[Talc--a standard drug in the treatment of malignant pleural effusion].

Currently, talc is the most effective and widely-used agent for chemical pleurodesis in the treatment of both malignant pleural effusion and pneumothorax. Its popularity has been growing due to the low incidence of side effects, low cost and higher success rate in comparison with other agents (tetracyclines, bleomycin, Corynebacterium parvum). The guidelines for talc pleurodesis are summarized and the history of the Corynebacterium parvum as an agent for chemical pleurodesis is mentioned. Its production in the Czech Republic has been halted due to the increased interest in the talc as a chemical sclerosant.

The influence of Propionibacterium acnes (Corynebacterium parvum) fractions on immune response in vivo.

Bacterin of Propionibacterium acnes (Corynebacterium parvum), its cellular fractions (lipids, fractions obtained by mechanical disruption and differential centrifugation, by phenol-water and pyridine extractions), and a polysaccharide from culture filtrate were prepared and tested in mice. The activation of RES by splenomegaly and hepatomegaly, prevention of listerial infection, prevention of the lethal effect of sarcoma 180, and depression of liver microsomal cytochrome P-450 were employed. The bacterin was effective in all tests. Lipid-free cells were less active, in particular in the activation of RES and in the listerial infection model. Fractions prepared by the disruption and differential centrifugation lost their activity in all tests along with a decrease in molecular weight. Lipids extracted by ethanol caused pronounced splenomegaly and decreased the cytochrome P-450 content. The residue left after the phenol-water extraction was very active, its delipidation did not destroy the activity. Pyridine extraction provided a completely inactive extract, but a very active residue. The possibility of reducing the complexity of bacterin while preserving immunomodulatory effect is demonstrated.

Class IgG, IgM and IgA antibodies against Staphylococcus aureus antigens in human serum and saliva.

Using the ELISA method antibodies against the sonicate, teichoic acid (TA) and exoproducts of Staphylococcus aureus were determined in sera and saliva of healthy individuals. Main serum antibodies against all the antigens used were shown to be class IgG antibodies. However, antigens of the sonicate stimulated significantly even the systemic IgA response. In the saliva class IgA antibodies predominated, but IgG antibody levels against TA and exoproducts approached the level of IgA antibodies. Levels of IgM antibodies against all antigens tested were low in both the serum and saliva which corresponds with the anamnestic type of response. On the basis of these results one may assume that not only IgG, but also IgA antibodies are important in the systemic immunity against staphylococcal infection and in the immunity of mucous membranes; besides IgA, even class IgG antibodies play an important role.

Immunostimulatory effect of Rothia dentocariosa in mice.

In mice killed Rothia dentocariosa cells in doses of about 1.5 mg dry weight activated anti-infection immunity to Listeria antigens and anti-tumour immunity to the ascitic form of mouse sarcoma S-180. Their probable target site is the macrophage. The Rothia-activated macrophages in human gingiva may take part in the pathogenesis of periodontal disease. Three models were employed to verify the immunostimulating properties of preventively administered Rothia dentocariosa bacterin-1) a spleen macrophage migration test, using mice immunized with Listeria innocua, with the soluble listeria Ei antigen as the antigenic signal, 2) determination of the increase in the Listeria monocytogenes LD50 for mice and 3) the prolongation of survival of mice carrying the S-180 tumour. In all three cases, the administration of Rothia bacterin stimulated the immune response to the later administration of other antigens. Furthermore, in the macrophage migration inhibition test, the chemotaxis of non-immune mouse macrophages was found to be stimulated. This gives evidence of the fact that Rothia bacterin has an activating effect on these macrophages.

[Monitoring natural killer cell activity in the blood of patients with malignant pleural effusion after intrapleural administration of Corynebacterium parvum].

In a group of 14 patients with a malignant pleural effusion treated by intrapleural administration of Corynebacterium parvum, in addition to other treatment of the generalized malignant process the activity of Nk cells was investigated before administration, on the 7th and 21st day after instillation of the vaccine. The mean rise of this activity was significant at the 5% level of significance. The activity of Nk cells was equally significantly influenced by age. The survival of patients, although longer in patients who responded positively, was not significantly correlated with the increased activity of Nk cells. Its increase was observed more frequently in the group of patients given the English preparation Coparvax. In the discussion some aspects of the relationship of Nk activity and anti-tumourous immunity are discussed.

[Comparison of the effectiveness and adverse effects of a Corynebacterium parvum vaccine made in Czechoslovakia with Coparvax, a British preparation made by Wellcome, in the treatment of malignant pleural effusions].

Malignant pleural effusion complicates various malignant diseases, most frequently lung and breast cancer. A group of 20 patients with this complication were treated with the Czechoslovak vaccine Corynebacterium parvum and the English preparation Coparvax, and their effects and side-effects were compared. Within 4 weeks following the application, 7 patients died. In 9 surviving patients, the exudate ceased to be replenished and in 4 persons, after intrapleural application of the preparation, it had to be evacuated one more time, but then its formation ceased. The therapeutical effect of both the Czechoslovak and English preparations was satisfactory. The side-effects of both preparations were comparable except for a significantly higher temperature elicited by the Czechoslovak vaccine. Both preparations Corynebacterium parvum are suitable for palliative treatment of malignant pleural exudates.

Fatty acid composition of a Propionibacterium acnes vaccine and its relationship to immunostimulatory activity.

The distribution of lipids of Propionibacterium acnes (Corynebacterium parvum) vaccine strain in chloroform-methanol, ethanol and light petroleum extracts was determined. Firmly bound lipids released by hydrolysis were also investigated. The petroleum extract differs from other lipidic fractions in its fatty acid composition. The presence of linolic, tuberculostearic and 10-ketostearic acids and branched fatty alcohols was observed in addition to previously described fatty acids of P. acnes. Changes in fatty acid composition during growth of the vaccine strain were determined and extremes were found at 95 h of cultivation. These extremes coincided with a maximum in the immunostimulatory efficiency as measured by the spleen enlargement test. The biological activity of static and stirred cultures of the vaccine strain was in correlation with their fatty acid composition.

Effect of vaccine prepared from Corynebacterium parvum on the growth of Gardner lymphosarcoma in C3H strain mice.

The present paper discusses the effect of a vaccine prepared with formol-killed Corynebacterium parvum (alias Propionibacterium acnes) on the growth of solid Gardner lymphosarcoma transplanted on C3H (H-2k) mice. The vaccine was injected at various time intervals prior to, simultaneously with, or following transplantation of the tumor to intact mice or mice immunized with 60Co-irradiated Gardner tumor cells. The effectivity was evaluated in terms of numbers of mice surviving without tumor 60-day observation period or prolongation of survival time. Reliable effectivity showed vaccine (0.8 mg dry weight of C.p.) in the mixture with tumor transplant or when introduced 6 days earlier into the site of tumor transplantation. The preventive action of the vaccine was more pronounced in the mice immunized with inactivated tumor. The vaccine applied to mice with a transplanted tumor was ineffective and failed to significantly prolong the survival time even in mice which had previously been immunized with an inactivated tumor.

Treatment of C3H mice bearing solid Gardner lymphosarcoma with vaccine prepared from Corynebacterium parvum combined with methotrexate and Alexan.

C3H mice after transplantation of solid Gardner lymphosarcoma were injected with Methotrexate (MTX) or Alexan. The prolonged survival time of mice treated with antimetabolites was in some cases evidently reduced after an additional injection of Corynebacterium parvum (C.p.). This reduction did not depend on the sequence of corynebacterial vaccine or MTX injections. Significantly better effect of combined treatment with MTX injection and C.p. was found only once in mice preimmunized with the tumor irradiated from 60Co gamma source.
