[Giant cell fibroblastoma associated with Darier-Ferrand dermatofibrosarcoma in an adult].

INTRODUCTION: Giant cell fibroblastoma is a rare mesenchyma tumor of childhood having many similarities with dermatofibrosarcoma protuberans in adults. OBSERVATION: We report the case of a 28-year-old woman presenting a subcutaneous inter-mammary mass associating both tumors. Immunohistochemistry showed an expression of CD 34 only by dermatofibrosarcoma protuberans cells. DISCUSSION: It is important to stress: the rarity of this association, the difficulty to confirm the diagnosis and to establish the links between these two tumors: simple association (as in our case), transformation or recurrence of giant cell fibroblastoma in dermatofibrosarcoma protuberans.

[Combined spinal and epidural analgesia for labor. Prolongation by addition of a minidose of clonidine to sufentanil. An initial study].

The effect of adding a minidose of clonidine to intrathecal sufentanil during the early first stage of a painful labour was evaluated in this preliminary open-label, non-randomised trial. Group 1 received sufentanil 5 micrograms + clonidine 30 micrograms intrathecally (n = 10) and group 2 only intrathecal sufentanil 5 micrograms (n = 11). The two groups were not statistically different regard-ing age, weight, height, primiparity (67 vs 50%), oxytocin use (37 vs 60%), initial cervical dilation (m +/- DS: 2.9 +/- 1.1 vs 2.9 +/- 1 cm) and VAS pain scores (70 +/- 14 vs 68 +/- 19 mm). In group 1, analgesia was markedly prolonged with a reduced variability in duration: 146 +/- 27 min vs 95 +/- 44 min, (P = 0.006). VAS pain scores were: 14 +/- 20 vs 19 +/- 13, 1 +/- 3 vs 9 +/- 12, 0 vs 5 +/- 7, 48 +/- 12 vs 65 +/- 15, five and fifteen minutes after intrathecal injection, during maximum efficacy, and at the time additional analgesia was required, in group 1 and group 2, respectively. Analgesia evaluated with the VAS pain scores was better in group 1 compared with group 2 (P = 0.02) and decreased somewhat slower. Side effects, such as hypotension, pruritus and sedation, were not statistically different between groups. Nausea and motor blockade did not occur. In conclusion, the addition of a minidose (30 micrograms) of clonidine to sufentanil 5 micrograms given intrathecally seems to potentiate markedly the analgesia obtained during the early first stage of labour.

[Encapsulated papillary carcinoma of the thyroid gland. Particular histological aspect of a case with review of the literature].

The authors report a case of encapsulated papillary neoplasm in the right lobe of the thyroid of a 36-year-old woman. This neoplasm was a papillary encapsulated carcinoma with variable histologic patterns not properly described in the literature. In the light of a review of the literature, the authors stress the macroscopic and histologic aspects of this tumor and they discuss the prognosis and therapeutic tools used to cure this neoplasm.

[Melanotic schwannoma. A case with review of the literature].

The authors report a case of melanotic schwannoma in a 46-year-old man after 2 years of bilateral sciatica. Surgical removal was incomplete, because of the posterior site of the tumor. The patient was alive and well, 2 years after surgical removal, with some surgical sequelae. The authors stress the diagnostic difficulties of this melanotic and review the previously reported cases in a comparative study.

[Bladder pheochromocytoma and pregnancy. A case report].

The authors report the case of a 31-year old patient who successfully completed a pregnancy despite unsuspected bladder pheochromocytoma. After delivery, the patient required care post-partum due to serious hemodynamic problems. Hypertension had been attributed to toxemia of pregnancy. The authors recall the diagnostic difficulties of tumors of this type, and the means which can be used for topographical diagnosis and the precautions to be taken before this tumor can be treated surgically.

[Malignant fibrous histiocytoma with giant cells of the skin. Anatomoclinical and immunohistochemical study of a case].

The authors report a case of malignant giant cell soft tissue tumor which appeared as a skin tumor of the thigh in a Tunisian women aged 17 years. Immunohistochemical study showed that the giant cells recognized KP1 antibody, while spindle interstitial cells reacted with anti-factor XIIIa antibody. The patient is alive and well eighteen months after large surgical removal. Two similar cases have been reported in the literature. Differential diagnosis, histogenesis and prognosis of this rare tumor are discussed.

[Correlation between epithelioid sarcoma and poorly differentiated fibroblastic synovialosarcoma].

The authors report an original case of tendosynovial sarcoma localizated in the side of the palm of the hand of an eight year old child. This tumor has two morphological forms. One is characteristic of undifferentiated synovialosarcoma, and the other of epitheloid sarcoma. The immuno-histochemical study confirms the diagnosis of undifferentiated synovialosarcoma. On the basis of this case and with reference to the literature, the authors suggest that epithelioid sarcoma and synovialosarcoma constitute the same entity.

[Systemic mastocytosis. A case report].

The authors report a case of systemic mastocytosis in a 65 year-old woman. The clinical picture was generalized abdominal and extra abdominal lymph nodes, ascites, splenomegaly, and hepatomegaly. As gastro-intestinal symptoms led to suspect a digestive malignant lymphoma; the patient underwent an exploratory laparotomy. The diagnosis of systemic mastocytosis was established by demonstrating an excessive number of mast cells in the cutaneous and extracutaneous tissues (liver, mesenteric lymph, nodes, spleen and bone marrow). Bone marrow biopsy revealed extensive myelofibrosis.
