[Herniation of a lumbar disk in the elderly. Results of chemonucleolysis].

The purpose of this study was to evaluate the outcome after chymopapain chemonucleolysis in elderly patients with low back pain and sciatica due to lumbar disc herniation without concomitant stenosis, unresponsive to conservative therapy. In the 42 study patients, aged 60 years or older, clinical manifestations were similar to those usually seen in younger patients. Evidence of nerve root tension, including a positive straight-leg raising test seen in 67% of our patients, proved valuable for outruling sciatica due to bony impingement. Outcome was favorable in 78% of cases after a mean follow-up of 2.5 years. This findings is consistent with previous studies performed without age restrictions. Chemonucleolysis had no adverse effects. Despite the biochemical changes which occur in the aging disc, chemonucleolysis is an effective tool for the treatment of sciatica due to disc herniation in elderly patients.

[Anatomo-radiological study of edentulous jaws prior to implant placement].

In the case of a total loss of teeth, a conventional understanding of descriptive anatomy is no longer sufficient. It does not take into account: The principles of osteo-architecture allowing the distribution of different types of bone structure (Haversian, spongy, papyracious) to be understood; The mechanisms of bone resorption; The new anatomical relationships created by such resorption.

[Sciatica caused by lateral stenosis of the spinal canal].

Eighty-eight patients (52 men, 36 women) were operated on for lateral recess syndrome, during the 1970-1986 period. 7 had priorly a disc excision; in 11 a chemonucleolysis had been performed. The pain affected one nerve root in 41 cases, two in 8, three in 1. In 33 patients the pain pattern was incomplete. Myelography was used in 77 cases, and was positive in 68. CT scan was used in 43 cases. A posterolateral fusion was added to the unroofing of the nerve in 9 patients, who had severe low back pain before surgery. The results were assessed with a special rating scale of 20 points. They were considered excellent or good in 70 per cent. 6 patients were reoperated from 3 months to 2 years post-operatively mainly for recurrent pain at another level or side.

[Genioplasty, indications and therapeutic methods in dento-maxillary malformations].

Genioplasty constitutes an adjunctive surgical procedure used in adult plastic surgery of the facial profile. It is used after dental malocclusion has been corrected and facial morphological features have been reassessed (soft tissue changes consecutive to maxillary osteotomy should be previously analyzed by cephalometry). Genioplasty permits achieving a modification of the chin point in the three planes. The authors describe several types of genioplastic operations, depending on the aim.

[The fate of fat grafts used in surgery of the lumbar spine].

The fate of fat grafts thicker than 5 mm applied to prevent post-operative peridural fibrosis was followed by CT scanning between the second and fourth months and in later months in four patients treated for lumbar spinal stenosis. The CT scanning findings were verified in two patients who were operated on again. The fat grafts lost thickness and became more even. In one case, the graft had completely disappeared.

[Restoration of the alveolar crest with a biomaterial filler. A clinical and histological study].

The implantation of biocompatible hydroxyapatite is used for human mandibular ridge augmentation in edentulous patients. Histologically, we studied such a material after a long period of implantation (1 1/2 to 2 years) in two patients. The histologic study showed a narrow meshed net of collagen tissue surrounding the hydroxylapatite particles. In the periphery of that tissue, we could see an osteogenesis: the bony trabeculae thus formed firstly were made of an immature bony matrix with an irregular "woven" pattern; secondarily, a mature bone, quite similar to the normal bone, occurred. In our two observations, there was an unusual inflammatory reaction characterized by cellular infiltration of lipophages, lymphocytes and plasma cells in the connective tissue and in one case by a true ischaemic necrosis with granuloma and bacteria. In spite of that inflammation, the whole material was well tolerated, without foreign-body granuloma. Such a granuloma was only found in superficial areas of the gingiva, around extruded particles of the biomaterial.
