[Use of harmonic echocardiographic contrast imaging and intravenous bolus of Levovist for evaluation of the left ventricle].

Harmonic imaging is a new imaging modality using nonlinear acoustic response, which is particularly sensitive for the particles of contrast agents. Our study was designed to compare the potential of harmonic echocardiographic imaging of the left ventricle using a contrast agent, Levovist to improve the detection of endocardium in patients with suboptimal image quality. 40 patients were studied using standard transthoracic apical views of the left ventricle patients using fundamental frequency and second harmonic frequency after and intravenous injection of 2.5 g Levovist. The quality of endocardial delineation in 16 standard segments was scored from 0 to 2. Endocardial visualization index was calculated as a mean of the scores to express overall diagnostic quality. Harmonic imaging with contrast significantly improved left ventricular endocardial border detection (endocardial visualization index at baseline 1.24 +/- 0.41, with contrast 1.63 +/- 0.38; p < 0.001). The improvement was qualitatively observed in all parts of the left ventricle: in apex (2.4 +/- 0.8), in the middle part (2.5 +/- 0.9) and slightly less in the basal part (2.1 +/- 1.1) as scored on a 0-3 scale. The number of invisible segments decreased from 124 (fundamental) to 50 in contrast harmonic mode. The persistence of the contrast enhancement, prolonged in harmonic as compared to fundamental imaging (284 +/- 136s vs 117 +/- 87s; p < 0.001) enabled convenient recording of all necessary views. Harmonic imaging after an intravenous injection of Levovist significantly improves the visualization of left ventricular endocardial border. Prolonged contrast effect after a single bolus enhances the pertinence of the method in clinical practice.

[Results of the treatment of peripheral arterial diseases with PGE1].

PGE1 (Prostin VR by Upjohn) was administered in the form of intra-arterial infusion to the patients with ischaemic disease of the limbs. The investigations aimed at collecting additional data contributing to the elaboration of optimal administration of the drug, its dosage and objective assessment of the obtained results. The obtained results have shown that the therapeutic efficiency of PGE1 depends on the advancement of ischaemia. Favourable clinical result was achieved in 45% of the patients manifested by the decrease of pain and healing rate. Such an effect persisted 2-7 months. An increase in serum total proteins and fibrinogen fraction and a decrease in platelet aggregation was seen during the PGE1 infusion. Skin temperature measurements and radioisotope examination with perfusion scintigraphy have shown an increase in blood flow in both muscles and other tissues in the treated patients. The obtained results confirmed a value of intra-arterial PGE1 infusion in the treatment of ischaemic limb diseases.

[Rheographic evaluation of the changes in blood flow after superficial electric stimulation in patients with Raynaud's disease or syndrome].

An effect of the superficial electrostimulation on skin microcirculation in the patients with both Raynaud's disease and syndrome was assessed with electro-impedance rheography. Significant increase of blood flow through the stimulated region was seen (mean value 42%). It was also shown that electro-impedance rheography is of value in both quantitative and qualitative investigation comparing Raynaud's disease and syndrome.

Studies on arteriovenous anastomoses in rabbits. Part I. Arteriovenous anastomoses and the effect of sympathectomy on the development of trophic ulceration after sciatic nerve transection in rabbits.

The effect of sympathectomy on the functional state of skin arteriovenous anastomoses (AVA) and on the development of trophic ulceration after sciatic nerve transection was studied in rabbits. The functional state of the AVA (their opening or occlusion) was determined on the basis of gasometric and radiological investigations. The experiments showed that sympathectomy leads to an occlusion of AVA or at least to a marked reduction in blood flow through that part of microcirculation. The improvement in nutritional flow due to the increase in capillary flow and reduction in AVA flow hampers to a certain degree the development of the ulceration and results in transient healing. A hypothesis is presented about different innervation pathways of AVA and precapillary sphincters.
