[Degenerative spondylolisthesis. Clinical manifestations and treatment apropos of 26 operated cases].

The authors report 26 cases of surgically treated degenerative spondylolisthesis. Twelve patients complained of permanent uni or bilateral sciatica. All the others had a syndrome of intermittent claudication with pain and paresthaesia. The authors stress once again the highly evocative nature of this syndrome which reflects involvement of the nerve roots in the stenosed lumbar canal. Treatment consisted of lamino-arthrectomy which resulted in the disappearance of symptoms in most of cases. In four cases, vertebral slipping worsened during the post-operative cowise with, in three cases a recurrence of symptoms necessitating a complementary arthrodesis. This risk of increased slipping is a result of the instability created by the arthrectomy, especially when it is bilateral, and has led the authors to widen their indications for routine complementary postero-lateral arthrodesis.

[Dorsal tenosynovectomy of the wrist in rheumatoid arthritis. Results obtained in 30 cases].

With reference to a series of thirty synovectomies of the wrist in patients with rheumatoid arthritis, the authors report the results observed after an average interval of three years and six months. There was a remarkable reduction in pain in three-quarters of the cases. On the other hand, loss of mobility was noted in 15 patients. Resection of the cubital head was carried out in half of the cases. This was followed in two cases by internal translocation of the carpus, and in three cases by internal translocation combined with cubital inclination of the hand. The results of the tenddon repairs carried out in six patients have been completely satisfactory. A study of the present cases and of data from the literature confirms the local prophylactic value of tenosynovectomy which may also prevent tendon ruptures. The best result are seen in the less evolutive forms in which the risk of relapse is least, and in cases in which surgery is carried out on stable wrists with cartilage showing little alteration. In the presence of inferior radiocubital arthritis the resection must be systematic as it makes it possible to carry out a complete synovectomy of this joint. The authors compare the value of surgical synovectomy of the wrist with that of radioactive synoviorthesis.

[2 new methods of retention in sagittal osteotomy of the ascending ramus].

The authors present two techniques designed to ensure contact between the two bone cortical plates in sagittal osteotomy of the superior ramus of the mandible used in the correction of prognathia and retrognathia. The first technique requires the use of a special suture guide, designed to aid in steel wire osteosynthesis. The second involves a bolt fixed by a screw, the role of which is to keep the external cortex applied against the medial fragment.

[Dentofacial orthopedics and orthopedic surgery].

The authors describe five cases of surgical correction of dysmorphisms of the maxillaries requiring orthodontic treatment before surgery. Pre-operative orthodontic treatment of the adult has the advantage of enabling the arches to be aligned, facilitating the operation when the osteotomized fragments are positioned and resulting in immobilisation in a good position.

[Surgical and prosthetic approach to the difficulties of fitting in the completely edentulous].

Prosthetic Surgery must not be considered as an exceptional process. It is likely to transform the comfort and stability of a complete prosthesis if the indications are well laid down. Two elements combine towards its success. 1. The surgical action, which may be -- a limited vestibuloplasty, destined to abolish the small ligaments, the hyperplasias or the polypoid formations; --an extensive vestibuloplasty, intended to deepen the non-retentive vestibules. The process of choice is a super-periosteal plasty of the CLARKE or VRASSE and BATAILLE type. It may be used above all on the mandible. For the maxilla, CELESNIK's intervention may be associated with it; --free skin grafts. They do not always give the results anticipated because of the cicatrical retraction which is almost constantly observed. 2. The retention is as important as the surgery. It is carried out by an anchorage with wires, in a hammock or transcutaneous, and by a plate of resin lined with a layer of elastomeres of silicon. The material appears very much superior to the sponges of polyvinylic alcohol advocated by VRASSE and NETTER.

[Benign giant cell tumors associated with Paget's disease. Apropos of 1 case].

The authors report the observation of two benign giant-cell tumours that developed in the cranium of Paget's disease patients. The two tumours were resected and cure was complete. Eighteen other cases of benign giant-cell tumours were found in the literature. All were discovered in relation to tumefaction occurring in an affected bone in a patient with generalized Paget's disease, often unrecognized. The tumours were usually unique although multiple tumours were found, with particular predilection for the bones of the cranium and the face. The radiological signs consisted of an osteolytic zone in an affected bone; there were no specific characteristics and it was not possible to distinguish the tumours from a malignant tumour. Diagnosis was based upon an anatomo-pathological examination. In the 18 cases in the literature, the benign caracter indicated by the biopsy was confirmed by the favourable evolution. In contrast in 17 other cases the atypical nature of the stroma, the irregular arrangement of the giant cells together with the occurrence of atypical mitoses and the abnormal character of the vascularization indicated straight away the malignant nature of the lesions, which was regularly and rapidly fatal.
