[An unusual site of bleeding from esophageal varices].

A case of massive upper gastrointestinal haemorrhage from proximal esophageal varices is reported. Upper esophageal varices occur rarely, and serve as collaterals in conditions of superior vena cava obstruction. In our patient, cervical esophageal varices were associated with portal hypertension due to occult liver cirrhosis. As the diagnosis was made at autopsy, this case serves to stress the importance of a through examination of the cervical portion of the esophagus during routine endoscopy.

Cholinergic enhancement by pyridostigmine increases the insulin response to glucose load in obese patients but not in normal subjects.

OBJECTIVE: To further investigate the role, if any, of acetylcholine and the parasympathetic nervous system in modulating beta-cell secretion in man. DESIGN: Oral glucose load (OGTT, 100 g p.o. at 0 min) alone and preceded by pyridostigmine (PD, 120 mg p.o., 60 min before OGTT), a cholinesterase inhibitor, were administered on two different occasions, in random order, two or three days apart. SUBJECTS: Ten women with central obesity (OB, body mass index (BMI): 34.2 +/- 2.1 kg/m2, waist to hip ratio (WHR): 0.83 +/- 0.01, aged 39.0 +/- 5.3y) and six normal women (NS, BMI: 22.7 +/- 1.9 kg/m2, WHR: 0.74 +/- 0.01, aged 37.1 +/- 4.8y) were studied. MEASUREMENTS: Serum insulin, plasma glucose and plasma noradrenaline (NA) were measured at -60, -15 and 0 min, and then every 15 min up to +120 min. Insulin concentrations were measured in duplicate by immunoradiometric assay, glucose by glucose oxidase colorimetric method and NA was assayed after extraction with alumina using high performance liquid chromatography with electrochemical detection. Pulse rate (PR), systolic (SBP) and diastolic blood pressure (DBP) were also measured every 15 min during the tests by an automated cuff device. RESULTS: OGTT raised glucose concentrations in OB and NS (incremental area: 420 +/- 44 vs 288 +/- 70 mmol/l. 2 h, respectively) without significant differences between groups (F = 0.6, P = ns). On the other hand, OB showed an insulin response to OGTT higher than NS (10,120 +/- 1074 vs 6692 +/- 1962 microU ml-1 2 h, respectively P < 0.01). After OGTT alone NA concentrations increased to the same extent in NS (peak vs basal: 1.40 +/- 0.16 vs 1.07 +/- 0.10 nmol/l, P < 0.05) and in OB (peak vs basal: 1.50 +/- 0.14 vs 1.04 +/- 0.18 nmol/l P < 0.05). Both in NS and in OB, PD administration failed to modify basal glucose and insulin (P = ns for both) as well as basal NA concentrations. In NS, the combined administration of PD and OGTT did not modify glucose and insulin responses compared to OGTT alone 335 +/- 65.4 mmol/l. 2h and 6348 +/- 1348 microU ml-1 2h, respectively) while in OB, PD significantly increased the insulin response to OGTT (14640 +/- 3030 microU ml-1 2h, P < 0.03), while the glucose response was not significantly different from OGTT alone (478 +/- 45 mmol/l. 2h). PD administration did not modify the NA response to OGTT, in NS or OB (P = ns). In both groups, pyridostigmine administration did not affect systolic or diastolic blood pressures, but decreased pulse rate to the same extent in NS (74 +/- 2 vs 66 +/- 2 beats/min, P < 0.05) and in OB (72 +/- 1 vs 67 +/- 2 beats/min, P < 0.05). CONCLUSIONS: Our present data indicate that in man, as in animals, acetylcholine has a stimulatory influence on insulin secretion.

Cognitive recovery with abstinence and its relationship to family history for alcoholism.

OBJECTIVE: The relationship between drinking relapse and family history for alcoholism to cognitive functioning was investigated in alcoholics who completed a 28-day inpatient treatment program. METHOD: The current study examined 144 primary alcoholics who completed a battery of cognitive tests on admission to treatment and at a 3-4 month follow-up visit. Participants were classified as either family history positive (FH+) (n = 86) or family history negative (FH-) (n = 58) based on information on alcohol dependence in first degree relatives. The subjects were further classified as drinkers (n = 56) or abstainers (n = 88) at follow-up, based upon information gathered about their drinking practices during the interval. RESULTS: There were no differences between the groups based on family history status at admission to treatment. Comparisons made at follow-up revealed that the abstainers' (both FH+ and FH-) performance improved. The FH+ drinkers' performance was worse than that of both the FH- drinkers and the FH+ abstainers at follow-up. The poor performance of the FH+ drinkers relative to the FH- drinkers was not attributable to differences in drinking consumption during the test-retest interval. CONCLUSIONS: These results suggest that alcoholics who maintain abstinence following treatment show improvement on cognitive testing at follow-up; both FH+ and FH- abstainers showed improvement, indicating that a positive family history of alcoholism does not impede recovery that occurs with abstinence. The most important finding, however, is that the FH+ drinkers show greater deterioration at follow-up compared to the other groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Muscular "modus agendi" and craniomandibular dysfunction.

It is very important to analyze the muscular "modus agendi" in diagnosing and treating dysfunctional problems. By identifying, then changing, the abnormal muscle posture and function, we are able to correct the muscular pathology and to start orofacial orthopedic treatment without the presence of dysfunctional counterforces.

Plasmid analysis in PPNG strains isolated in Italy.

Italian reports of PPNG strains are rare. In this paper we report the plasmidial characteristics of two strains: one harbouring 3.2 and 2.6 Mdal plasmids and the other 24.5, 4.5 and 2.6 Mdal plasmids. Restriction analysis was carried out on plasmids transferred to E. coli by transformation for "Africa" and conjugation for "Asia" plasmid.

Streptococcus mutans: classification in bacteriocin-types.

A sample of S. mutans bacteriocins was studied to obtain a useful outline of strain typing since their synthesis has proved stable and not under plasmidial control. The inhibiting effectiveness against 9 oral streptococci and the sensitivity of mutacins produced by 49 S. mutans strains to heat, chloroform and proteasic activity were evaluated. On the basis of our results the producing strains are classified into five different types. We examine the possibility of obtaining a useful typing with bacteriocins and we discuss the choice of the most suitable number of indicators to arrange the strains in a limited cluster number for epidemiological purpose, or to classify freshly isolated S. mutans strains into bacteriocin-types.

Carbamimidothioic acid phenylmethyl ester salts and their N,N'-tetramethyl derivatives as possible antimicrobial agents.

A series of carbamimidothioic acid phenylmethyl ester salts and their N,N'-tetramethyl derivatives were synthesized, many of which exert an interesting inhibiting action on Gram-positive and Gram-negative bacteria and also on fungi. This activity is noteworthy in view of the large number of strains antagonized; the (3,4,5-trichlorophenyl) methyl ester chloride of carbamimidothioic acid appeared to be the best term of the series. The possible influence on this activity of decomposition kinetics to the corresponding mercapto-derivatives was investigated and compared with that of a series of N,N'-tetramethyl derivatives of previously studied carbamimidothioic acid phenyl ester salts.

S-Aryl(tetramethyl)isothiouronium salts as possible antimicrobial agents--III.

A series of S-Aryl(tetramethyl)isothiouronium salts were prepared and evaluated in vitro for antimicrobial activity. Some compounds revealed interesting inhibiting action on Gram-positive bacteria which is noteworthy in view of the large number of strains antagonized and of the low MIC values. The possible influence of decomposition kinetics to the corresponding mercaptoderivatives was intestigated and compared with those of salts previously studied.

In vitro antibacterial activity of enoxacin, a new pyridonecarboxylic acid.

The antibacterial activity of enoxacin was determined against 1015 strains of Gram-positive and Gram-negative bacteria, mainly freshly isolated from clinical specimens. The minimum inhibitory concentrations (MIC 50-75-90) were determined in comparison to three commonly used antibiotics: ampicillin, cefotaxime and gentamicin. Enoxacin has shown a broad spectrum of action and antibacterial activity in general higher that than of three currently available antibiotics. The antibacterial activity seems similar to that of other quinolones of second generation.

Production of bacteriocin-like substances by human oral streptococci.

A sample of human streptococci (mainly Streptococcus mutans species) from dental plaques was examined in order to evaluate the production frequency and activity spectrum of bacteriocin-like substances (mutacins). 89% of the 55 Streptococcus mutans strains produced substances with a wide spectrum of antibacterial activity. The bacteriocins produced showed a marked inhibitory activity against Gram-positive bacteria. Among the Gram-negative species tested, only Neisseria sicca was inhibited by 25% of Streptococcus mutans strains. Also, 16 strains belonging to oral streptococci other than Streptococcus mutans, were examined for their inhibitory capacity against the same indicator. The authors stress the importance of mutacins production in oral ecology and Streptococcus mutans pathogenicity.

Plasmids in Streptococcus faecalis subsp. zymogenes: transferability and molecular properties.

Seven strains of Streptococcus faecalis subsp. zymogenes were studied. Plasmids originally harboured by these bacteria, codifing for drug resistance, hemolysin and gelatinase activity, were transferred to a recipient strain ( JH2 -2) by conjugation. Studies on bacteria in mating showed the formation of protoplasmatic bridges between donors and recipients; these bridges should provide for the passage of genetic material. Kinetics of acquisition of the transferable resistance were carried out in liquid medium. Plasmids harboured in donor and transconjugant strains, responsable for antibiotic-resistance, for hemolysin and gelatinase activity, were analyzed later by agarose gel electrophoresis. These analysis permitted us to demonstrate the presence of many plasmids, some conjugative with a high molecular weight and others small and non-conjugative.

Conjugal plasmids in group D streptococci.

159 strains of group D streptococci isolated from clinical specimens were examinated for plasmids content. Our objective was to study some characters carried by plasmids: drug-resistance, hemolysins and bacteriocin activity. 73,6% of the strains were antibiotic resistant and in 69% of these, the drug-resistance was transferable by conjugation. In mating of S. faecalis subsp. zymogenes strains we could also isolate three different types of transconjugants in hemolytic activity. The interpretation of this observation was facilitated by the research of bacteriocin activity. We also classified the bacteriocins found in our strains into different types.

Bile acid transformation by the intestinal flora and cholesterol saturation in bile. Effects of Streptococcus faecium administration.

In 8 normal subjects the daily administration for 30 days of at least 750 X 10(6) Streptococcus faecium, a bacteria producing substances acting against Clostridia and other intestinal microorganisms reduced the cholesterol saturation and the molar percentage of deoxycholic acid (DCA) in bile. The percentage of cholic acid (CA) and chenodeoxycholic acid (DCA) increased significantly in feces, whereas lithocholic acid (LCA) decreased from 49.2 to 40.5%. In vitro fecal samples of subjects taking S. faecium transformed more slowly CA to DCA and CDCA to LCA. Moreover, 7-ketolithocholic seems an intermediate step in the biotransformation of CDCA to LCA. During SF administration, the anaerobic bacteria count of feces decreases, whereas aerobic bacteria increase; the changes of fecal flora may reduce the degradation of primary bile acids influencing the bile acid composition in both the bile and the intestine.

R factors in group D streptococci: classification by compatibility.

Antibiotic transferable resistance determined by R-factors is largely distributed among the enterococci. Over a two year period, 106 strains of D streptococci have been tested. Transferable resistance has been detected in 45 strain (57.7% of the resistant strains). The compatibility group of 24 plasmids has been determined: 12 belong to the group I; 4 belong to the group II; only one to the group III; one belongs to compatibility group IV; k R-factors have attributed to a new group V and the other two are not classifiable in any of the described groups.

[The incidence and variation of transferable antibiotic-resistance in Salmonella strains isolated in Modena in the years 1975-1977 (author's transl)].

The antibiotic-resistance of 450 strains of Salmonella isolated by the Microbiology Unit of the hospital of Modena during the years 1975-1977 was examined. During the study, the following antibiotics were assayed: ampicillin, kanamycine, tetracycline, chloramphenicol, streptomycin, sulfonamides, trimethoprim, tobramycin, gentamicin and nalidixic acid. The transfer capacity of antibiotic-resistance was measured by the double conjugation of E. coli K-12. During the period studied, a remarkable reduction was noted in those strains with multiple resistance (3 or more elements) decreasing from 72.9% in 1975 to 23.5% in 1977. These phenomena are due to the decrease of the serum-type wien during the years under study. Antibiotic-resistance demonstrated itself most frequently to streptomycin and tetracycline; and chloramphenicol-resistance showed the highest transfer capacity (97.2%).
