Level of nutrition and visceral organ size and metabolic activity in sheep.

Thirty-two crossbred wether lambs (initial live-weight 31 kg) were fed on a diet (metabolizable energy (ME) 12.8 MJ/kg) ad lib. (ADLIB) or restricted to maintain body-weight (MAINT) for a 21 d period. On days 0, 7, 14 and 21, four lambs per treatment were slaughtered, visceral organs weighed and tissues sampled. During the 21 d period, ME intake in ADLIB lambs increased quadratically with an average rate of live-weight gain of 425 g/d. In MAINT lambs, live weight (30 kg) was maintained, and daily ME intake (kJ/kg empty body-weight (EBW)0.75) declined (P less than 0.01) quadratically with time. Weights of liver, stomach and small intestines as a percentage of EBW were increased in ADLIB lambs and decreased by 10-33% in MAINT lambs (treatment x day, P less than 0.01). In vitro liver oxygen consumption was not affected by level of feed intake. Estimates of whole-liver O2 consumption (mmol O2/d per kg EBW) increased in ADLIB lambs and were relatively constant in MAINT lambs. These findings suggest that level of feed intake changes the relative proportion of visceral organs to body mass. In addition, the effect of level of feed intake on changes in the relative contribution of visceral organs to whole-body metabolic rate appears to be primarily a result of differences in organ size rather than tissue-specific metabolic activity.

Skinfold thickness measurements in obese subjects.

To determine the accuracy of skinfold thickness measurements in body composition assessment, skinfold thicknesses, bioelectrical impedance, body density by underwater weighing, and total body water by deuterium dilution were determined in 105 healthy adults who varied widely in body fatness. Body fat ranged from 2.9% to 61.2% of body weight and a substantial number of obese subjects was included. The correlation between body fat calculated from skinfold equations and body fat measured by underwater weighing was in the same range as that between body fat estimated from impedance and underwater weighing. However, body fat calculated both from skinfold equations and bioelectrical impedance tended to be underestimated compared with body fat calculated from underwater weighing and total body water in the most obese subjects. In obese subjects it was not possible to measure significant numbers of skinfold thicknesses because of the inadequate size of the calipers.

[Differential breast diagnosis using CW-Doppler ultrasound].

There is no method that allows satisfactory differentiation between benign and malignant breast lesions. We were able to differentiate 94% of palpable malignant tumours with CW-doppler ultrasound, which helps to increase the efficiency of clinical examinations, breast sonography and mammography. Two hundred patients were examined with an 8 MHz CW-doppler probe. The symmetry of arterial vessels and flow velocity in normal breasts is very high. Carcinomas appear as areas with localized flow increase. Only in two normal patients was a minimal focus of two arteries found.

[Psychiatric disorders in immigrants. I. History, symptoms and diagnostic classification].

155 people who had left East-Germany and sought psychiatric help within six weeks after their arrival in West Berlin, were examined. History, living situation and psychopathological symptoms were studied. The disorders were diagnosed according to ICD-9 and DSM-III-R. 85% of the patients reported that they had already suffered from similar complaints in East Germany. 50% stated they have had symptoms before they had made the decision to leave. On average, that decision had been taken 22 months before the actual leaving. Most often patients complained about sleep disturbance, nervousness, and headaches. According to ICD-9, 55% of the disorders were classified as reactive and 39% as neurotic or personality disorders. The most frequent diagnoses according to DSM-III-R were adjustment disorders (41%), major depression (21%), anxiety disorders (16%), and dysthymia (14%). Regardless of diagnosis most patients were found to have symptoms of anxiety and depression associated with vegetative complaints. There were no clear relationships between psychopathological symptoms and data of history or present living situation.

[Esthesioneuroblastoma--treatment of recurrences using the remote-control afterloading technic. A case report].

At the Radiological Hospital of the Heidelberg University, one patient with a local recurrence of an esthesioneuroblastoma situated on the left-hand side of the skull base was treated between February 11 and March 18, 1986, by an afterloading contact therapy with curative intent. After preceding surgery and percutaneous radiotherapy, repeated local recurrences and locoregional lymph node metastases had occurred. The contact therapy was performed after adjusting an individual face mask in which the afterloading probe was placed and brought into its position in the tumor region. The irradiation scheme was based on the transformation of the source coordinates form the stereoscopic X-ray localization system into the coordinate system of the computed tomogram by means of X-ray tight reference points. The irradiation planning by computed tomography allows to optimize the arrangement of sources within the tumor region. The principles of the method as well as the treatment result are presented.

[Community psychiatry in Offenbach--a short-cut for local psychiatric service].

The question as to whether psychiatric units in district general hospitals can replace the existing large mental hospitals, is still controversial. Recently, this issue was discussed in a governmental report on the reform of the psychiatric services in the Federal Republic of Germany. It has been shown by the example of the community-orientated comprehensive mental health care system of the city of Offenbach, that a psychiatric unit of 80 beds is capable of handling and managing the district responsibilities of a defined area of 110.000 inhabitants, provided qualified and sufficient complementary and outpatient service facilities are available, especially for the chronically ill patients and their families.

Brain graft surgery: a new treatment for Parkinson's disease.

Brain graft surgery remains controversial, and the beneficial effects, though promising, are mixed. For those patients who are eligible, it offers new hope for an alternative to early disability and family hardship. Outlined here are the etiology, pathophysiology, and manifestations of Parkinson's disease, plus the surgical procedure and postoperative nursing care.

Prevalence of Helicobacter pylori infection and histologic gastritis in asymptomatic persons.

We estimated the prevalences of Helicobacter pylori (formerly called Campylobacter pylori) infection and histologic gastritis in 113 asymptomatic persons, using endoscopic biopsy of the gastric antrum and corpus. Unsuspected lesions, mainly mucosal erosions, were revealed at endoscopy in 16 subjects (14 percent). Gastritis was found in 42 subjects (37 percent), of whom 36 (32 percent of the total) were found to be infected with H. pylori on the basis of hematoxylin-eosin staining. H. pylori was not found in any of the 71 subjects with normal histologic features. Gastritis and H. pylori were noted in both the antrum and corpus in 75 percent of those infected (n = 27). The prevalence of H. pylori infection increased from 10 percent (2 of 20 subjects) in those between the ages of 18 and 29, to 47 percent (7 of 15) in those between the ages of 60 and 69, but the effect of age did not reach statistical significance. The prevalence of gastritis increased significantly with advancing age. Stepwise logistic regression analysis revealed that the relative risk for H. pylori infection associated with recent (within six months) antibiotic use was 5.8 (95 percent confidence interval, 1.5 to 22.1), whereas the relative risk was 6.5 (95 percent confidence interval, 1.4 to 29.2) for those who had never used bismuth compounds. We conclude that histologic gastritis and H. pylori infection commonly occur in the stomach of apparently normal persons and increase in prevalence with advancing age. All the subjects with H. pylori infection had gastritis, suggesting a possible etiologic role for the bacterium in the histologic lesion.

Natriuretic action of central angiotensin II in conscious rats.

The effects of intracerebroventricular (i.c.v.) injections of angiotensin II (ANG II, 10 pg, 100 pg and 10 ng) on renal sodium excretion were investigated in conscious rats instrumented with a chronic urethral catheter. ANG II increased renal sodium excretion dose-dependently with a threshold i.c.v. dose of 10 pg. Only after the highest dose was a concomitant increase in arterial blood pressure and urinary flow observed. The ANG II-induced natriuresis began within 5 min of the i.c.v. injection and lasted for more than 1 h. The angiotensin receptor antagonist saralasin (1 ng, i.c.v.) largely prevented the natriuretic effect of i.c.v. injected ANG II (100 pg). Our results lend further support to the hypothesis that brain ANG II by its potent natriuretic actions may be instrumental in central osmotic control.

Pharmacokinetics of ranitidine in geriatric patients with multiple diseases.

Ranitidine pharmacokinetics was measured after intravenous administration of 50 mg of the drug in 18 geriatric patients suffering from multiple diseases. Elimination half-life was prolonged (3.05 +/- 0.58 h) and total clearance was reduced (282 +/- 97 ml/min) as compared to the results of studies on normal younger persons by other investigators. In comparison to other studies on healthy elderlies, the pharmacokinetic parameters of ranitidine were only slightly different. Statistical analyses showed correlations of pharmacokinetic parameters of ranitidine with creatinine clearance, serum creatinine, uric acid concentration and alkaline phosphatase activity. These results disclose that reduced creatinine clearance or serum creatinine concentration in the upper normal range, for example, might serve as indicators for a possible dose reduction of ranitidine to prevent unnecessary overmedication in elderly patients.

Radiation therapy alone or combined with misonidazole in the treatment of locally advanced non-oat cell lung cancer: report of an RTOG prospective randomized trial.

From September 1979 to February 1983, 268 patients with unresectable, locally advanced (RTOG Stage III), non-small cell lung cancer were randomized to receive radiation therapy alone (RT) (50 Gy large field and 10 Gy boost), or combined with misonidazole (400 mg/m2 2-4 hr prior to RT daily for 5-6 weeks to a maximum dose of 12 g/m2 or until tumor progression). One hundred twenty-three patients who received irradiation alone and 116 given RT + misonidazole were evaluable for toxicity, time to tumor progression, and survival as of April 1987. The distribution of patient characteristics was similar in both treatment groups; 59% of the patients had a Karnofsky score of 90 or better, 53% had adenocarcinoma or large cell tumors, and 47% had Stage T3 tumors. Complete tumor regression was reported for 33 (27%) patients treated with radiation therapy alone and 24 (21%) who received misonidazole + RT. Median survival was 8 months with RT alone and 7.4 months with misonidazole + RT. Ninety-five percent of the patients have died. Seventy percent of the patients treated with radiation alone and 77% of those treated with misonidazole + RT died of progressive disease. Three patients treated with radiation alone and two with RT + misonidazole died subsequent to radiotherapy-related pneumonitis or pulmonary fibrosis. There was no significant improvement in response rates, local control, or survival for patients who received daily misonidazole along with irradiation compared with patients treated by irradiation alone.

[Gangliocytic paraganglioma of the duodenum].

In a 49-year-old patient who was investigated because of abdominal complaints we found a polypoid tumor in duodenum near the papilla of Vater. The histological investigation after endoscopical polypectomy showed the rare case of a gangliocytic paraganglioma. After complete endoscopical removal no further intervention is necessary.

[Tubal manometry: the question of normal values, values in chronic mesotympanic purulent middle ear infections].

We investigated tubal function by equalization manometry in 68 patients with unilateral traumatic perforation of the tympanic membrane, and in 116 patients with chronic mesotympanic suppuration (85 dry and 31 discharging ears). Our criterion of normal tubal function is the ability to equalize a difference in pressure of +/- 30 hPa by means of at most 7 swallowings. This result was achieved in only 25% of the dry perforations and in none of the patients with discharging ears. Normal tubal function may only be expected in 25% of patients with chronic otitis media.

[The radiobiology of gynecologic HDR and LDR afterloading technics].

In brachytherapy the advantages of high dose rate over low dose rate afterloading therapy are obvious. Outpatient treatment becomes possible, the position of the sources is reproducible and can be observed during the treatment, and the patients have to be immobilised for only a short time, giving less psychological stress and a decreased risk of thrombosis and embolism. When changing from LDR to HDR afterloading therapy we are not yet able to evaluate its biological impact. Radiobiological considerations and our experimental data, however, give us the following clinical consequences by using HDR brachytherapy: There is a need for about 15 fractions or more and each increase in dose rate requires higher fractioning. Due to the steep dose rate decline and the inhomogeneous dose distribution, multiple equivalence factors are necessary when fractioning is not sufficiently high. Correction factors to reduce the dose close to the source are low, with increasing distance from the source they increase. If HDR radiation therapy is used, the percutaneous dose in the pelvic wall region should be reduced. The reduction of the dose in HDR brachytherapy is a compromise to limit the side effects caused by the radiation. The drawback is a small therapeutic range and reduced therapeutic efficacy at the tumour.
