Lithium transport pathways in human, chicken and eel erythrocytes.

The contribution of four transport pathways to Li+ influx in human, chicken and eel erythrocytes was studied. All pathways were measured simultaneously in each kind of blood sample to avoid possible temporal variations in transport parameters. We found that: 1) Li+ influx via Na(+)-K+ pump increased 2-3 fold in the order human < eel < chicken; 2) by the countertransport exchange system lithium influx increases its variance between 15% and 35% when saline medium is replaced by choline Cl; 3) the anion exchange system (band 3 protein) shows very little interindividual variability on the lithium influx; 4) the lithium leak pathway, for human and eel red blood cells, is the major contribution, whereas it is negligible for chicken erythrocytes. It is concluded that a similar transport system exists in the red cell membranes of the three species which can transport lithium. However, the exchange system does not exhibit identical transport characteristics in the three species and shows a marked inter- and intra-species variability in maximum transport capacity and some differences in susceptibility towards the inhibitors.

Lowering body temperature with a cooling suit as symptomatic treatment for thermosensitive multiple sclerosis patients.

A cooling system (Mark VII Microclimate System) was used to give six thermosensitive multiple sclerosis patients two 45-minute daily coolings for a period of one month. Before the first cooling, a baseline clinical and electrophysiological examination was performed. The same tests were repeated after the first application and after the thirtieth cooling day, thus providing information relating to acute and chronic efficacy. A clinical improvement was observed after both acute and, more unexpectedly, chronic cooling, whereas a significant improvement in central somatosensory conduction was recorded only under acute conditions. Our data suggest that cooling with this device leads to an improvement in some functional performances (mainly fatigue and strength) of about two hours' duration in thermosensitive patients.

NEUREX: a tutorial expert system for the diagnosis of neurogenic diseases of the lower limbs.

Specialist tutors have to transfer two types of knowledge to doctors who are specialising in a particular clinic: public declarative knowledge, including facts, notions, principles in that particular field; and their own private procedural knowledge acquired in years of direct experience. Embodying this knowledge into an expert system means that this information can be shared more rapidly, and tutoring is easier. This paper presents a tutorial expert system for neurological clinics which can emulate the diagnostic process of an expert neurologist for neurogenic diseases of the lower limbs, assist users in planning the optimal sequence of NG and EMG tests, interpret the results of these tests, and help users to achieve the most suitable diagnosis.

A family study of schizotypal disorder.

Direct, blind interviews were used to study the risk for and prevalence of DSM-III-R Axis I and II disorders in 93 first-degree relatives of outpatients with schizotypal personality disorder (SPD) and outpatients with other personality disorders. Risks for SPD (at a slightly loosened diagnostic threshold) and schizoid personality disorder were significantly higher in the families of probands with SPD. Schizophrenia was present only among relatives of probands with SPD, accounting for a morbid risk of 4.1 percent. Neither familial risks for mood and anxiety disorders nor the prevalence of other Axis II disorders significantly differed in the two groups of relatives. It is suggested that SPD is a familial disorder representing a phenotypic expression of liability to schizophrenia.

Age at onset of panic disorder: influence of familial liability to the disease and of childhood separation anxiety disorder.

OBJECTIVE: The authors investigated the relation of age at onset of panic disorder to liability to panic disorder and agoraphobia. METHOD: Two hundred thirty-one outpatients with panic disorder were compared with 131 surgical outpatients on demographic variables and familial risk of psychiatric disorders. The distribution of patients' ages at onset of panic disorder and several covariates were entered in a stepwise survival analysis. RESULTS: The patients with panic disorder had a significantly higher rate of childhood separation anxiety disorder and higher familial risks of panic disorder/panic disorder with agoraphobia and alcoholism. A family history of panic disorder with agoraphobia and the presence of childhood separation anxiety disorder influenced age at onset of panic disorder. CONCLUSIONS: Age at onset of panic disorder may reflect genetic penetrance, and separation anxiety disorder may be an individual predictor of earlier onset of panic disorder.

Subclinical impairment of lung airways in patients with panic disorder.

Lung function was assessed in 17 panic patients and 20 healthy controls. Panic patients had abnormal values for some dynamic lung volumes, namely Peak Expiratory Flow Rate (PEFR), Expiratory Flow at 75% of Vital Capacity (FEF75) and Maximum Mid-Expiratory Flow Rate (MMEF). Such functional abnormalities might indicate subclinical obstruction of lung airways, possibly relevant to the mechanisms related to panic disorder (PD).

Carbon dioxide/oxygen challenge test in panic disorder.

The effects of a single inhalation of a 35% CO2/65% O2 gas mixture were examined in 71 patients with panic disorder with or without agoraphobia and 44 normal control subjects. Compared with the placebo condition, inhalation of air, the CO2/O2 mixture elicited a clear anxiety reaction only in panic disorder patients, who experienced a sudden rise of subjective anxiety as well as of several panic symptoms. Respiratory symptoms and the fear of dying best distinguished the patients from the control subjects. Baseline anxiety was not the key factor in explaining this differential reaction. The clinical features of panic disorder (namely, frequency of panic attacks, agoraphobia, anticipatory anxiety, and duration of illness) were not significantly related to the response to the challenge test, suggesting that CO2 reactivity might be a trait marker of panic disorder.

An assessment of the Wisconsin Card Sorting Test as an indicator of liability to schizophrenia.

In order to test the hypothesis that a poor performance in the Wisconsin test (WCST) may be an indicator of liability to schizophrenia, we compared the WCST performances of patients with DSM III-R schizophrenia, normal controls, and patients with schizotypal personality disorder (SZT PD). While schizophrenic patients performed significantly worse than subjects in the other two groups, schizotypal and normal subjects showed no significant differences in the WCST execution. Moreover, patients with SZT PD with or without positive family history for the schizophrenic spectrum had similar WCST performances. Our observations are in keeping with other studies employing the WCST in paradigms of heightened liability to schizophrenia, and suggest that a poor performance in the test is more probably a feature of the disease process, than a trait marker of vulnerability to the illness demonstrable in high-risk subjects.

Reversal of chlorpromazine-induced avoidance depression by the N-methyl-D-aspartate antagonist, dizocilpine, in mice.

The non-competitive N-methyl-D-aspartate (NMDA) antagonist MK-801 (dizocilpine) was tested, alone or in combination with chlorpromazine, in mice previously trained in the shuttlebox. The lowest doses of dizocilpine (0.02 and 0.04 mg kg-1) attenuated the disrupting action of the neuroleptic (1.5 mg kg-1) on avoidance-performance, while avoidance depression induced by 1.5 and 2 mg kg-1 chlorpromazine was completely or almost completely reversed by 0.08 mg kg-1 NMDA antagonist. The highest dose (0.16 mg kg-1) of dizocilpine did not ameliorate avoidance-performance of mice receiving 2 mg kg-1 chlorpromazine, perhaps because of ataxic effects produced by the drug combination, at these doses. The results support suggestions for a potential use of NMDA antagonists in the treatment of extrapyramidal side-effects of neuroleptics.

Effect of caffeine and nicotine on avoidance learning in mice: lack of interaction.

Tested alone, nicotine (0.25 or 0.5 mg kg-1) improved shuttle-box avoidance learning in mice of the CD-1 strain. Caffeine had no effect at doses of 2.5 and 5 mg kg-1 and impaired performance at a dose of 10 mg kg-1. Combinations of the two drugs did not increase avoidance responses more than nicotine alone, nor was nicotine able to attenuate performance depression induced by the highest dose of caffeine. Lack of drug interaction in the avoidance test contrasts with the occurrence of interactive effects of the two drugs in a locomotor activity test. When given in combination, caffeine and nicotine increased locomotor activity at doses ineffective by themselves. The results seem to indicate no advantage in combining caffeine and nicotine to improve active avoidance learning.

Hoechst 33258 staining for detecting mycoplasma contamination in cell cultures: a method for reducing fluorescence photobleaching.

DNA fluorochrome staining with Hoechst 33258 bisbenzimide is commonly used for detection of mycoplasma contamination in cell cultures. Photobleaching of Hoechst 33258 is pronounced under the conditions of intense illumination, high magnification and resolution required for detection of mycoplasmas. To reduce photobleaching we investigated the effects of some antioxidant molecules, p-phenylenediamine (PPD), n-propyl gallate (NPG) and 1,4-diazabicyclo(2,2,2)octane (DABCO), which are known to reduce the fading rate of fluorescein. Mycoplasma-contaminated cell monolayers were stained with Hoechst 33258 and mounted in glycerol containing different amounts of antioxidant additives. The cells were examined in an epifluorescence microscope, and the emitted light intensity was recorded. Results showed that PPD and, to a lower degree, NPG, retarded the photobleaching of Hoechst 33258-stained cells, whereas DABCO was not effective. However, fluorescence half-life was increased about three-fold by NPG and almost 20-fold by PPD. The rate of fluorescence fading of Hoechst 33258 can therefore be retarded by PPD, with obvious advantages for reading and photographic recording of results.

[Use of computerized tomography in asthmatic patients].

This study was aimed at assessing the role of high resolution Computed Tomography (HRCT) in the asthmatic patient, during disease worsening. Chest radiography plays a major role in the assessment of the most common complications of asthma, such as pneumothorax, pneumomediastinum, or lobar collapse. Conventional CT and HRCT are sometimes used when a complication is suspected, particularly chronic eosinophilic pneumonia and allergic bronchopulmonary aspergillosis. We prospectively examined 31 asthmatic patients with clinical and functional worsening submitting them to chest radiographs and HRCT scans. In 5 of them, radiographic findings were suggestive of a complication: 3 patients presented allergic bronchopulmonary aspergillosis and 2 chronic eosinophilic pneumonia; in these cases HRCT confirmed the diagnosis and yielded other useful pieces of information, but did not change the prognosis or the therapeutic approach. In the remaining 26 patients, HRCT findings were abnormal in 61% of cases, while chest films were abnormal in 26% of cases only. An outstanding diagnostic contribution came from HRCT which demonstrated a high incidence of small airway disease and, above all, cylindrical bronchiectasis (53.8%), especially in the upper lobes. HRCT also demonstrated that asthma does not usually cause emphysematous destruction in the patients that never smoked. Even though our study enrolled only a small number of patients, the results suggest that asthma is a more destructive disease than previously believed and that HRCT could be a useful examination to perform even when chest film findings are normal.

[Diltiazem helps perfusion of the kidney after transplantation: intraoperative assessment using echo-color Doppler].

Diltiazem is a dihydropyridinic calcium-antagonist which acts an arterial muscle, producing an increase in vascular capacity. We administered it intraoperatively in 2 patients subjected to kidney transplantation, to asses the maximal vascular reserve of the transplanted organ. After 2' and 10' from administration of Diltiazem, Color Doppler Ultrasound showed a 30% increase in mean flow in the renal artery and 13% reduction in the intraparenchymal resistance rates with respect to the basal parameters. Systemic arterious pressure remained unchanged. This result was associated with an improvement in the perfusion of the transplanted organ.

[Transrectal echography of the prostate in the long-term follow-up of prostatic carcinoma in advanced stage].

In the last eight years, 60 patients with carcinoma of the prostate in an advanced stage underwent transrectal ultrasound examinations of the prostate both before and after treatment with LHRH analogues, alone or associated with antiandrogens. Mean follow-up was 30 months (9-96 months) and mean survival 33 months (9-81). In all patients, the volume and echogeneicity of the prostate were assessed before treatment and once every three months thereafter. A significant reduction in volume was observed in 91% of the cases (54/60), while a variation in the echogeneicity was noted in 80% of the patients (48/60); these data do not seem to have been affected by the type of treatment administered. The greatest reduction in volume occurred during the first three months, after which a virtually constant volume was observed. Metastatic progression occurred in 29 of the 60 patients assessed, whereas no increase in prostate volume was demonstrated by ultrasound in 18 of these patients. These data, reported by other authors, too, may indicate that transrectal ultrasound examinations alone cannot constitute a complete follow-up in prostatic carcinoma: they can only show local progression and should therefore be associated with other diagnostic methods, in particular the specific prostate antigen.

[High-resolution computed tomography study (HRCT) in so-called emphysema B. The correlations between the radiogram, the anatomical findings yielded by high-resolution computed tomography and the clinico-functional assessment].

The results are reported of a prospective study on 28 emphysematous patients with clinical, functional and radiologic correlations. Thirteen of 28 patients had type-B, the clinical "blue and bloater", emphysema. The grade of emphysema was investigated on conventional chest films and on HRCT scans, with a visual scoring system. Hyperinflation was assessed on conventional chest films and correlated with the extent of emphysema on HRCT images and with functional impairment. In our series, the extent of emphysema in B-type patients was markedly lower than in other patients of the same age, with the same smoking habits, dyspnea and functional impairment. Moreover, the incidence of centrilobular involvement was higher, though not statistically significant. The B-type exhibited more severe dilatation of subsegmental pulmonary arteries (76.9% vs. 20% p < 0.001) and a striking incidence of small airway disease (84.6% vs. 26.6%, p < 0.002). This findings may explain the radiographic pattern of "increased vascularity" seen on chest films. Bronchiolectasis and small airway wall thickening are much more frequent in type-B emphysema, together with patchy areas of ground-glass opacity and small peripheral nodules. A close correlation was observed between signs of small airway disease on HRCT images and functional clinical impairment. Thus, the small airway disease might eventually prove to be a more critical factor in causing functional and hemodynamic impairment in B-type emphysema than the actual extent of centrilobular emphysema.
