Comment on the prune belly syndrome: a 11-week fetus with megacystis.

An 11-week human fetus with megacystis, prostatic dysgenesis, and lateral displacement of the abdominal muscles is described. We suggest that a subtle outflow obstruction of the very early bladder may give rise to both bladder dilation and bladder wall dysgenesis. The bladder dilation may produce abdominal muscle dysplasia or atrophy and almost certainly produces dilation of the prostatic urethra. The dilation of the prostatic urethra disrupts the formation of the prostate.

An exploration of the role of hydroxyurea injection time in fetal growth and teratogenesis in rats.

Pregnant Wistar rats were injected with hydroxyurea (HU) intraperitoneally (IP) t one of several 6-hour intervals on days 9-10.75 to study the role of circadian growth variations in the teratologic response of the fetuses. Two stocks of rats were studied and the results in each compared. The fetal response to HU, as observed at day 21, was not detectably modified by circadian fetal growth variation. No correlations between hour of HU administration and fetal weight, placental weight, resorption, or total malformation rates were found. Cyclic variations in the incidence of hydronephrosis and left umbilical artery was observed, but it was not clear that these were related to maternal light:dark cycles. Difference of response between two stocks of rats included marked variation in the incidence and type of malformations and variations in the timing of peak incidence for some but not all malformations.

Pediatric health aspects of PBBs.

Many Michigan farm children are viewed as having suffered a deterioration in health coincident with contamination of dairy cattle by PBBs. Among Wisconsin dairy farm children such a deterioration was a rare event. There is a suggestion that Michigan farm children who had multiple symptoms during the 1973-76 period are getting better. A discrete syndrome of ill health has not been identified among Michigan farm children. The symptoms complained of indicate a disruption of various physiological functions, including neurobehavioral, gastrointestinal and immunological. Children from quarantined farms do not have a higher prevalence of multiple symptoms than those from nonquarantined farms.

Phytohemagglutinin-induced lymphocyte transformation in humans receiving delta9-tetrahydrocannabinol.

Eight otherwise healthy male chronic marijuana smokers were hospitalized for a period of 30 days. Initially they received placebo, then a sustained dose of 210 milligrams of delta9-tetrahydrocannabinol (delta9-THC) per day for 18 days, followed by placebo. Lymphocyte responses to phytohemagglutinin were examined during each of these periods. Neither the daily ingestion of marijuana extract containing 210 milligrams of delta9-THC for 18 days nor the history of chronic marijuana smoking had a depressive effect on the lymphocyte responses of these subjects to phytohemagglutinin.

Warfarin-associated embryopathy in a 17-week-old abortus.

Histological and anthropometric studies were made of the hands and face of a 17-week-old (postmenstrual) human fetus that was exposed to warfarin throughout gestation. The fetus had marked nasal hypoplasia and the facial cartilages contained discrete foci of abnormal cartilage. When compared with age-matched control fetuses hand pattern profile analysis showed that the brachydactyly was due to shortening of the terminal phalanges. Histologically chondrogenesis was disordered in the carpals and phalanges, with the most deviant development occurring at the distal ends of the terminal phalanges. In none of the areas of abnormal cartilage was there evidence of hemorrhage or hemosiderin deposition. It is our contention that warfarin is teratogenic in human beings and that its teratogenicity is not due to focal hemorrhage.

Tracheal aspirate lecithin/sphingomyelin ratios as predictors of recovery from respiratory distress syndrome.

In thirty infants with severe respiratory distress syndrome who required endotracheal intubation, serial tracheal aspirate lecithin/sphingomyelin ratios were determined. There was 19 survivors and 11 nonsurvivors. The L/S ratios were plotted as a function of postnatal age; the ratios of the survivors demonstrated a significant increase between 24 and 96 hours. Nonsurvivors did not show this change. There was a significant difference on Days 3, 4, and 5 between survivors and nonsurvivors in the percentage of L/S ratios greater than 2.5. Daily mean L/S ratios correctly identified the outcome of 23 of 24 infants alive on Days 4 and/or 5 and were a more accurate prediction of survival than a single L/S ratio. The changes in the L/S ratios of the tracheal aspirates were not related to gestational age.

Spontaneous pleural effusion in a human fetus.

Left sided pleural effusion was found in a human fetus spontaneously aborted at 20 weeks postmenstrual. Pulmonary hypoplasia associated with the effusion suggested that the fluid had been present for some time before delivery. Absence of other anomalies supports the view that the effusion was due to maldevelopment of the thoracic lymph channels.

Adipose tissue metabolism in essential fatty acid deficienty. Effects of prostaglandin e1, epinephrine, and ACTH.

In an effort to better define some of the metabolic changes that accompany essential fatty acid deficiency (EFAD), we studied glucose metabolism in adipose tissue of EFAD and normal mice under basal conditions and in the presence of prostaglandin E1 (PGE1), epinephrine, and ACTH1-18. Isolated fat cells were incubated in Krebs-Ringer bicarbonate medium containing glucose 1(-14C) or 6(-14C), and the incorporation of radioactive carbon into CO2, total fat, fatty acids, and glyceride-glycerol was determined. It was found that EFAD increased glucose uptake over controls which could be attributed to increased oxidation to CO2 and fatty acid synthesis. The contribution of the pentose cycle to glucose oxidation was 50-80% higher in EFAD adipocytes as compared to controls. ACTH1-18 (0.1 mug/ml) suppressed this by 18 and 30% in the control and EFAD groups, respectively, while epinephrine decreased pentose cycle activity by 83 and 55% in the two groups, respectively. PGE1 alone had no significant effect, but in combination with epinephrine it abolished the inhibitory action of the catecholamine in both groups. It is suggested that, although EFA serve as prostaglandin precursors, the effects of EFAD on the metabolism of fat cells cannot be reversed by PGE1, in vitro. However, these fat cells retain their responsiveness to the action of both lipolytic (ACTH and epinephrine) and antilipolytic (PGE1) agents.
