First-trimester ultrasonic diagnosis of twin reversed arterial perfusion sequence.

Twin reversed arterial perfusion (TRAP) syndrome sequence is a rare specific anomaly of twin gestation with fused placentae and umbilical anastomosis. This syndrome occurs once in very 35,000-48,000 births and has been described in the second trimester (23-29 weeks of gestation). We report on early sonographic diagnosis (10 and 12 weeks' gestation) of two cases of TRAP sequence, together with their umbilical cord Doppler studies.

Ultrasonic diagnosis of mediastinal cystic hygroma.

The incidence of cystic hygroma, which represents dilated obstructed jugular lymph sacs, is 1 in 6000 pregnancies. Cystic hygromas can be located in the nuchal area or in any other location. The prenatal ultrasonic diagnosis of a cystic hygroma in the mediastinum is presented.

[Ultrasonic estimation of fetal weight].

To generate formulas to estimate fetal weight on the basis of sonographic measurements, 226 pregnant women were scanned within 3 days of delivery. Measurements included: biparietal diameter, head circumference and area, abdominal circumference and area, and femoral length. The contribution of each parameter to the estimation of fetal weight was tested by stepwise regression analysis. The best dependent variable was found to be the log of weight. The formula based on cranial and abdominal parameters proved to be inaccurate, especially when fetal weight was less than 2500 g. The addition of femoral length improved weight estimation, with a multiple r of 0.95174. The addition of gestational age led to even more accurate formulas. The most accurate found was based on biparietal diameter, abdominal circumference, femoral length and gestational age, with a multiple r of 0.95651 and an absolute mean error of 7.976 +/- 5.657%. It also produced better weight estimates at the 2 extremes of fetal weight.

Ranged-gated pulsed Doppler of the umbilical artery in human fetuses during normal pregnancies.

Range-gated pulsed Doppler has permitted the study of umbilical artery flow velocity waveforms (FVWs) in human fetuses and is becoming part of the antepartum fetal evaluation. No uniform method of describing such velocity waveforms and no significant body of normal values using pulsed Doppler can be found in the literature. The present study reports on a cohort of 268 patients who had pulsed Doppler umbilical artery FVWs performed during their pregnancy. The gestational age at the time of the study ranged between 16th and 42nd weeks of gestation in normal pregnancies. The systolic to diastolic (S/D) ratio was found progressively to decrease as gestation advanced; however, the range of normal values in this study was found to be quite large at all gestational ages. The present data represent normative data of the umbilical artery FVWs S/D ratio in a large number of normal pregnancies as shown by the follow-up of all the patients to delivery. Before umbilical artery waveform becomes part of the antenatal armamentarium, we advise caution before adopting rigid values that are derived from small groups. Lack of standard values for the 95th percentile may result in a premature delivery of an otherwise normal fetus.

Duplex Doppler ultrasound in the evaluation of pregnancies complicated by pregnancy-induced hypertension.

Umbilical artery velocimetry using pulsed Doppler technique was carried out in 49 pregnancies complicated by pregnancy-induced hypertension (PIH). Outcome of the pregnancies was evaluated after delivery. We found the systolic to diastolic ratio of the umbilical artery to be highly sensitive and specific in predicting abnormal outcome in pregnancies complicated by PIH. In our opinion, the pulsed Doppler technique can be a useful adjunct to other methods for evaluation of fetal well-being in patients with PIH.

Predictive value of the femur length to abdominal circumference ratio in the diagnosis of intrauterine growth retardation.

The ratio between sonographically measured fetal femur length to abdominal circumference (F/A) was studied in 471 consecutive normal singleton pregnancies from 18 to 40 weeks' gestation. The 90th (0.2715) percentile of this ratio was evaluated as a predictor for intrauterine growth retardation (IUGR) in an additional 1,502 pregnant women; 146 fetuses were found to be above the 90th percentile, of which 20 were born growth retarded. A total of 69 fetuses was found post partum to be small for date; 44 were asymmetrically growth retarded and 25 were symmetrically growth retarded. Of the 20 growth-retarded fetuses, which were correctly diagnosed in utero, 19 were asymmetrically growth retarded. The additional 49 growth-retarded newborns were not detected using the F/A ratio as a diagnostic parameter. Low sensitivity (29.98%) together with low positive predictive value (13.69%) rendered the parameter unsuitable for screening IUGR. Nevertheless, the high specificity (91.20%), together with the high negative predictive value (96.38%), makes it a useful age-independent parameter in high risk pregnancies, where asymmetric IUGR is suspected.

Aortic valve replacement during pregnancy. A case report and review of the literature.

Aortic stenosis (AS) is an uncommon complication during pregnancy, but when it occurs it results in significant maternal and fetal morbidity and mortality. We present a gravida with severe AS in which aortic valve replacement was performed in the 3rd trimester of her pregnancy. She had severe AS with mild aortic regurgitation and severe congestive heart failure. Fetal heart rate and uterine activity were monitored and recorded during the operation. The pregnancy was carried uneventfully to term and the patient was delivered of a healthy baby. The literature is reviewed and recommendations are made for the management of AS during pregnancy.

Imerslund syndrome with dolichocephaly.

Imerslund syndrome is a rare autosomal recessive disorder of megaloblastic anemia as a result of selective vitamin B12 malabsorption associated with proteinuria. An Arabic Muslim family is described, with three children who had inherited selective vitamin B12 malabsorption with proteinuria. Dolichocephaly was noted in all the male children of this family in association with congenital megaloblastic anemia and proteinuria. The findings of this anemia are compatible with Imerslund-Gräsbeck syndrome, and coexistence of this syndrome with dolichocephaly in a single family has not been previously reported.

[Toxocariasis in Emek Israel].

Toxocariasis is a rare zoonotic disease in Israel. It usually affects children under the age of 10. Toxocara canis and Toxocara catis are common parasites among dogs and cats which affect man when he ingests the eggs of these helminths. We describe 3 children with different clinical presentations of the infestation. A 6-year-old boy had pain in the muscles of the limbs and diffuse patches in the right lower lobes on X-ray; a 7-year-old girl presented with a limp; and a 3-year-old boy had abdominal pain and a maculopapular rash covering the whole body. All 3 had eosinophilia. They illustrate the importance of toxocariasis in the differential diagnosis of eosinophilia.

[Tuberculous meningitis].

2 cases of tuberculous meningitis are presented, 1 in a 54-year-old woman who had immigrated to Israel from Turkey 36 years before, and the other in a 10-year-old girl who recently immigrated from Ethiopia. Since diagnosis is difficult and prompt antituberculous treatment is life-saving, treatment may be instituted before the bacteriological diagnosis is established. A short term treatment of 9 months and a combination of rifampicin and isoniazid were successful in our patients.

Evaluation of pulsed Doppler echocardiography for measurement of aortic blood flow in the fetal lamb.

The purpose of this study was to assess the accuracy of a quantitative, range-gated, two-dimensional Doppler echocardiography method for measurement of blood flow in the fetal lamb descending aorta. Comparison was made between this method and measurements determined by an electromagnetic flowmeter placed directly on the aorta in the chest of the fetus. Stroke volume was manipulated acutely either by the removal of blood, the addition of fluid by injection, or by pharmacologic means. During each procedure, descending aortic blood flow was estimated by the Doppler ultrasonography method and compared to the flowmeter recording. The size of the lumen of the aorta determined by echocardiography was correlated with direct measurement during surgery and at autopsy. A total of 359 flow measurements were obtained in 15 fetuses. Doppler ultrasonographic flow studies and electromagnetic flowmeter measurements were found to be highly correlated (r = 0.93). The study demonstrates the accuracy and reliability of the two-dimensional pulsed Doppler echocardiography method for measurement of blood flow in the descending aorta in the fetal lamb.

A preventive dentistry component in the curriculum of Israeli dental students.

A preventive dentistry program conducted by 34 and 27 senior dental students as an integral part of children's clinic was evaluated. At the initial meeting of the three-session program, dental knowledge and oral hygiene levels were all 265 participating children. Follow-up who had participated in the prevent program the previous year and were now continuing their treatments, demonstrated significantly improved dental knowledge oral hygiene as compared to new patients. All of the children demonstrated significantly improved dental knowledge and oral hygiene at the final session of the program. The effectiveness on the participating children, this program was also successful in being accepted by students and faculty as a regular component of the clinical curriculum. The model presented here will be modified and improved on for future continued implementation.
