A tumour in the breast: vaccination granuloma as a differential diagnosis.

AIMS: Breast cancer is the most frequent type of cancer in women; special attention is therefore paid to tumours in the breast region. Vaccination granuloma is a differential diagnosis of tumours in the upper part of the breast. METHODS: All granulomatous lesions in the breast region since 1970 were retrieved from the files of the Departments of Pathology at Odense University Hospital and Svendborg Hospital. RESULTS: Fourteen cases with a histology compatible with vaccination granuloma were found. Eight patients had a known history of tetanus vaccination. The tumours were ovoid, or even rod-shaped, and measured between 8x3 mm and 12x10 mm. Histologically, there were necrotic foci surrounded by histiocytes, lymphocytes, and plasma cells. Aluminium was detected by staining with solochrome azurine. CONCLUSIONS: The possibility of a vaccination granuloma should be kept in mind in patients with a palpable tumour in the upper part of the breast, as well as in mammography screening conditions and in follow-up patients after previous treatment for breast cancer.

Chronic lithium treatment inhibits amiloride-sensitive sodium transport in the rat distal nephron.

Chronic treatment of rats with lithium leads to Na+ loss and a reduced antinatriuretic response to aldosterone, suggesting that lithium reduces conductive Na+ transport in the distal nephron. This was investigated in the present study by measuring the renal response to aldosterone infusion followed by amiloride in chronically instrumented conscious rats given lithium for 3 to 4 weeks to achieve plasma Li+ concentrations of approximately 0.5 mM. A servo-controlled infusion system was used to maintain sodium and water homeostasis, thereby preventing misinterpretation of the findings as a consequence of drug-induced changes in Na+ balance. In a control group of rats, Na+ excretion decreased in response to aldosterone (p <.01) and subsequent amiloride administration led to a marked increase in Na+ excretion (p <.001). In contrast, in the lithium-treated group, there was no significant response to either aldosterone or amiloride. It is concluded that long-term treatment with lithium, even when plasma Li+ concentrations are below 1 mM, reduces aldosterone-stimulated Na+ transport through the amiloride-sensitive Na+ channels in the principal cells of the distal nephron.

[Percutaneous drainage of recto-cecal abscess due to appendectomy].

The authors presented 26 year old women suffered from lumbal and hypogastric pain, fever and recurrence diarrhea. She was performed appendectomy 3 years previously. From that time she has had abdominal pain, lack of appetite and moderate anemia. Based on ultrasound examination the diagnosis was established abscessus of retrocecalis. The diagnostic and therapeutic difficulties resulted from the situation of the abscessus were discussed.

Characteristics of early- and late-diagnosed schizophrenia: implications for first-episode studies.

First-episode studies of schizophrenia are being carried out in many places. However, previous work has suggested that only half of the patients with schizophrenia receive the diagnosis in the initial stages of the illness. We examined whether cases of early- and late-diagnosed schizophrenia differed with respect to key sociodemographic characteristics and indicators of service use that might bias first-episode studies. Individuals who (i) presented for the first time between 1983 and 1993 to psychiatric services in a defined urban area with a cumulative mental health case register; and (ii) received a diagnosis of schizophrenia at least once during their mental health career were identified (n = 186). This sample was divided into those who received the diagnosis of schizophrenia for the first time within the first year of service contact (early-diagnosed schizophrenia; EDS), and those who received it for the first time after the first year of service contact (late-diagnosed schizophrenia; LDS). The 10-year incidence of EDS and LDS were 10.4 and 7.0 per 100,000 person-years, respectively. EDS and LDS did not differ in their pattern of association with sex, single marital status and higher levels of neighbourhood socioeconomic deprivation. However, EDS was more incident in the higher age groups, and the level of service use was higher for EDS cases in the first years of contact with mental health services, with LDS cases gradually catching up and exceeding EDS service use later in the illness course. Although differences between EDS and LDS were few, studies of patients with schizophrenia in the 'first' episode are likely to be most representative if patients who receive the diagnosis for the first time after previous episodes of care for non-schizophrenic episodes are also included.

[Bcl-2 expression in testicular cancer in relation to tumor progression].

Bcl-2 expression has been studied extensively in a variety of human tumors. However, there are lack of clinical data in regard to its expression in germ cell testicular tumors (GCTTs). In this study we screened bcl-2 expression in 70 patient with GCTTs using the immunohistochemistry (IHC) and streptavidin biotin alkaline phosphatase method. Furthermore, we correlated this expression with metastatic behaviour and clinical stage. Overall, 41 (58%) carcinomas stained with anti-bcl-2 (DAKO-124) monoclonal antibody, By histologic type, these lesions included 11 (42.3%) of 26 seminomas (S) and 30 (68.18%) of 44 non seminomatous germ cell testicular tumors (NSGCT). The incidence of bcl-2 immunostaining was higher (P = 0.05, two-tailed, Fisher's test) in NSGCT than in seminomas. Bcl-2 expression was higher in tumors from metastatic patients than in tumors from metastatic-free patient (p = 0). There was a significant difference between the three stages of disease as to the expression of bcl-2 (chi 2 = 0). High level of bcl-2 was clearly dominant in tumors of advanced stages. The present finding revealed that bcl-2 expression occurs in GCTTs. Further, they suggested that bcl-2 is associated with a more progressed malignant phenotype in these tumors.

Expression of bcl-2 in testicular carcinoma: correlation with tumor progression and MDR1/Pgp.

BACKGROUND: The expression of bcl-2 has been studied extensively in a variety of human tumors. However, there is a lack of clinical data regarding its expression in germ cell testicular tumors (GCTTs). METHODS: In this study, the authors screened 70 patients with GCTTs for bcl-2 expression using immunohistochemistry (IHC) and the streptavidin-biotin-alkaline phosphatase method. This expression was also correlated with the metastatic behavior, clinical stages, and multidrug resistance gene product protein (MDR1/Pgp) immunostaining of GCTTs. RESULTS: Overall, 41 carcinomas (58%) stained positively with anti-bcl-2 monoclonal antibody. According to histologic type, these lesions with positive staining included 11 of 26 seminomas (42.3%) and 30 of 44 nonseminomatous germ cell testicular tumors (NSGCTs) (68%). The incidence of bcl-2 immunostaining was higher (P = 0.05, two-tailed Fisher's exact test) among NSGCTs than among seminomas. The expression of bcl-2 was more prevalent among tumors from patients with metastases than among tumors from metastasis free patients (P = 0.000). There was a significant difference between the three stages of disease in the expression of bcl-2 (chi2 = 0.000), i.e., bcl-2 expression was clearly dominant among tumors at advanced stages. A significant association between bcl-2 and Pgp immunostaining was established (P = 0.004). CONCLUSIONS: These findings revealed that bcl-2 expression occurs in GCTTs. Furthermore, they suggest that bcl-2 is associated with a more advanced malignant phenotype of this tumor.

[A lump in the breast. A rare first symptom of ovarian cancer].

The breast is an uncommon site for metastasis from extramammary primaries. A 43-year-old woman presented with a lump in the left breast. A tumour with atypical microcalcifications was seen on the mammogram. Needle aspiration cytology revealed adenocarcinoma cells. The final histological diagnosis was papillary adenocarcinoma with psammoma bodies, probably secondary to an ovarian carcinoma. A bilateral carcinoma of the ovaries was subsequently diagnosed and treated. A mammary tumour with an atypical growth pattern and the absence of in situ carcinoma should always prompt the pathologist to considering the possibility of a metastatic tumour even though this is a rare occurrence. Earlier recognition of metastatic tumours to the breast may result in initiation of appropriate therapy and will preclude unnecessary surgical procedures.

Reversible S-nitrosation of creatine kinase by nitric oxide in adult rat ventricular myocytes.

We have previously demonstrated that the nitric oxide (NO) donor S-nitroso-N-acetylcysteine (SNAC) reversibly decreases the activity of creatine kinase (CK) in an isolated rat heart preparation, markedly suppressing myocardial contractile responsiveness to an inotropic challenge. We wished to further examine the role of exogenous and endogenous sources of NO species on S-nitrosation of CK and subsequent enzyme activity in adult rat ventricular myocytes (ARVM). Two S-nitrosothiol groups were formed in the CK dimer after nitrosation of rabbit skeletal muscle CK in solution. CK inactivation due to S-nitrosation was time- and concentration-dependent in solution and in ARVM lysate for both NO donors S-nitroso-N-acetylpenicillamine (SNAP) and SNAC, and was rapidly reversible with the sulfhydryl dithiothreitol (DTT). Similarly, SNAC or SNAP dose-dependently decreased CK activity in intact ARVM, which was further attenuated by increasing the metabolic activity of the cells with electrical pacing for 1 h. Co-cultures of ARVM with interleukin 1 beta (IL-1 beta)- and interferon gamma (IFN gamma)-pretreated cardiac microvascular endothelial cells (CMEC) caused no detectable decline in myocyte CK activity. Increasing GSH levels attenuated the decline in myocyte CK activity with SNAC, while decreases in myocyte GSH levels enhanced the inhibitory effect of SNAC on intact myocyte CK activity. These data indicate that the degree of inhibition of cardiac myocyte CK by NO is dependent on the extent of myocyte metabolic activity and the intracellular GSH content.

Do metallothioneins affect the response to treatment in testis cancers?

PURPOSE: Data on the involvement of elevated metallothionein (MT) expression in resistance to some of the commonly used anticancer treatments are scattered and conflicting. This encouraged us to examine further the contribution of metallothionein expression to the development of this resistance phenotype. PATIENTS AND METHODS: Formalin-fixed, paraffin-embedded blocks of primary untreated germ cell testicular tumor specimens, obtained from 77 patients following radical orchiectomy, were examined for their MT expression using monoclonal antibody and immunohistochemistry. Clinical staging, the chemotherapeutic schedule and evaluation of response to treatment (defining objective response) were performed according to UICC criteria. RESULTS: All tumor types, including seminomas and nonseminomas, expressed MT, regardless of their histology and clinical stage. The immunoreactivity of MT showed a significant positive correlation with the clinical sensitivity of cancer to antitumor therapy (P = 0.0001). CONCLUSION: In patients with germ cell testicular tumors, high MT expression, as detected by immunohistochemistry, predicts a better response rate to chemotherapy whereas tumors lacking or demonstrating low MT expression show a worse prognosis. These data do not support the hypothesis that MT overexpression contributes to cisplatinum resistance, at least in this tumor type.

Angiogenesis in breast cancer: a comparative study of the observer variability of methods for determining microvessel density.

The purpose of this study was to evaluate the reliability of different methods for estimating neovascularization in breast cancer and to compare them in terms of observer variability. The microvessel endothelium was stained immunohistochemically by antibodies against CD34. The investigated methods included Chalkley counting, estimation of intratumoral microvessel density (MVD) by one hot-spot, MVD by the mean value of three hot-spots, and the highest value of MVD in three hot-spots. In addition, we applied stereology in the quantification of angiogenesis in the whole tumor section by random and systematically distributed sampling fields. Each of forty tumors was measured with all methods, twice by the same observer and once by another observer. Observer variation was analyzed by orthogonal regression, estimating the slope and intercepts with 95% confidence intervals (CI), and by analysis of agreement using difference plots. Intraobservationally, the methods had variations of the same magnitude (coefficient of variation [CV] approximately 20%). Interobservationally, the stereologic estimate of vessel profiles, Q(A), from the whole tumor section and the Chalkley counting method had the lowest variation (CV approximately 21%), with a small contribution by observers alone (CV 8% to 9%). Interobservationally, the MVD methods had considerable variation with a large contribution by observers alone (CV approximately 30%), which was lowest using the mean of three hot-spots. Correlation slope and 95% CI of Chalkley were 1.18 (0.95, 1.48), CV 20%; slope of MVD (mean) was 1.14 (0.91, 1.43), CV 31%; and slope of MVD (max) was 1.15 (0.92, 1.45), CV 36%. The slope of MVD on one hot-spot was 1.33 (1.08, 1.63); CV 38%. Additional measurements performed using a conference microscope, eliminating subjectivity in hot-spot selection and field sampling, optimized the reproducibility: slope was 1.02 (0.99, 1.04); CV of differences, 3.5%. On the other hand, reproducibility was not necessarily optimized by choosing the same hot-spot area, because variation in selecting a microscopic field could yield different counting numbers. The stereologic estimation of QA based on the whole tumor section had a high reproducibility, with low variation due to observers. The Chalkley and MVD methods had moderate reproducibility, and the Chalkley method had low variation due to observers alone. For all methods, the biologic variation among patients was the major contributor to the total variation. The Chalkley and MVD methods have been published to provide significant prognostic estimates in breast cancer, but the Chalkley method has less observer variation and may be superior from a methodologic point of view.

[Metallothionein expression as a marker of therapeutic sensitivity in the early stages of testicular cancer].

Data concerning the involvement of elevated metallothionein (MT) expression in drug resistance are obviously scattered and contrasting. The presence of the MT gene product protein was screened in 51 untreated human germ cell testicular tumours, furthermore a relationship between MT expression and clinical resistance was investigated. Using monoclonal antibody and immunoenzyme staining elevated MT level could be demonstrated in nuclei and cytoplasm of both seminomas and non seminomatous germ cell testis tumours. Thirty-one tumours (61%) showed extensive, 15 (29%) focal positive staining. In contrast teratomas expressed this antigen negatively or scarcely. The highest level of MT was stated in early stages (I, IIA) compared with progressed stages (IIB, III) (p = 0.0004). Between the high level of MT and clinical resistance a converse correlation could be shown because the resistant tumours expressed no or low, while the sensitive tumours significantly high level of MT protein which can be used as an useful marker to identify patient subgroups sensitive to anticancer therapy, at least in testis tumours.

REPULSION, A Novel Approach to Efficient Powder Averaging in Solid-State NMR

A novel approach to efficient powder averaging in magnetic resonance is presented. The method relies on a simple numerical procedure which based on a random set of crystallite orientations through simulation of fictive intercrystallite repulsive forces iteratively determines a set of orientations uniformly distributed over the unit sphere. The so-called REPULSION partition scheme is compared to earlier methods with respect to the distribution of crystallite orientations, solid angles, and powder averaging efficiency. It is demonstrated that powder averaging using REPULSION converges faster than previous methods with respect to the number of crystallite orientations involved in the averaging. This feature renders REPULSION particularly attractive for calculation of magic-angle-spinning solid-state NMR spectra using a minimum of crystallite orientations. For numerical simulation of powder spectra, the reduced number of required crystallite orientations translates into shorter computation times and simulations less prone to systematic errors induced by finite sets of nonuniformly distributed crystallite orientations.

Measurement of renal vein blood flow in rats by high-field magnetic resonance.

The aim of the present study was to examine whether magnetic resonance imaging (MRI) based method for non-invasive in vivo measurement of vein blood flow in rats could be used to estimate renal blood flow (RBF). Measurements were performed using a high-field (7 Tesla) MRI scanner with a short echo time phase contrast velocity measurement pulse sequence. The method was evaluated in vitro by flow measurements in an acrylic pipe and in vivo by recording left renal vein blood flow in normal and unilaterally nephrectomized rats. In a subset of animals RBF was measured by a direct method using 14C-tetraethylammoniumbromide. In vitro a high accuracy was found between applied and MRI measured flow rates in the range from 0.5 to 33 ml/min (r = 0.997; P < 0.001). In vivo the MRI measured left renal vein blood flow was 70% higher in unilaterally nephrectomized animals compared to control animals (3.4 +/- 0.4 ml/min/ 100 g body wt vs. 2.0 +/- 0.1 ml/min/100 g body wt, P < 0.001). Direct measurements of RBF revealed comparable values (3.4 +/- 0.3 ml/min/100 g body wt vs. 2.3 +/- 0.4 ml/min/100 g body wt, P = 0.05). In addition, the left kidney volume was recorded by MRI with an increase amounting to 40% (1.18 +/- 0.05 ml vs. 0.84 +/- 0.02 ml; P < 0.001) in the nephrectomized group compared to controls. Finally, a positive correlation was seen between left renal vein blood flow and MRI measured renal volume (r = 0.91; P < 0.001). In summary, MRI is a non-invasive tool by which measurement of renal vein blood flow can be performed, and it is concluded that MRI-based renal vein flow measurements can be used to estimate RBF in small rodents.

Arthritis in a patient with homozygous familial hypercholesterolemia.

Familial hypercholesterolemia is a disorder of lipoprotein metabolism characterized by elevated cholesterol, low-density lipoprotein cholesterol, xanthomas and early onset atherosclerosis. Tendinitis and arthritis have been reported in patients with familial hypercholesterolemia. A report is presented of a 9-year-old girl with an acute arthritic attack who was diagnosed as homozygote familial hypercholesterolemia with hypercholesterolemic arthritis.

Cytobrush and endocervical curettage in the diagnosis of dysplasia and malignancy of the uterine cervix.

BACKGROUND: The validity of cytobrush and endocervical curettage combined with colposcopically directed biopsies in the diagnosis of cervical dysplasia and malignancy has not been evaluated in randomized trials. We aimed to elucidate the diagnostic validity of the two methods. METHODS: A prospective, randomized study of 180 consecutive patients. All patients were examined without anesthesia by colposcopically directed biopsies of the ectocervix and randomly assigned to either cytobrush or endocervical curettage. Patients with < or = CIN 1 were investigated with the alternative method three months later. Patients with > or = CIN 2 had a cone biopsy. RESULTS: One hundred and thirty-one patients were evaluable. The sensitivity of cytobrush and endocervical curettage combined with colposcopically directed biopsies of the ectocervix was 96% and 84% (p = 0.08), respectively. The specificities of the two investigations were 95% and 88%, respectively (p = 0.78). All cytobrush specimens were evaluable but because of a low recovery of endocervical material a diagnosis could not be made in 12% of the patients examined by endocervical curettage. CONCLUSION: The sensitivity of the combined use of cytobrush and biopsies of the ectocervix was equal to or higher than the sensitivity of endocervical curettage and ectocervical biopsies. The specificities of the two investigations were much alike. All cytobrush specimens were evaluable but a diagnosis could not be performed in 12% of the endocervical curettage specimens because of too little endocervical material. Furthermore, cytobrush is less inconvenient to the patient. Therefore, in the follow-up of patients with cervical dysplasia endocervical curettage may be replaced with cytobrush.

[Correlation between p-53 expression and clinical resistance in testicular cancer].

One of the most common cellular gene which negatively regulates the cell cycle, thus functioning as tumour suppressor gene, is the p-53 gene. The presence of this mutated gene has been correlated with, the aggressiveness of several malignant neoplasmas. Expression of the p-53 gene product protein was screened in 55 untreated human germ cell testicular tumours, furthermore a relationship between p-53 expression and clinical resistance was investigated. Using monoclonal antibody and immunoenzyme staining elevated p-53 level could be demonstrated in nuclei of embryonal carcinoma (84%) and seminoma components (56%). Most of the choriocarcinoma cases showed positive staining. Teratomas expressed this antigen negatively or scarcely. In seminomas the highest level of p-53 was stated in stage I. In contrast the opposite tendency could be demonstrated in embryonal carcinomas where p-53 was ++ positive in stage III. Between the high level of p-53 and clinical resistance a converse correlation could be stated because the resistant tumours expressed no or low, the sensitive tumours high level of p-53 protein (P 0.01). These results suggest that elevated p-53 expression could be a prognostic marker of sensitivity in testis cancer.

Nodular intra-abdominal panniculitis: an accompaniment of colorectal carcinoma and diverticular disease.

Intra-abdominal panniculitis is a tumour-like inflammatory condition of adipose tissue. The aetiology and pathogenesis of the disease is unknown, but a number of associated diseases have been recorded. It has been customary to deal with only primary cases in the literature. This study was undertaken in order to describe the entire spectrum of the disease including primary as well as secondary cases. Eleven patients are reported, nine of which had an associated colorectal disease in direct continuity with areas of intra-abdominal panniculitis. It is concluded that intra-abdominal panniculitis should not be regarded as a specific nosological entity but merely a result of injury to the fat cells. Intra-abdominal panniculitis is seen more often as a secondary local phenomenon than as a primary condition, and in both cases it is associated with considerable differential diagnostic problems. Primary diseases involved are, among others, colorectal carcinoma and diverticulosis. The aetiologic agent(s) are still unknown, but substances liberated from a damaged bowel might play a pathogenetic role.
