Placental transfer of cefoperazone and sulbactam in the isolated in vitro perfused human placenta.

The transfer of cefoperazone and sulbactam across the human placenta was studied qualitatively and quantitatively with the isolated in vitro bidirectionally perfused human placental lobule. Clearance indexes for both cefoperazone and sulbactam were derived. The absence of active transport and facilitated diffusion were documented, implying simple diffusion as the mechanism of placental transfer for both drugs. Placental tissue levels were also studied.

The relationship between histological grade, oestrogen receptor status, events and survival at 8 years in the NATO ('Nolvadex') trial.

A pathological review was carried out on 600 patients with breast carcinoma entered into the 'Nolvadex' Adjuvant Trial Organisation (NATO) study. The tumours were graded histologically and these results were compared with the oestrogen receptor (ER) status of the tumours, the numbers of recurrences and the length of survival of the patients. It was found that histological grading was predictive both in terms of events and survival, and correlates significantly with oestrogen receptor status; within histological grades I and II, patients receiving 'Nolvadex' had fewer events and deaths compared with patients in the control group. For patients with grade III tumours qualitatively it was in the same direction as the benefit obtained in patients with grade I and II tumours.

Structural organization and functional analysis of centromeric DNA in the fission yeast Schizosaccharomyces pombe.

Centromeric DNA in the fission yeast Schizosaccharomyces pombe was isolated by chromosome walking and by field inversion gel electrophoretic fractionation of large genomic DNA restriction fragments. The centromere regions of the three chromosomes were contained on three SalI fragments (120 kilobases [kb], chromosome III; 90 kb, chromosome II; and 50 kb, chromosome I). Each fragment contained several repetitive DNA sequences, including repeat K (6.4 kb), repeat L (6.0 kb), and repeat B, that occurred only in the three centromere regions. On chromosome II, these repeats were organized into a 35-kb inverted repeat that included one copy of K and L in each arm of the repeat. Site-directed integration of a plasmid containing the yeast LEU2 gene into K repeats at each of the centromeres or integration of an intact K repeat into a chromosome arm had no effect on mitotic or meiotic centromere function. The centromeric repeat sequences were not transcribed and possessed many of the properties of constitutive heterochromatin. Thus, S. pombe is an excellent model system for studies on the role of repetitive sequence elements in centromere function.

Phorbol myristate acetate and dioctanoylglycerol inhibit transport in rabbit proximal convoluted tubule.

The present in vitro microperfusion study examined the effect of protein kinase C activation on transport in the rabbit proximal convoluted tubule (PCT). PCT were perfused with an ultrafiltrate-like solution and were bathed in a serumlike albumin solution. Addition of 5 x 10(-8) and 5 x 10(-7) M bath phorbol 12-myristate 13-acetate, an activator of protein kinase C, inhibited volume absorption from 1.06 +/- 0.10 to 0.77 +/- 0.07 nl.mm-1.min-1 (P less than 0.05) and 0.76 +/- 0.14 to 0.48 +/- 0.08 nl.mm-1.min-1 (P less than 0.01), respectively. Bath phorbol 12-myristate 13-acetate (5 x 10(-9) M) had no effect on volume absorption (Jv, 0.82 +/- 0.13 to 0.81 +/- 0.12 nl.mm-1.min-1). In contrast, 5 x 10(-8) M bath 4 alpha-phorbol, an inactive phorbol that does not activate protein kinase C, had no effect on Jv (0.95 +/- 0.14 to 0.94 +/- 0.11 nl.mm-1.min-1). Bath L-alpha-dioctanoylglycerol (10(-4) M), another known activator of protein kinase C, inhibited volume absorption from 0.96 +/- 0.08 to 0.71 +/- 0.08 nl.mm-1.min-1 (P less than 0.001). A 10-fold lower concentration of L-alpha-dioctanoylglycerol (10(-5) M) had no effect on Jv (0.81 +/- 0.18 to 0.78 +/- 0.17 nl.mm-1.min-1). Both 5 x 10(-8) M phorbol 12-myristate 13-acetate and 10(-4) M L-alpha-dioctanoylglycerol inhibited glucose, bicarbonate, and chloride transport in the PCT. These data are consistent with protein kinase C activation playing a role in the modulation of proximal tubular transport.

Effect of luminal chloride on cell pH in rabbit proximal tubule.

The present in vitro microperfusion study examined whether apical membrane chloride transport is mediated by chloride-base exchange in the rabbit proximal convoluted (PCT) and proximal straight tubule (PST) by examining the effect of the addition of luminal chloride on intracellular pH. Intracellular pH was measured fluorometrically using the pH-sensitive dye 2',7'-bis(carboxyethyl)-5,6-carboxyfluorescein. In PCT initially perfused without chloride, changing the luminal perfusate to a high chloride (148 mM)-low bicarbonate (5 mM) solution simulating late proximal tubular fluid produced a cell acidification (7.56 +/- 0.06 to 7.52 +/- 0.06, P less than 0.02) when 1 mM formate was present in the perfusate and bathing solution. This acidification was inhibited by 0.5 mM 4,4'-diisothiocyanostilbene-2,2'-disulfonic acid. This chloride-base exchange was not observed in the absence of formate, and neither acetate nor lactate produced the cell acidification observed with formate. Because the Na+-H+ antiporter could blunt a pH change, 2 mM amiloride was added to the luminal perfusate. While addition of luminal chloride produced a small cell acidification in the absence of formate (7.63 +/- 0.06 to 7.60 +/- 0.05, P less than 0.05), a much greater cell acidification was observed in the presence of 1 mM formate (7.69 +/- 0.05 to 7.58 +/- 0.06, P less than 0.01). Chloride-base exchange was only detected in the presence of formate in the PST. These studies demonstrate apical membrane chloride-base exchange in the presence of formate in the rabbit proximal tubule consistent with chloride-formate exchange.

Simplifying infusion chemotherapy: preliminary communication.

A low-intervention policy for Hickman catheter maintenance has been evaluated and found to be safe and cost effective. A simple, lightweight, disposable device has been used for 24-hour ambulatory home infusion. Implementation of this policy as part of a prospective randomized trial of single-agent chemotherapy in advanced breast cancer has demonstrated that slow intravenous infusion reduces the acute toxicity of epirubicin when compared with bolus injection.

[High frequency ventilation with a conventional respiratory following heart surgery interventions].

This study was designed to compare the effects of Continuous Positive-Pressure Ventilation (CPPV) and, by using the same unmodified conventional ventilator, High-Frequency Positive-Pressure Ventilation (HFPPVkonv). First, CPPV and HFPPVkonv were studied in a lung model with both normal (R = 5 mbar/1/second) and elevated (R = 20 mbar/1/second) resistance. Our results indicate that in HFPPVkonv the large compressible volume of the conventional ventilator did not influence lung model ventilation at normal resistance. The adjusted (300 ml) tidal volume (VT) and the measured volume of actual expiration (270 ml) were about the same (Fig. 1). However, with elevated resistance air trapping occurred. The large compressible volume influenced model ventilation during both CPPV and HFPPVkonv (Fig. 2). As a second step we evaluated the effects of HFPPVkonv on gas exchange, airway pressure, and hemodynamics in 12 patients (aged 43-69) postoperatively after elective cardiac surgery. After a period of stabilization at the intensive care unit every patient was first ventilated with CPPV. The ventilator settings were: VT = 10-12 ml/kg, inspiratory: expiratory ratio (I:E) = 1:2, frequency (F) = 12/min, V = 60 1/min, PEEP = 5 cm, FiO2 = 40%. After 20 min of CPPV baseline measurements were made (series I). Then the initial ventilator settings of CPPV were switched to HFPPVkonv, the conventional ventilator remaining unmodified. The settings were changed as follows: I:E = 1:3, F = 60/min, V = 120 1/min, PEEP = 5 cm, FiO2 = 40%. During 60 min of HFPPVkonv variables were measured first after 20 min (series II) and again after another 40 min (series III). Minute volume had to be doubled after changing from CPPV to HPFFVkonv to achieve eucapnia. As a result of the new ventilatory settings, VT and hold showed a significant decrease (P less than 0.01) (Table 2).(ABSTRACT TRUNCATED AT 250 WORDS)

Is there a role for perioperative adjuvant therapy in the treatment of early breast cancer?

Despite having been investigated for many years, the role of adjuvant perioperative chemotherapy in the treatment of early breast cancer has still to be fully defined. This paper reviews the early trials of perioperative cytotoxic therapy and overviews the two largest trials; the Scandinavian Adjuvant Chemotherapy Study Group (SACSG) Trial, which began in 1965 and recruited 1026 patients, and the Cancer Research Campaign (CRC) Adjuvant Trial with 2230 patients entered between 1981 and 1985. Overview analysis of these two trials clearly shows a small but highly significant increase in time to first event (P less than 0.001). Increased survival, although significant in the SACSG trial, has not yet been demonstrated in the CRC study, but at this stage of follow-up (median 2.5 years) this is not surprising.

Community outbreak of adenovirus type 7a infections associated with a swimming pool.

In July 1982, an outbreak of pharyngitis caused by adenovirus type 7a occurred among children in a small town in western Oklahoma. Predominant symptoms were fever and sore throat (by case definition), headache (83%), abdominal pain (64%), and conjunctivitis (51%). At least 77 persons were identified whose symptoms met the case definition for illness. Onsets of illness peaked during the week of July 5 to 12, and the outbreak resolved within three weeks. A systematic telephone survey of the town revealed that persons who had swum at the community swimming pool were more likely to be ill than those who had not (P less than .001). A second survey of families with season passes to the pool showed that among swimmers, illness was directly related to average number of hours of exposure to the pool each week (P less than .001 by chi-square for trend). In addition, swimmers who reported swallowing pool water were more likely to be ill (29 of 56, 52%) than persons who did not (ten of 41, 24%) (P = .01). Throat-swab specimens from five of seven ill persons (71%) grew adenovirus 7a compared with one of 12 well persons (8%) (P = .01). The pool chlorinator had reportedly malfunctioned during early July. The outbreak resolved with proper operation of the chlorination system. Swimming pools continue to be a potential source of community-wide outbreaks of adenovirus infections.
