Dioxins in U.S. food and estimated daily intake.

Congener-specific analyses for polychlorinated dioxins and dibenzofurans were performed on eighteen dairy, meat, and fish samples obtained from a supermarket in upstate New York. Dioxin toxic equivalents (TEqs) on a whole or wet weight basis for these food samples ranged from 0.02 to 1.5 parts per trillion (ppt). Data on American food consumption rates were used to estimate a U.S. range of dioxin intake from food. Based on these first congener-specific dioxin analyses of U.S. food, U.S. average daily intake of "International" dioxin toxic equivalents for an adult weighing 65 kilograms (kg) was estimated to be between 18 to 192 picograms TEq or 0.3 to 3.0 picograms per kilogram of body weight. Our previous results for cow's milk, human breast milk, and soy-based infant formula were used to calculate intake for infants. The relatively high level of dioxins commonly reported in human breast milk from America contributes from 35 to 53 picograms of dioxin toxic equivalents per kilogram of body weight to the nursing infant per day in its first year of life. This value greatly exceeds the U.S. Environmental Protection Agency estimated dose which would lead to one excess cancer per one million people of 0.006 pg of TCDD per kg of body weight per day over a seventy year lifetime or similar reference doses used by some other countries of up to 10/pg/kg/day over a 70 year lifetime.

PCDD/PCDF in humans, a 1993-update of background data.

70 blood samples--collected in 1993 in Germany--have been analysed for PCDD/PCDF. For all persons involved, no certain expose, except for food consumption, has been reported. The levels measured show a decrease of PCDD/PCDF compared to previous data on blood collected mainly in 1989 and in 1992. This trend for the dioxin values in blood corresponds well to the observations reported for human milk.

Measurement variability of carotid atherosclerosis: real-time (B-mode) ultrasonography and angiography.

To quantify the within- and between-reader agreement of carotid B-mode ultrasonography and angiography interpretation, images from 117 patients examined by both modalities were read multiple times. Angiographic measurements were more reproducible than those of B-mode scans for all parameters except lesion width, but variations for B-mode scan measurements were similar to those for angiographic measurements. Within-reader agreement on the presence of ulceration was substantial for both modalities, whereas between-reader agreement was poor for B-mode scan and only moderate for angiography.

Total parental nutrition using conventional and medium chain triglycerides: effect on liver function tests, complement, and nitrogen balance.

Conventional long chain triglyceride (LCT) was compared with a new emulsion containing 50% medium chain triglyceride (5% MCT/5% LCT) in a randomized cross-over trial of 10 days duration. Plasma concentrations of albumin, prealbumin, the complement components C3 and C4, and prothrombin times measured daily at 8 am, before lipid infusion, showed no progressive change during the 10 days of the trial, nor in each separate 5-day period when LCT or MCT/LCT was infused. Aspartate transaminase and alkaline phosphatase activities were similar over the two periods. There was a significant increase (compared with preinfusion levels) in C3 and C4 levels after 5 hr of either lipid infusion. Nitrogen balance was improved, and plasma bilirubin levels were lower on the regimen containing MCT/LCT.

Dietary management of patients with familial hypercholesterolaemia treated with simvastatin.

The effect of diet on plasma lipids and lipoproteins was examined in 19 patients with familial hypercholesterolaemia treated with the HMGCoA reductase inhibitor, simvastatin. The study was a randomized double-blind cross-over trial of two diets: a low fat diet and a higher fat diet, and was performed to determine whether ongoing attention to diet is necessary for patients receiving this drug for the treatment of familial hypercholesterolaemia. Plasma cholesterol concentration was 6.5 per cent lower on the low fat diet than on the high fat diet. The difference was within the range predicted by the Jacobs modification of the Keys formula from the reported intake on the two diets. The fall on the low fat diet was principally accounted for by a reduction in low density lipoprotein cholesterol, although high density lipoprotein cholesterol and apolipoprotein A1 also fell. Patients taking HMGCoA reductase inhibitors for the treatment of familial hypercholesterolaemia respond to changes in dietary fat intake. They are thus likely to achieve further reduction in their coronary heart disease risk, and should remain on a low saturated fat low cholesterol diet even when taking these powerful lipid lowering drugs.

[Red grain mycetomas (A. pelletieri). Apropos of 20 new cases in Dakar].

20 cases of mycetoma A. pelletieri coming from Sénégal are reported. This series confirms the peculiar localisation, extension and severity of this type of mycetoma: 14 cases located outside the foot (buttock, perineum, scrotum, inguinocrural area, abdominal and chest wall); frequency of lymph node involvement; importance of superficial extension; surgical possibilities overreached in more than 1/3 of the cases. Effectiveness of sulfa-methoxy-trimethoprime in the medical management is outlined.
