Adjuvant intermittent chemoimmunotherapy for primary breast cancer: a prospective study with immunologic follow-up.

In an interdisciplinary prospective study 50 patients surgically treated for breast cancer were treated with six monthly courses of aggressive adjuvant chemotherapy (adriamycin and cyclophosphamide) and were randomized to receive either immunotherapy with levamisole or no additional therapy. Probability of disease-free survival for the whole group is 54.9% at 42 months. There was no noticeable difference in disease-free survival for either pre- and postmenopausal women or for patients treated with or without levamisole. The addition of levamisole had no effect on the depression of in vitro immunologic functions during chemotherapy.

[Rectum extirpation or rectum resection in rectal cancer?].

Rate of recurrences and morbidity of anterior resection and abdomino-perineal extirpation in cancer of the rectum and especially of the midrectum are correlated to tumor localization, -staging and -grading. Regarding stage of the tumor both surgical procedures are equivalent. Poorer prognosis after abdomino-perineal extirpation is mainly due to advanced tumor stage. In cancer of the midrectum anterior resection should be performed.

[Anterior resection and abdomino-perineal extirpation in patients with rectal cancer: a retrospective analysis (author's transl)].

Eighty-seven patients with an abdomino-perineal extirpation of the rectum and 75 patients with an anterior resection of the rectum could be analysed retrospectively. The mean observation period was 36 months for anterior resection and 38 months for abdomino-perineal extirpation. The rate of recurrences, mortality, and postoperative complications of each surgical method was correlated to tumor-localization, -staging, and -grading. The poor prognosis of patients with abdomino-perineal extirpation was mainly due to relatively more cases in advanced tumor stage. In rectal cancer above 7 cm anterior resection should be performed.

[Quantitative analysis of immune complexes and assessment of autorosette-forming cells in patients with malignant disease (author's transl)].

Circulating immune complexes were isolated by precipitation with low concentration EDTA and PEG and their components analysed by specific immunoprecipitation in a laser nephelometric system; a good correlation between this method and other classical immune complex assays could be achieved. This method is simple, reproducible and practicable in the clinical routine. Quantitative analysis showed markedly increased levels of IgM and IgA specific immune complexes in cancer patients, especially those in an advanced stage, in comparison with non-malignant control patients. The percentage of autorosette-forming cells was markedly reduced in cancer patients, especially in a progressive disease, in comparison with non-malignant control patients.

[The effect of parenteral nutrition on cellular immune status in patients with gastrointestinal cancer].

In a pilot study, the distribution of lymphocyte subpopulations in the peripheral blood and skin tests with recall antigens have been examined in 42 patients with gastrointestinal cancer before and during a perioperative parenteral hyperalimentation. Concerning the calories and the concentrations of amino acids and carbohydrates, two different solution regimens were applied. During parenteral nutrition with 2400 and 2200 kcal especially the absolute numbers of lymphocyte subpopulations increase reflecting the changes of total lymphocyte count. There was no marked difference between the 2 groups. Both solutions are qualified to balance the preoperative catabolic situation indicated by preoperative lymphopenia. This kind of parenteral nutrition did not influence the patient's cellular immunological status which was tested by 3 recall antigens. It must, however, be considered that the physical i.e. nutritional situation of the patients was not reduced to an extent where the immunological status was impaired. Our results suggest that in the perioperative period cancer patients should be supported by hypercaloric parenteral nutrition. On the other hand more sensitive immunological tests such as DNCB for skin testing and functional assays for in vitro tests are necessary to assess the effects of this kind of therapy.
