[The activity of urinary cystine-aminopeptidase in normal pregnancy (author's transl)].

Knowing that examining the levels of aminopeptidases in the serum of pregnant women can give us information about normal feto-placental function, we have tried to see if testing for these enzymes in urine of women with normal pregnancies could lead us to a more adequate and easy screening for fetal distress. With this end in view we examined the activity of urinary cystine aminopeptidase (CAP) in a group of 60 pregnant women at different stages of normal pregnancies. This activity was determined on 24 hour specimens of urine collected at different times in pregnancy and the results showed that there were variations from one week to another. As compared with activity in serum in normal pregnancies, urinary activity of this enzyme is raised in the first trimester of the pregnancy and lowered in the last trimester. The explanation of other authors about the possibility of these enzymes with high molecular weights passing into the urine is discussed. Asymptomatic bacteriuria is common in pregnancy, and so it can explain how CAP is excreted in the urine. But this hypothesis has still to be studied and proven.

Tubular necrosis complicating intravesical formaldehyde treatment.

Intravesical formaldehyde instillation for the treatment of profuse hemorrhage due to inoperable bladder carcinoma has been done for the last three years. Results have been satisfactory. Recently, an anuria developed after such treatment in a 76-year-old patient. Renal biopsy showed focal tubular necrosis. The possibilities of a toxic or an ischemic lesion are discussed. Diuresis reappeared spontaneously after six days and the patient was discharged from the hospital twelve days after the formaldehyde treatment.

Adenomyosis: study in a Jewish female population.

In a series of 920 hysterectomies, performed during a three-year period at the Gynecological Clinic of Hasharon Hospital, adenomyosis weas found in 89 cases, an incidence of 9.06%. Although this result is in accordance with that reported by other authors, we found a marked difference between the incidence of adenomyosis in the two Jewish female groups under study: the Ashkenazic women (natives of European countries or U.S.A.), and the Sephardic women (natives of Near East countries, Yemen or North Africa); 69.66% of the adenomyosis cases were in the former group as against only 30.34% in the latter. A preoperative diagnosis of adenomyosis was made in 19.10% of the cases. This comparatively high percentage appears to be due to the routine examination of the serum activity of cystine aminopeptidase (CAP) and leucine aminopeptidase (LAP), performed in all patients before operation. Increased values of these enzymes indicate the presence of uterine or ovarian tumoral processes. It is suggested that the diagnosis of adenomyosis must be considered in cases of uterine bleeding, dysmenorrhea and increased CAP and LAP values discovered in multiparous women of around 50 years of age, even when the bimanual examination does not reveal any other pathologic condition.

[Activity of serum leucine aminopeptidase lamp in normal and pathologic pregnancies].

The activity of the enzyme leucine aminopeptidase (LAmP) in serum was examined in 305 pregnant women (174 women with normal pregnancy, and 131 women with disturbed pregnancy). The activity of this enzyme increases during the normal development of pregnancy. In disturbed pregnancies, the LAmP activity is lower as compared to normal pregnancies. A peculiar case: toxemia of pregnancy, where the activity of the enzyme is considerably increased. This fact may be compared with the increase of LAmP activity in cases of liver damage. The LAmP activity test in pregnancy is reliable and can be performed practically in every laboratory, thus deserving its place among the many tests meant to assess the normal function of the foeto-placental unit. This test may also help in the diagnostic of some specific pregnancies such as twin pregnancy, hydatidiform mole and extra-uterine pregnancy.
