[Trans-symphyseal approach in the treatment of cervico-urethro-vaginal fistulas. Apropos of 2 cases].

We opted for a symphysiotomy approach in the management of cervico-urethro-vaginal fistulae in two patients. This approach offers an excellent exposure of the region of the bladder neck. After section of the symphysis pubis with a Gigli's wire saw, the anterior wall of the bladder was taken down to the fistulae and we excised the fistulous tract. Longitudinal closure of the fistulae and the bladder followed. Results were satisfactory and there were no urinary incontinence.

[Female hypospadias. Apropos of 3 cases].

Female hypospadias (FH) is a rare congenital abnormality caracterised by total or partial agenesy of uretero-vaginal septum. We report there cases of this anomaly. In one case, FH was isolated. In the others two, it was associated with other genital abnormalities. The diagnosis was established in adult age. Urinary incontinence is the most constant revealing symptom. Treatment consists of an uretral reconstruction using the anterior wall of vagina. In one case, associated vaginal atresia was treated during the same operation. Perfect continence and good bladder emptying was noted in two cases. One patient had been reoperated to perfect the continence.

Polycyclic aromatic hydrocarbons in clear and cataractous human lenses.

In this histochemical study on the ocular lens, the authors test for the presence of various sorts of the (unsaturated) polycyclic aromatic hydrocarbon (PAH) in 66, both clear and cataractous human lenses. A statistically significant greater amount of PAH is found in the cataractous lenses studied (p < 0.01), with two kind of PAH, phenanthrene and 1,2-benzoanthracene, appearing exclusively in lenses with cataracts. The authors put forward a hypothesis on the cataractogenic effect of PAH on the basis of its interference with lens metabolism and the subsequent production and release of free-radicals.

[A modified cervico-prostatic incision technic in hypertrophic adenoma in young subjects desiring to preserve ejaculation].

The treatment of bladder neck obstruction by transurethral resection of the prostate is responsible for retrograde ejaculation, which is poorly tolerated by our younger patients. Bladder neck incision, initially proposed as treatment for bladder neck sclerosis and for small prostates, was performed according to a modified technique in 36 patients with a mean age of 57.6 years (range: 41-72 years), with benign prostatic hypertrophy less than 30 grams and wishing to retain antegrade ejaculation. This technique consists of creating a deep groove with the resector hook extending from the ureteric orifice to 5 mm above the verumontanum, incising the full thickness of the detrusor and prostatic urethra as far as the retrocervical fat. This preserves a supramontanal ring of urethral muscle whose contraction during orgasm prevents retrograde ejaculation of semen. Resection of the median lobe was also performed in 8 patients, while sparing the cervical muscular ring. The mean follow-up was 2.4 years (range: 4-84 months). Dysuria was very considerably improved in 32 patients (91.5%), with a Madsen score of less than 2. Antegrade ejaculation was preserved in 32 patients (91.5%). Two patients had to undergo secondary prostatic resection because of persistent dysuria (these 2 patients retained antegrade ejaculation). Unilateral bladder neck incision, sparing a supramontanal muscular ring is an easy, rapid technique with low morbidity, effective in the treatment of prostatism due to a small prostate (less than 30 grams). It is the operation of choice in young patients with small prostates who wish to retain antegrade ejaculation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Total septate uterus with hemi-vagina and homolateral renal dysplasia].

We report a rare case of uro-genital malformation in 18 years old girl. The malformation associate total septus uteri, hemivagina atretica dextra and homolateral dysplastic kidney. Dysmenorrhea, purulent discharges from cervix and vaginal tumor evocated the diagnosis, which was confirmed by ultrasonography, hysteroscopy and coelioscopy. Therapeutic attitude was total resection of vaginal septus and right nephrectomy. At today, seven cases were published in international literature. In all cases, renal aplasia was reported. Embryologic acknowlegements can't explain this malformation.

[Extra-renal retroperitoneal angiomyolipoma].

Angiomyolipoma is a benign kidney tumour. Its extrarenal, retroperitoneal site is exceptional. The authors report a case and emphasise the value of ultrasonography and computed tomography in the diagnosis of these tumours with a marked fatty component, without neglecting the contribution of arteriography which can be useful in the diagnosis of the origin of very large retroperitoneal tumours.

[The treatment of ureteroceles complicated by calculi].

In a series of 13 adults with calculi in ureteroceles, 10 cases underwent endoscopic meatotomy associated with endoscopic lithotripsy. In 2 bulky ureteroceles with calculi, the authors performed excision of the ureterocele and reimplantation of the ureter. In one case, the authors successfully performed extracorporeal lithotripsy. The authors obtained good results in all cases. Endoscopic meatotomy with lithotripsy is a very useful procedure for treatment of small or medium sized ureteroceles with calculi in adults.

[Renal tuberculosis with pseudotumoral form: apropos of a case].

The authors report about one rare case of renal tuberculosis in a pseudo-tumoral form, which had radiologic appearances compatible with a kidney cancer invading soft tissue. The fast spontaneous evolution towards fistulization allowed confirming the diagnosis of urinary tuberculosis. The outcome with treatment was favorable. The authors recommend that needle biopsy should be made in cases of doubtful kidney tumors to provide an exact diagnosis.

[Spontaneous subcapsular hematoma of the kidney during periarteritis nodosa].

The authors report a case of spontaneous bilateral subcapsular haematoma of the kidney in a patient with polyarteritis nodosa. Although often easy to diagnose, the aetiology of these spontaneous subcapsular haematomas is sometimes difficult to establish despite ultrasonography and computed tomography. The authors emphasise the value of arteriography in these cases, which may reveal pathognomonic signs of polyarteritis nodosa. In this case, the diagnosis was established on the basis of arteriographic findings and the American College of Rheumatology criteria and not on histological criteria.

[Retrocaval ureter. Apropos of 6 cases].

The authors report six cases of retrocaval ureter discovered in quite various circumstances. A genital malformation associated to the retrocaval ureter was found in only one case. Five of our six patients were operated. Four of them underwent uncrossing of the ureter relative to the vena cava, and a patient underwent nephrectomy because her kidney was destroyed. The sixth patient was not operated because there was no impact on the excretory cavities of the kidney. This series and the review of the literature allow specifying the course of action for this pathology.

[Retrovesical hydatid cyst. Apropos of 2 cases].

A hypogastric mass compressing the bladder in an adult, in a zone endemic for hydatid disease, should raise to suspicion of a retrovesical cyst, especially as the cyst sometimes opens into the bladder resulting in haematuria and hydatiduria. Intravenous urography and ultrasonography facilitate the topographical diagnosis. Treatment is surgical: transperitoneal cystectomy and pericystic resection as extensive as possible with exploration of the abdomen looking for other sites (liver, mesentery, omentum). This treatment was successful in the two cases reported here. In conclusion, a stable cure can be obtained by adopting a well defined therapeutic approach to these rare lesions.

Theoretical and experimental investigations on the fast rotating clinostat.

We have investigated both theoretically and experimentally the validity of the fast rotating clinostat to simulate microgravity for a free swimming single-cell organism such as the paramecium. Computer simulations show that cells on suspension move as cells cultivated in space. However, rotated paramecia are still affected by gravity, as shown by the variations in the rate of paramecium rotation on their axis. Using a fast clinostat, which allows to investigate simultaneously twenty cultures, we have observed a stimulating effect on cell growth rate similar to that previously reported in space. All these results point towards the fact that the fast clinostat can reproduce some of the effects of microgravity on paramecia.

[The valves of the ureter. Apropos of 2 cases].

The authors report two cases of congenital ureteral valves of the upper third of the ureter complicated by renal stone in two children and diagnosed by I.V.P. The two patients underwent a resection of the pathological segment and a pelvic-ureteral anastomosis successfully. The review of the literature data regarding ureteral valves allowed to emphasize the uncommon of the bilateral form complicated by renal stone and the better preoperative diagnosis by ureteroscopy and antegrade pyelogram.
