Correlation of the modified scratch test with the radioallergosorbent test (RAST) in atopic children.

The usefulness of the modified scratch test in predicting clinical allergy and its correlation with the RAST were studied in 83 atopic children. A total of 423 scratch tests were compared with the same number of RAST. Percentage agreements between the two tests for individual allergens ranged from 87.5% (milk) to 50% (cat epithelium). The RAST showed more positive results only for the house dust allergen. Both RAST and scratch tests were not indicative of clinical allergy in the skin and gastrointestinal tract. It was concluded that the modified scratch test may give useful information and remain as one of the routine investigative procedures in pediatric allergy.

[A case of advanced male breast cancer treated effectively with estrogen].

A 47-year-old man was admitted because of a left axillary tumor. A biopsy of the tumor disclosed adenocarcinoma. The bone survey showed multiple sclerotic metastases. Thirteen months after his first admission, a left breast tumor developed and a simple mastectomy revealed a papillotubular carcinoma. Skin metastases appeared postoperatively and were exacerbated with accumulation of pericardial effusion and a high CEA level (401.7 ng/ml) despite radiation and chemotherapy. Estrogen therapy with diethylstilbestrol sodium phosphate was started, resulting in the disappearance of pericardial effusion and skin metastases. The patient remains well 10 months after starting estrogen therapy with a normal CEA level.

Pharmacological properties of N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7), a calmodulin antagonist in arterial strips from rats and rabbits.

Effects of N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7), a calmodulin antagonist on vascular smooth muscle contraction were determined with helically cut arterial strips from rats and rabbits, since adrenergic innervation is quite different between the two species. The pD2 value of W-7-induced relaxation was significantly larger in rat aorta than in rabbit aorta. Treatment of the rabbit aorta with phenoxybenzamine, an alpha adrenoceptor antagonist, augmented the W-7 induced relaxation, whereas in rat aorta there was no such augmentation. Media-intimal strips of rabbit aorta showed a larger pD2 value of W-7 as compared to the value seen with whole aorta. The pD2 values of W-7 obtained in phenoxybenzamine-treated and media-intimal strips of rabbit aorta were in good agreement with the value obtained in rat aorta. Aortic strip contraction induced by W-7, which has been demonstrated as a calmodulin-independent effect of this compound, was significantly smaller in rats than in rabbits, suggesting that the relaxation of rabbit aorta induced by W-7 was inhibited by its own contractile effect. W-7 exhibited a typical noncompetitive antagonism against both norepinephrine- and Ca++- (K+-depolarized muscle) induced contractions, and the potency of antagonism was similar between the two agonists. W-7 did not affect 5-min 45Ca incubation value of the K+-stimulated increase in cellular Ca++ content in rabbit aorta, while trifluoperazine, another calmodulin antagonist, and D-600, a calcium antagonist, reduced this value.(ABSTRACT TRUNCATED AT 250 WORDS)

Changes in duodenal mucosal blood flow and mucus glycoprotein content during cysteamine-induced duodenal ulceration in rats.

The effects of cysteamine on duodenal mucosal blood flow and duodenal glycoprotein were studied during the development of duodenal ulceration in conscious rats. Cysteamine at an ulcerogenic dose (300 mg/kg s.c.) produced a remarkable decrease of duodenal mucosal blood flow which preceded the appearance of duodenal ulcers. The reduced blood flow was followed by a significant decrease of tissue levels of glycoprotein which also occurred prior to the time when duodenal injury had reached a maximum. Cysteamine was without significant effect on the rate of incorporation of 3H-glucosamine into duodenal glycoprotein at concentrations up to 10(-3) M. These results suggest that the reduction in duodenal mucosal blood flow in response to cysteamine could possibly contribute to a decrease of duodenal glycoprotein and that both may be at least in part responsible for the incidence of duodeno-ulcerogenecity.

Histologic review of breast cancer cases in survivors of atomic bombs in Hiroshima and Nagasaki, Japan.

A panel of Japanese and American pathologists reviewed existing histologic material used to study breast cancer risk among the A-bomb survivors in Hiroshima and Nagasaki, a population in which incidence studies have found a strong relationship between breast cancer risk and radiation dose. The primary charge to the panel was to define a body of confirmed cases in the Life Span Study sample of the Radiation Effects Research Foundation that would require little or no review for inclusion in future studies of breast cancer incidence. Broad agreement on histologic type was reached for 298 of 300 confirmed cases. The distribution of histologic types was, overall, similar to that seen in other studies of breast cancer in Japanese women, and did not appear to depend on dose; thus radiation-induced breast cancer appeared to be no different histologically from other breast cancer. Also, no evidence was found of variation in histologic type by city, age at exposure, age at diagnosis, or calendar time.

Ketogenesis during sepsis in relation to hepatic energy metabolism.

The concentrations of acetoacetate, beta-hydroxybutyrate, and adenine nucleotides, and the mitochondrial phosphorylative activities, induced by cecal ligation and punctured in the liver of septic rats, were determined. The concentrations of glucose, free fatty acids (FFA), and free amino acids in arterial blood were also studied along with ketone body concentrations. Hepatic energy charge levels decreased from 0.84 to 0.77 at 12h after the induction of sepsis (P less than 0.01) and to 0.60 at 18h (P less than 0.001). Mitochondrial phosphorylative activity was enhanced at 6h (P less than 0.001) and decreased at 18h later. Ketone body concentrations in the liver and the arterial blood decreased concomitant with the decrease in hepatic energy charge. The mitochondrial redox state increased significantly at 12 and 18h after the induction of sepsis (P less than 0.01) concomitant with a marked decrease in the concentrations of ketone bodies (P less than 0.01). Blood glucose levels remained within normal limits except for a transient increase at 6h, but plasma FFA levels decreased (P less than 0.01). The plasma concentrations of aromatic amino acids (P less than 0.001), proline, and alanine (P less than 0.05) increased slightly at 18h. It is suggested that the ketogenic capacity of the liver is inhibited during sepsis, but that the liver maintains gluconeogenesis at relatively normal levels until a more advanced stage of sepsis.

Nails as useful materials for detection of methamphetamine or amphetamine abuse.

Methamphetamine and amphetamine could be demonstrated in nail clippings obtained from methamphetamine users by sensitive gas chromatography/chemical ionization mass spectrometry with N-methylbenzylamine as an internal standard. The methamphetamine levels in fingernails were comparable to those in hair. Both stimulants were more concentrated in toenails than in fingernails. The detection of methamphetamine and amphetamine in nails provides an alternative information to that in hair on their past abuse.

Potentiating effects of a ventricular premature beat on the alternation of the ST-T complex of epicardial electrograms and the incidence of ventricular arrhythmias during acute coronary occlusion in dogs.

A relation between ST-T alternans (STTA) and incidence of ventricular arrhythmias (VA) was examined, and the effects of a premature beat on these were tested during acute coronary occlusion in dogs. Epicardial unipolar electrograms, epicardial and endocardial bipolar electrograms and monophasic action potentials were recorded during coronary occlusion. During the period of STTA, a remarkably late conduction and its alternation were also observed in ischemic areas. The more serious delay appeared during the negative deflection of the ST-T complex. VA followed mainly the negative deflection of the ST-T complex. The first premature activation appeared in either ischemic areas or the border areas. A ventricular premature beat (VPB) which occurred spontaneously or was produced by an electrical stimulation potentiated STTA, and additional VA frequently followed the negative deflection of the ST-T complex of the second sinus beat after the VPB. A VPB pronounced the conduction delay during the second sinus beat. The discordant STTA became concordant after the VPB. The duration of a monophasic action potential of the second sinus beat after the VPB in mildly ischemic areas decreased. These changes and enhanced STTA may contribute to the occurrence of the additional VA.
