A custody and placement evaluation of an infant with a psychotic, mentally retarded mother.

An emergency evaluation was requested on a 16-week-old infant and her parents. The child's mother had a long history of schizophrenia and mental retardation and was noted to decompensate following the child's birth; the mother was hospitalized and the child placed in foster care. Although the mother was severely impaired, the family had a reasonable and appropriate plan for the infant's care and reunification of the family was recommended. Aspects of custody and placement recommendations, the risk of the child for subsequent disorder, and legal issues involved in the evaluation are discussed.

[Noninvasive diagnosis of ventricular septal defects using echo-pulsed Doppler velocimetry, localization, evaluation of the shunt and the associated lesions].

A preliminary study of 15 patients with ventricular septal defect demonstrates that Echo-Doppler velocimetry allows the non-invasive diagnosis of this malformation in 80% of the cases, as well as its localisation and detection of associated lesions; tin addition evaluation of the shunt is made feasible by non invasive recording of the flow velocity curves of the shunt and of the pulmonary artery according to a technique derived from the basic one. This method turns out to be essential for establishing the aetiological diagnosis of left parasternal systolic murmurs.

[Ascending aortic aneurysm with mucoid infiltration of the media and aortic leaflets. Pathogenic hypotheses- surgical consequences].

The theoretical and practical problems posed by aneurysms of the ascending aorta with mucoid infiltration of the Media and the aortic leaflets are discussed with reference to two suggestive cases. These histological changes are not specific for Marfan's syndrome, the relation with which is very inconstant. Incomplete replacement of the affected tissues risks a recurrence which is especially serious when the sinus of Valsalva is involved. Therefore, total replacement of the ascending aorta with aortic valve replacement and reimplantation of the coronary arteries is generally indicated. Histology of the excised tissues should be performed routinely with specific stains for mucopolysaccharides. Close follow-up of operated patients with this condition is recommended, especially those with only partial replacements.
