system. Its mechanisms is probably auto-immune.

[Hemoglobin abnormalities and glucose 6 phosphate dehydrogenase deficiency (G6PD) in 101 refugees from southeast Asia].

Hematological studies were performed systematically in 101 refugees from South-Eastern Asia living in the Grenoble area at the present time. We found: --4 haemoglobin E homozygosity, --29 haemoglobin E heterozygosity, --2 beta-thalassemia heterozygosity, --4 alpha-thalassemia heterozygosity, --14 glucose 6 phosphate dehydrogenase deficiencies.
