[Color Doppler in the echographic evaluation of breast nodules].

The authors report on their experience with color Doppler sonography in the diagnosis of solid breast masses. Twenty-two patients were examined; breast masses were studied with B-mode US first, and then with color Doppler US to evaluate eventual tumor vascularization. Color Doppler US demonstrated only one vascular pole in histologically confirmed benign masses. On the contrary, in 92.8% of histologically confirmed malignant masses, color Doppler easily depicted 2 or more groups of nutritional arteries. Color Doppler makes the diagnosis of malignant masses easier, thus allowing, in the author's opinion, a reduction in the number of biopsies of solid breast masses clinically/mammographically detected. The use of color Doppler US is therefore suggested: the technique is noninvasive, fast, and easy and its widespread use would translate into advantages for both the patient and the clinician.

[Portal hypertension of hepatic origin. A qualitative assessment by color and echo Doppler US].

One hundred and twenty-eight subjects were studied: 103 of them were affected with portal hypertension diagnosed both radiologically and clinically. Twenty-five healthy subjects were studied, as a control group, by means of combined real-time US and color Doppler. US parameters were evaluated, specific to chronic hepatopathy, together with the Doppler qualitative parameters relative to splanchnic vessels hemodynamics. Our results allowed a sort of noninvasive angiogram of the portal system to be obtained, which is to be of use for diagnosing portal hypertension, and for assessing its causes, risks, and consequences. This study was also aimed at suggesting an examination protocol for portal hypertension, employing real-time and color Doppler US, which any radiologist with enough experience in abdominal US could use. Color Doppler, although not strictly necessary to obtain good results, dramatically shortens execution times. Moreover, color Doppler allows the method to be more quickly learned.

[Doppler color in the echographic study of hyperplastic parathyroid glands].

The sonographic examination of hyperplastic parathyroid glands is a well-known and appreciated technique. However, its diagnostic contribution is still somehow inadequate, due to the difficult differential diagnosis of the various solid hyperechoic nodular structures in the neck and to the presence of frequently ectopic glands. The combined use of B-mode and color-Doppler US allows the vascular features of suspicious parathyroid nodules to be satisfactorily demonstrated. Higher sensitivity and specificity than conventional US are the main advantages of this technique. Still, further research is needed for B-mode color-Doppler US to actually replace fine needle biopsy in confirming the diagnosis.

Prevention of deep venous thrombosis in orthopedic surgery: effects of defibrotide.

The effectiveness of defibrotide was compared with that of calcium heparin and acetylsalicylic acid (ASA) in the prevention of deep venous thrombosis (DVT) resulting from orthopedic surgery. Sixty-three patients scheduled for elective or traumatological surgery, for the most part involving the hip joint, were recruited. The patients were randomly assigned to one of the following treatment groups: defibrotide (400 mg twice daily), administered intramuscularly (n = 19); calcium heparin (5,000 IU thrice daily), administered subcutaneously (n = 25); and ASA (100 mg on alternate days), administered orally (n = 19). Administration of the drug was started one day before surgery and continued until the seventh day after surgery. Each patient was monitored daily by means of the fibrinogen uptake test. The incidence of increased uptake did not differ significantly in the three groups (defibrotide, ten out of 19; calcium heparin, ten out of 25; ASA, seven out of 19). Conversely, a trend in favor of defibrotide was noted in cases of symptomatic DVT (defibrotide, one out of 19; calcium heparin, two out of 25; ASA, four out of 19) and pulmonary embolism (defibrotide, no cases; calcium heparin, one case; ASA two cases). On the strength of these findings, defibrotide therapy qualifies as a valid alternative to conventional DVT prophylaxis in orthopedic surgery.

Effects of chronic prazosin treatment on the renin-angiotensin-aldosterone system in man.

The effects of chronic prazosin treatment (3 mg/day for three weeks) on plasma renin activity (PRA) and plasma aldosterone (PA) levels were evaluated in 12 hypertensive patients, under conditions of metabolic balance. After three weeks of drug administration no significant change occurred in PRA as well as PA levels, with respect to pretreatment values, both in basal conditions and following 2 hours of ambulation. No change was observed in heart rate, while a fall in both systolic (P less than 0.02) and diastolic (P less than 0.05) blood pressure occurred in supine as well as in deambulation-stimulating condition. A mild increase in body weight (P less than 0.05) and a decrease in serum sodium (P less than 0.05) was induced by prazosin treatment. These findings are in keeping with the pharmacologic properties of prazosin, which is a selective blocker of postsynaptic alpha adrenoreceptors and therefore lowers vascular resistance without reflex sympathetic overactivity. The moderate volume expansion after prazosin does not appear to be aldosterone mediated.

Thallium-201 myocardial imaging in young asymptomatic subjects with "ischaemic" changes in exercise electrocardiogram.

In order to assess the significance of a positive electrocardiographic response to exercise test in 10 asymptomatic subjects with normal resting ECG, a myocardial perfusion scanning with Thallium-201 at rest and during exercise was performed. The following ECG tests were also performed on these subjects: forced hyperventilation, exercise test after nitroglycerin (TNG), exercise test after propranolol. Myocardial perfusion scans did not reveal any defect at rest nor during exercise. Hyperventilation determined abnormalities of ventricular repolarization in all subjects. TNG did not improve the response to exercise test and even decreased the ischemic threshold. After propranolol the electrocardiographic response to exercise became normal in 9 cases, while in 1 subject the ST depressions were impressively reduced. All subjects had a follow-up of 14 to 91 months. During this period none presented symptoms suggesting a coronary disease. This study suggests that combined evaluation of Thallium myocardial perfusion imaging during exercise and exercise ECG test after TNG is most useful in differentiating ischemic from nonischemic exercise ST depressions in asymptomatic population.

[Use of radioactive isotopes in the diagnosis of pulmonary tumors. Analysis of the results of 18 anatomically verified cases].

18 cases of anatomically verified lung cancer were studied scintigraphically with microspheres of human albumin labelled with 99Tc, mercury bichloride (197Hg) and citrate of gallium (67Ga), to assess the diagnostic value of this technique. Using albumin microspheres, changes in the scintigraphic picture were obtained in 100% of cases, but inferior results were obtained with the two other tracers (gallium 75% and mercury bichloride 58.8%). The poor specificity of scintigraphy with albumin, which was changed in numerous non-neoplastic pathologies, removes all significance from this apparently brilliant result. From an overall assessment of all three scintigraphies, no increase in positivity percentages is obtained; the agreement of all three with respect to the neoplastic nature of the condition is however very good. Although possible conclusions are less favourable than those reported by other workers, the investigation with radioisotopes plays an important rôle in the diagnosis of lung tumours, in view also of its harmlessness and tolerability, although it is not enough on its own for diagnosis. Large series and constant anatomical and histological monitoring are needed before any definitive assessment can be attempted of the diagnostic value of this method.
