[Diagnostic x-ray computed tomography pf craniocerebral tumors. Apropos of 108 cases collected at the Soweto Center of Dakar].

The authors report a retrospective study over two years in order to assess computed tomography (CT) in the diagnosis of intracranial tumors. Results were analysed on the epidemiological, clinical and CT point of view. CT is a sensitive and non invasive exploration which should be performed first when intracranial tumor is suspected. However its cost remain high for most of the patient.

Presenting features at diagnosis and complications of hemophilia in Dakar: apropos of 25 cases.

In order to describe the presenting features at diagnosis and complications of hemophilia in Dakar, we conducted a study of hospital records between October 1991 and January 1993. Twenty-five cases of hemophilia were identified. We found that only 4% of our patients were diagnosed in the first 6 months of life whereas 64% of patients were diagnosed between 6 months and 5 years of age, 32% were detected between 5 years and 14 years of age. The presenting feature at diagnosis was external bleeding in 60% of cases and internal bleeding in 40%. 92% of cases were hemophilia type A and only 8% hemophilia type B. 56% of patients had mild hemophilia, 40% moderate and only 4% severe disease. Hemophiliac arthropathy was present on radiography in 76%. Complications were dominated by repeated joint bleeding, which was present in 92% of patients, and repeated hematomas (80% of patients). A functional handicap was present in 60% of cases. 12% of transfused hemophiliacs developed an inhibitor and 4% of patients were HIV positive. Greater awareness of hemophilia amongst the medical community as well as continued efforts to improve care for hemophiliacs in Senegal are necessary.

[Dorsal cord compression by aspergilloma. Apropos of a case].

Spinal cord compressions are mainly due to Pott disease. We present a rare case. A 31 years old woman presenting a spinal cord compression due to an aspergilloma. Our patient presented also a pulmonary aspergillosis. The spinal compression was due to a contiguous extension of aspergillus to the epidural site. The treatment associated surgery and medical treatment (Ketoconazole), with efficient results.

[Contribution of mammography and echography to the diagnosis of breast cancer in Dakar: apropos of 25 cases].

The authors studied mammographic features of 25 patients with malignant breast disease. 24 of them had ultrasonographic (US) exploration. The study appreciates epidemiological, mammographic and US aspects; It also appreciates the correlation between images and histology. The mean age is 40. Six women (26%) had family history cancer. The mean reason of the exploration is a mass in 76%. At mammography, contours of cancerous opacities are irregular in 56%, regular in 16%. In 20%, the cancer showed no opacity. At US, we find a lack that appears irregular in 48%, regular in 24%, without traduction in 4%, non precised in 24%. Mammography sensitivity was 80%, false negative rate 28% and false positive rate 8% against respectively 96%, 32% et 12% for echography. This study shows that US is more sensitive but less specific than mammography. However, we recommend the use of mammography and echography together because in Senegal, cancer happens at an earlier age at with breast is easier to analyze at US.

[Peritoneal pseudocysts: complications of ventriculo-peritoneal shunts. Apropos of 3 cases].

From 1975 to 1995, 335 patients were operated for hydrocephalus in neurosurgical clinic of CHU Fann, and got ventriculo-peritoneal shunt. 3 cases of patients presenting an uncommon complication of ventriculo-peritoneal shunt: abdominal pseudo cyst, are reported and discussed. The digestive symptoms are the first to manifest themselves for a period of time, then confusing with other forms of abdominal cysts. The suggested treatment is to remove the shunt from the peritoneal cavity and perform a ventriculo-atrial shunt, then to proceed to a laparotomy, drain the fluid and resect the cyst wall. The results are excellent.

[Caudal regression syndrome associated with Klippel-Feil syndrome].

In this paper, one case of syndrome of caudal suppression associated with a syndrome of Klippel Feil is discussed. It concerned a 14 years old boy, born form a diabetic mother with congenital abnormalities such as: urinary incontinence, facial dysmorphia, a short neck, an arachnodactylia, a lumbar kyphosis, a permanent flexion of the knee, a kalipe cavus, the spine, a partial agenesis of the third and fourth lumbar vertebrae a total agenesis of the fifth and sacrum; This syndrome is infrequent and the pathophysiological mechanisms are still under discussion and no therapeutic strategy is proposed. This case offers an opportunity to the authors to make review of the medical literature on this subject. They emphasized on the interest to look for a vertebro-medullary malformation every time a child comes to us with walking problem or enuresia, this fact should be enforced when mother is diabetic.

[Gastric volvulus in the infant. Clinical and radiological diagnosis].

The authors reported two cases of gastric volvulus in female and male babies respectively 5 and 3 months old. Patients presented functional symptoms as vomiting, regurgitation. Baryum meal X ray allows to make diagnosis in meso-axial and organoxial varieties. They showed the interest to evocate this diagnosis related to an infant vomiting's diagnosis and discussed about therapeutic modality.

[Pulmonary tuberculosis in adults: radiological aspects before starting treatment].

We studied chest radiological images in 69 patients with tuberculosis, 4 of them were affected by HIV-infection. The sex ratio was 2.63. We observed 177 abnormal pulmonary images associated in different ways. Bilateral and diffuse radiological signs were noted. Tubercular cavern was the most frequent image (92.90%). It corresponded to a clear circular and oval image surrounded with a regular wall. It can locate in lesional source or healthy parenchyme. Caverns size was situated between 10 to 80 mm. The reticular and nodular infiltration (69%) as indicated associated linear and micro-nodular images. Lobar pneumoniae (44.90%) and segmental pneumoniae (4.30%) corresponded to alveolar lesions showing an opaque and homogeneous tonality image limited by seizures. They were retractile (P = 0.015). Principal mediastinal signs were relative to the presence of adenopathies (11.6%) associated with pleuropulmonary images. Four patients (5.79%) presented tuberculosis association with positive HIV serology. These patients did not present any radiological particularity. They did not show mediastinal adenopathies aspects. Pleural images better corresponded to a localize pachypleurite (13 cas) than a liquid extravasation (8 cases).

[Prenatal echographic diagnosis of esophageal atresia. Apropos ofa case].

We reported an antenatal diagnosis case of oesophageal atresia in 21 years aged woman in second pregnancy. The echographic diagnosis of antenatal oesophageal atresia was suspected from the non visualisation of stomach liquid after separate following examinations at 8 days interval, the presence of a great abundant hydramnios and a very important delay of in utero foetal growth. This study permitted us to emphasize the antenatal diagnosis interest in supporting a child with oesophageal atresia during neonatal period. The vital prognosis was essentially conditioned in this context of neonatal emergency by putting a feeding tube (cul-de-sac) and the absolute prohibition of any oral alimentation, waiting for the transfer of the child to a specialized institution.

[Echographic and velocimetric aspects of arteriopathies in the diabetic].

AIMS: Study of Diabetic arteriopathies Arteriopathies in cervico encephalic arteries and lower limbs were detected both with Echography and Doppler flow velocity waveform analysis from 60 treated patients. They were shared into 34 women (57%) and 26 men (43%). Patients age varied between 39 and 77 years. 85% were non insulin dependent versus 15% insulin dependent patients. 36 patients (60%) presented arterial abnormalities, 14 of which (23.33%) were located in the lower limbs, 19 (31.66%) in the carotid arteries, and 3 (7%) in both carotid arteries and the lower limbs. Arteriopathies were found in medium aged patients (40-60 years). Arteriopathies are often noted in medium aged diabetics. The prevalence of lesions were relatively higher in supra-aortic trunks (36%) than in lower limbs arteries (28%). They are distal in lower limbs and constitute a factor of diabetes prognosis. The sensitivity and the less expensive cost of ultrasonor methods give a first choice to these technics in the patients following up and prevention of vascular complications of diabetes in Africa.

[Amoebic liver abscess: echographic aspects].

Amoebic liver abscess is the most frequent location of the extra-intestine amibiasis with an epidemio-endemic repartition in our areas. We are reporting in this study the main echographic patterns that can be found. 117 documents were collected and studied between 1982 and 1988 in the main hospitals of Dakar (SENEGAL). Most of the patients were young, the range of age being between 25 and 55 years old and 83% of them, were male. The diagnosis of the amoebic liver abscess was evocated on the basis of the following clinical and biological symptoms: 54.38% of painful haetomegaly, 42.10% of pleuro-pulmonary and digestive signs, 3.50% of long lasting isolated fever, non specific biological sign of inflammation, 74.57% of positive hemaglutination test. An echographic test was performed before the anti-parasitic treatment with an echotomograph PHILIPS SDR 1500 in real time using a probe of 3 MHZ. The amoebic abscess of liver was detected by the echography in all cases. The unique abscess (83.10%) was the most frequent form. It was localized in the right liver (64%) and had an heterogeneous echostructure (55.70%). The hypo-echogeneous form (36.50%) was the earlier stage of the collecting abscess. The liquid form (07.80%) was observed in the latter stages of the disease. Some difficulties to determine the amoebic abscess may appear when primitive liver cancer or pyogensus abscess are present. In these cases it is necessary to analyse the liquid of ponction to be affirmative.

[Biometric aspects of the lumbar canal in Senegalese of the African negro race (apropos of 21 autopsy parts)].

Because of frequency and seriousness of myelo radicular complications of narrow lumbar canal, the authors studied its radiological and anatomical measurements in order to specify the standards for senegalese negro african. For that purpose, they collected samples of 21 lumbosacral spines from new cadavers through medico legal necropsies. Pieces have been first X rayed, then treated. They measured the antero posterior diameter of vertebral corpse, the anterio posterior diameter of vertebral canal, the narrowest transversal diameter of vertebral corpse and the inter pedicular distance. The authors found no significant differences between radiological and anatomical measurements, but the latest changed from one stage to another. The variations also were not important comparatively to standards established by two authors.

[Sacro-ilial tuberculosis (sacrococcygeal). Apropos of 1 case].

In a single case review the authors recapitulate the different methods for the presentation of sacro-ilial infectious and underscore imagery techniques which are of prime importance in diagnostic work. X-rays can only reveal advanced sacro-ilial bone joint lesions which are under-evaluated. In the absence of HI-Tech imagery, surgical biopsy must be widely recommended to identify the germ and allow early diagnosis.

[Spinal meningiomas. Apropos of 14 cases].

Fourteen cases of spinal meningiomas were reviewed. There were 5 males and 9 females, with an average age of 40 years who underwent surgical treatment at the Neurosurgical Division of Dakar University Teaching Hospital. The thoracic spine was involved in 10 cases. The location of the tumor in the spinal canal was epidural in 6 cases and subdural in 8 cases. 7 patients (50%) had marked improvement or stabilization: 4 patients died and in 3 cases, a recurrence was noted. The authors emphasize diagnostic difficulties encountered in the epidural forms.

[Seckel's nanism: apropos of 1 case].

This report covers the first case of Seckel's syndrome diagnosed at Dakar UHC. It concerns the main type of bird head nanism. The interesting thing about this observation is that it is associated with hepatoma. Would this be just a coincidence, or could the two afflictions be connected?

[Asthmatic illness: basic trial treatment by ketotifen (apropos of 48 cases)].

This study draws on 48 Asthma files of which 68.7% were of allergic origin, and has led to the following conclusions: Ketotifene is particularly active on mainly allergic asthma, and 72.7% of the results obtained were good. the younger the asthma, the more effective the product effectiveness is greater where the sickness is recent and the patient not on medication the economy in corticoids is significant: 60% on average, it can attain 100%. On the other hand, the medicine has little effect on mainly non-allergic asthma, such as infectious asthma, late asthma and the recurrence of former asthma, all difficult to treat. One author stresses that results improve as etiological factors diminish. While awaiting the fruit of progress and research in providing other medicine to conclusively conquer asthma, Ketotifene constitutes a new, choice weapon for the therapeutic arsenal ranged against the causes of this sickness.
