Growth rate of secondary hydatid cysts of the brain. Case report.

Bilateral intracerebral hydatid cysts developed in a 14-year-old patient after an infarct of presumed embolic origin in the left frontotemporoparietal region. The average rate of growth of these cysts was about 5 cm per year. This suggests that the growth rate is far from uniform and indeed, particularly in young patients, may be much faster than originally estimated.

[Pathogenicity of Fusobacterium. Apropos of 2 cases].

Two cases of severe septico-pyohaemia due to Fusobacterium necrophorum are reported. Clinical data from human cases and experimental results from infections of laboratory animal with Fusobacterium necrophorum suggest: the important role of individual factors which enhance anaerobic growth (i.e. decrease of tissular redox potential); the involvement of thrombophlebitis with septic thrombi increasing the frequency of metastases; the early and long-lasting bacterial localisation in tissues, particularly in the liver where the proliferation of Fusobacterium necrophorum in Küpffer cells impairs macrophage function.

Rhino-orbital phycomycosis complicated by cerebral abscess.

The incidence of phycomicosis has increased in the last decades. Its diagnosis is very difficult and usually not established ante morten. Early treatment is of crucial importance, because despite the antifungal drugs, the mortality rate remains around 80%. The present report describes a successfully treated diabetic patient with a rhino-orbital form of the disease and an unusual complication--a cerebral abscess--in whom the clinical diagnosis was supported by the CT findings.

Gyratory epilepsy in a patient with a thalamic neoplasm.

A patient with a right thalamic oligodendroglioma developed seizures characterized by circling behavior, speech arrest, and secondarily generalized seizures. Gyratory epilepsy is relatively uncommon and may either represent a benign form of primary generalized epilepsy or occur secondary to a focal cortical lesion. Thalamic stimulation has been shown experimentally to induce circling movements, but no other clinical cases with a thalamic lesion have been described.

Cardiovascular reflexes and pudendal evoked responses in chronic haemodialysis patients.

The bulbocavernous reflex (BCR), cortical pudendal evoked responses (CPERs), non-invasive cardiovascular tests and nerve conduction studies were performed in 16 patients on chronic haemodialysis and in a group of normal subjects. BCR and CPERs were more severely affected in patients with impotence. There was a significant correlation between the Valsalva ratio and P1 latency of the CPERs. The BCR and CPERs are alternative techniques for the assessment of impotence in uremic patients.

[Value of echocardiography in the diagnosis of a complicated form of Kawasaki's disease].

Kawasaki disease affects children under 4 years of age and is characterized by fever, mucocutaneous rash and cervical lymph node enlargement. It is often complicated by coronary vasculitis and/or pericarditis, myocarditis and endocarditis. Echocardiography is indispensable to diagnose and follow up these complications. A study of the literature and of 4 personal patients showed that it is also useful for the early detection of coronary aneurysm and simple dilatation of the coronary arteries. The sensitivity and specificity of echocardiography in recognizing these complications are such that coronary angiography is exceptionally required. In the search for a cause of prolonged fever in children, the sensitivity of echocardiography makes it possible to diagnose an atypical form of Kawasaki disease.

Acute atypical psychosis following a right hemisphere stroke.

An acute atypical psychotic episode characterized by hallucinations and delusions suddenly developed in a 63-year-old right-handed male following an extensive right hemisphere infarction in the carotid artery distribution. While hallucinations were visual, tactile and auditory, delusions were associated with specific neurologic defects (anosognostic phenomenon, reduplication for place, body-parts and objects and confabulation). Distractibility, inappropriate sexual behavior, agitation or seizures were lacking. This case supports the presumption that the right hemisphere damage plays a major role in the genesis of organic psychotic episodes.

Medications exposure during pregnancy. A study in a university hospital.

To collect informations about drugs prescribed during pregnancy in France we carried out a retrospective survey from august to december 1989. This study was based on a questionnaire at delivery for 225 women. Socio-economic status, obstetrical past history and patterns of prescribing medications were studied. 99.5% of the women were found to have used drugs during pregnancy with a mean of 6.84 medications per woman. There was no influence of age, geographic origins, number of previous pregnancies on drug consumption; socio-professional status was also found to have a poor correlation with drug intake. Patterns of prescribing were compared to those of a previous French study made in 1976: changing pattern of prescribing were found for progestatives, corticoids, neurotropes, aspirin and beta adrenergic agents. We also pointed out that self-administered drugs decreased from 25.9 to 17.9%. This evaluation supports the need of further epidemiological studies in our country.
