Peritoneal adhesions: prevention with the use of hydrophilic polymer coatings.

Postoperative adhesions, especially peritoneal adhesions, pose an increasingly serious surgical problem. Modern surgical studies have generally attempted to deal with the problem after extensive surgical tissue damage has already occurred. This report focuses on our preliminary findings from exploratory laparotomies with canine and rat animal models, which suggest (1) that inadvertent and generally unappreciated serosal trauma may occur by contact adhesion as well as abrasion and drying mechanisms and (2) that hydrophilic polymer coatings, such as providone (polyvinylpyrrolidone), may be effective in protecting the peritoneal surface from damage. This opens up an important new approach to dealing with the problem: application of adhesive and lubricating tissue and surgical material coatings prior to surgical manipulations to limit tissue damage and thereby minimize postoperative complications. The implications of this study to many other surgical procedures may also be of considerable consequence.

Biophysical evaluation of the tumorigenic response to implanted polymers.

An evaluation of the biophysical changes of methylcholanthrene induced tumor (M4) in laboratory animals receiving four different alloplastic implants was conducted. The implants are silicone, polyether urethane, poly(methyl methacrylate), and bioglass. The tumor was allowed to grow to a large size in 28 days and then removed by shelling it from the surrounding tissue. Three different analytic volumetric measurements were carried; size in cm3, volume in milliliters and weight in grams. Statistical analysis on the three different analytic measurements did not disclose any significant differences in tumor growth potential between the control and the experimental groups. We conclude that in this study we were unable to demonstrate any tumor "facilitation" effect by any of the implantable polymers used. Therefore, there is no contraindication in using the polymers to reconstruct defects produced by cancer ablative procedures.

Immunological evaluation of the tumorigenic response to implanted polymers.

This short report concentrates on a close look at the immunological status of experimental animals which were implanted with a known dose of tumor and polymer implant. The implanted polymers used represented the most common biocompatible materials employed in current surgery. The immunological studies all were done in vitro on the cellular level. The studies suggested that there is a noticeable change of the immune system when inorganic biocompatible polymers are used in experimental animals.

Effect of applied dc currents on experimental tumor growth in rats.

Morris Hepatoma experimental tumor in ACI rats is treated with small anodal direct current via an implantable power pack unit. There was major retardation in tumor growth when the treatment was started early. There was enhancement of tumor growth when the treatment was started early and then was discontinued. There was no statistical difference in growth control of the experimental tumor when the electrotherapy was given after the half-life of the tumor was achieved. This last point will be the most important to the clinician scientist, for it simulates the clinical situation that faces the treating physician.

De novo cutaneous neoplasm: biologic behavior in an immunosuppressed patient.

The treatment of cutaneous squamous cell carcinoma in patients who are immunosuppressed should include early and adequate excision and extremely close follow-up. If the patients' tumor is aggressive, treatment should be aggressive even with discontinuation of the immunosuppression, which may lead to the death of the allograft. Sacrifice of the allograft in such an instance will lead to survival of the patient. In our patient, the continuous survival of the allograft with good renal function, in spite of the discontinuation of the immunosuppressive agents, can be interpreted only on the basis of graft adaptation.

The management of inadvertent subcutaneous adriamycin infiltration.

Once the inadvertent infiltration of Adriamycin occurs, it is recommended to immediately treat the problem. This has been accomplished by wide excision and meshed split thickness skin grafting for resurfacing of the defect. Obtaining a healed wound alleviates the stiffness from lack of motion or ruptured tendons. Pain from the open ulcer, which is prone to infection in the immunosuppressed patient, is lessened. An analogy between pit viper envenomization and Adriamycin infiltration is discussed.

Mandibular reconstruction and craniofacial fairing.

A method of restoring bony contour defects in the craniofacial regions employing a combination graft of autogenous bone and an alloplastic mesh is presented. Osteoinduction occurs within the confines of the alloplastic mesh from the cancellous bone and the resultant new bone conforms to the outline imposed by the alloplastic device. The advantages of this method include those of alloplastic materials combined with those of bone grafting. There is immediate restoration of an aesthetic contour with adequate strength and sound fixation.

Cranio-orbital correction for massive enlargement of the cranial vault.

A case is reported in which an immense cranial vault was reduced as part of the rehabilitation of a patient with severe hydrocephalus who had preservation of the intellect. This patient was selected carefully, and we do not advocate such procedures on every hydrocephalic. This patient was recumbent and bedridden prior to the procedure because of the size and weight of his head, but he was able to move freely in a wheelchair and attend a special school after the reduction of the skull.

Evaluation of speech after correction of rhinophonia with pushback palatoplasty combined with pharyngeal flap.

A comparison is made of the preoperative and postoperative speech evaluations of 15 selected subjects who had pharyngeal flap operations combined with palatal pushback. Postoperatively, 13 of the 15 patients (86 percent) showed no abnormal nasal emission and no evidence of significant hypernasality during word production. Gross substitution errors were also corrected by the surgical repair. While the number of patients is small, this study indicates equal effectiveness of the surgical technique described--regardless of the sex, the medical diagnosis, whether the procedure was primary or secondary, or the amount of postoperative time--providing there is good function of the muscles of the soft palate.

Current status of biomaterial's clinical applications in plastic and reconstructive surgery.

The use of biomaterials as replacement parts is not new. The concept has fascinated physicians for thousands of years. Using gold plates to reconstruct defects in the skull over 3000 years ago constituted the early record in the literature of use of biomaterials in implantology. The recent advances in space age technology and in clinical biocompatibility has been a major contributing factor in introducing new materials for use as implantology polymers. However, the ideal implant is still not there, and whenever a major reconstruction is required consideration should be given first to the use of autogenous substitutes; when that is not available, then we should resort to inorganic substitutes.

Implantology for correction of cranio-facial defects with an elastomer composite and bone.

The correction of cranio-facial disfigurement in the upper part of the face is the most difficult to achieve. Various materials have been utilized and are still in use. Each has advantages and disadvantages. Autogenous bone has been used with good results, except for the unacceptable irregularities of the forehead and upper orbital region. The combination graft offers the advantages of both: the bone graft will be in contact with the dura, the periorbita and the contaminated sinuses. The polyrane implant produces a smooth and contoured forehead and immediately protects the vital structures during osteoneogenesis. Twenty-six selected patients had this technique used to correct their deformity without any incidence of major complication.

The rejuvenation of the anal sphincteroplasty.

Anal incontinence, as a result of traumatic lesions upon the anal sphincter, is a devastating problem. Direct approximation by appropriate repair may not produce the appropriate animation of the anal sphincter and return of function. The transfer of the gracilis muscle for anal sphincteroplasty, which was initially described for neurogenic dysfunction, is recommended for repair and reconstruction of traumatic and dysfunction. The use of this technique in two patients with anal incontinence following trauma proved to be helpful in alleviating the problems of the patients. A diverting colostomy is used prior to the repair of the anal sphincter. Physical therapy regimens of the reconstructed anal sphincter by the muscle transfer is done prior to closing of the colonic stoma and restoring normal continuity and function of the lower part of the alimentary tract.
