Long term results of stereotaxic surgery for infantile dystonia and dyskinesia.

Our experience is based on a series of 25 patients suffering from infantile dyskinesias and dystonias who underwent stereotaxic surgery of basal ganglia. We first note the immediate good results (77%) obtained by thalamo-sub-thalamotomy. Concerning dyskinesia this type of surgery enjoys in an opinion a place of choice. Secondly we have been able to estimate secondary deterioration on long term results in patients examined 2 to 15 years post-operatively. In contrast to various results reported in the literature good long term results are not superior to 50%. Some deterioration is noted in patients operated on for choreo-athetosis over the age of 20. A pallido-subthalamic lesion is efficient at the beginning of the disease course since it improves motor performance and thereby helps possibilities of intellectual acquirement. Bilateral lesions have after improved I.Q. Effects of this treatment specially on spasticity must be discussed among other types of surgery.

[Arthrodesis of the sub-occipital spine by screwed occipito-cervical plate. Clinical case and technical notes].

The authors report a new technique in arthrodesis of the sub-occipital spine which has been experienced since 1973 in case of metastases at CI-C2 level. An occipitovertebral metal plate, inverted Y shaped has been realised after anatomical and biomecanical studies. It is adapted to the occipito-vertebral curvature and screwed in the articular processes of C3 and C4 (14 mm screw), and in the pedicles of C2 (32 mm screw). At the upper part, the occiptal ridge offers fixation by 12 and 14 mm Muller's screw. In addition CI is attached to the plate, by a nylon thread. After the report of the first clinical case with a nine month period of follow up technical features of the material as well as practical problems are exposed. Tomograms of the cervical spine as well as bilateral brachial angiography are of paramount value. The latter investigation may reveal anomalies of the vertebral artery that can hinder of forbid the pedicular fixation. Finally the authors discuss the extension of this technique to complexe trauma of the suboccipital column.

[Mechanism of quenching of hematoporphyrin and mono-L-aspartyl chlorine e6 triplet states by metronidazole].

The quenching of the triplet state of hematopophyrin (3HP.) and mono-L-aspartyl chlorine e6, (3MACE.) by metrotlidazole (METRO) ground state, has been investigated in binary mixtures of water and aliphatic alcohol by means of 532 nm laser flash. In water, both 3HP. and 3MACE. quenched by METRO with at rate: kQw = 1.9 x 10(9) M-1.s-1. The quenching is accompanied by the appearance of a transient absorbance near 690 nm. This transient absorbance corresponds to the tetrapyrrole radical-cation formation in result of an electron transfer between 3HP. or 3MACE. and METRO. In neat alcohol, the electron transfer was not observed. In binary mixtures the quenching constant is reduced as the molar fraction of water is decreased. In water, the activation barrier of the quenching is 5.9 kcal/mol for HP and 3.9 kcal/mol for MACE. In going from water to water-alcohol mixtures the energy of activation is practically unchanged. Based on flash photolysis data, and analysis of absorption spectra of METRO we conclude that the electron-acceptor ability of metronidazole essentially depends on the intermolecular interaction with nearby solvent molecules. The mechanism of the quenching is discussed.
