[An autopsy case of rectal cancer receiving cisplatin at total dose of 3.250 mg and platinum concentrations in autopsy tissue samples].

An autopsy case of a 57-year-old man who had received cisplatin, total dose of 3,250 mg (1,920 mg/m2) after proctectomy for advanced rectal cancer with hepatic metastasis is reported. Platinum concentrations in autopsy tissue samples are also reported. Cisplatin at a dose of 20-40 mg was administered intravenously once a week for 82 weeks, but renal failure did not occur. The patient died 529 days after his last cisplatin therapy, due to acute suppurative cholangitis. On autopsy, the hepatic hilar metastasis involved the bile duct, but the other metastasis of the liver became regeneration and necrosis, and the kidneys showed few findings but congestion and arteriosclerotic change. The spleen evidenced mild hemosiderosis. Tissue samples obtained at the time of autopsy were assayed for platinum using flameless atomic absorption spectrometry. The spleen had the highest concentration, about 2.7 times as high as that of the liver. The liver, the heart and the kidneys had high platinum concentrations (greater than 3.04 micrograms/g), but those of the jejunum and the ileum were low (less than 0.95 micrograms/g). Although cisplatin, FT 207 and krestin were effective for the rectal cancer, platinum was undetectable (less than 0.13 micrograms/g) in the ascending and the transverse colons.

[Studies on the behavior of lipid peroxide in reperfusion induced arrhythmia].

Lipid peroxidation of the cell membrane is one of the most important factors of reperfusion induced arrhythmia (RIA). To determine the role of lipid peroxidation in RIA, lipid peroxide (LPO) and superoxide dismutase (Total, Mn-SOD) were examined in this experiment. Thirty-four male Donryu strain rats (250-350 g body weight) were studied. After left thoracotomy and 5-minute ligature of LAD coronary artery, reperfusion was done under artificial ventilation with room air. The electrocardiogram (ECG) was recorded via standard limb leads. Twenty-four rats were divided into 5 groups and the heart tissue and blood samples were collected 1. preligature 2. after 1-minute ligature 3. after 4-minute ligature 4. 1-minute after declamping and 5. 10-minute after declamping. To examine the correlation between LPO and duration of RIA, samples from another 10 rats were collected as soon as RIA disappeared. LPO, SOD, CPK, and MDH (malate dehydrogenase) were measured in all samples. The total SOD level in heart tissue decreased after reperfusion (p less than 0.05). The heart tissue LPO (t-LPO) decreased after ligature (p less than 0.05), but each value in 2. to 5. was lower than 1., and no increase was found after reperfusion. A negative correlation of R = 0.87 (p less than 0.01) was found between t-LPO and RIA duration, but a positive correlation of R = 0.79 (p less than 0.01) was found between t-LPO and CPK. Moreover, the LPO level was lower in the RIA occurrence group than in the non-occurrence group (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Analysis of pathological state of benign monoclonal gammopathy by beta 2-microglobulin and CEA].

In order to obtain the informations concerning benign monoclonal gammopathy (BMG)-related diseases, beta 2-microglobulin (beta 2-m) levels in sera from the patients with BMG were plotted against their serum carcinoembryonic antigen (CEA) levels, and then this figure was divided into the two regions, i.e., A-region (beta 2-m less than 7 mg/l & CEA less than 7 ng/ml) and B-region (beta 2-m greater than or equal to 7 mg/l & CEA greater than or equal to 7 ng/ml). Also, the normal region was expressed as C-region within A-region. In consequence, it was revealed that A-region and B-region mainly consisted of the inflammation and degeneration-related diseases and the tumor-related diseases, respectively.

[Optimum dose of intravenous butorphanol as a supplemental drug during regional anesthesia].

The optimum doses according to age of butorphanol tartrate as a supplemental drug during epidural and spinal anesthesia were investigated in 60 patients without complications. We classified patients into 4 groups by age as A, B, C and D-group. A-group consisting of patients between 20 to 40 years received 1.5mg of butorphanol. B-group 41-60, C-group 61-75 and D-group over 76 years received 1.0mg, 0.5mg and 0.25mg respectively. After operation was started without pain, and during the stable period, we determined by age, doses of butorphanol injected intravenously in one minute. HRs, mean BPs, respiratory rates (RRs) and so on were obtained at the preinjection point and 5 minutes after injection. In addition, in 15 patients, arterial blood gas analysis was performed at the same times. Butorphanol injections decreased HRs, mean BPs and RRs significantly. But there was no one whose PaCO2 increased more than 50 mmHg and no difference was found in degree of side effects between the groups. In conclusion, this study suggests that the decision to administer doses depending on the age is useful to decrease frequency of the grave side effects, especially respiratory depression which the elderly people frequently fall into.

Simple, sensitive assay of polyamines by high-performance liquid chromatography with electrochemical detection after post-column reaction with immobilized polyamine oxidase.

This simple, rapid liquid-chromatographic assay of urinary polyamines (putrescine, spermidine, spermine, and cadaverine) involves electrochemical detection with a post-column immobilized enzyme, polyamine oxidase (EC 1.4.3.6) from soybean seedlings. Polyamines are separated by isocratic ion-pairing reversed-phase chromatography, then enzymatically converted, with release of hydrogen peroxide, via the post-column reactor with immobilized polyamine oxidase; the hydrogen peroxide is detected by electrochemical oxidation on a platinum electrode. The detection limits for injected putrescine, spermidine, and spermine were 0.3, 0.5, 0.6, and 4 pmol, respectively, with linear ranges of two to three orders of magnitude. Reproducibility was also good, with CV values less than 7%. The efficiency of the immobilized enzyme column was not decreased after analysis of 300 urine samples. Putrescine and spermidine excretion in urine from patients with blood cancers and solid cancers was significantly increased.

[Risk factors for peptic ulcers treated surgically].

To elucidate the risk factors of peptic ulcer which require surgical treatments, we examined the influences of following factors such as sex, age, smoking, and alcohol in 308 peptic ulcer patients. In the group with gastric ulcers, high age group was likely to be operated. On the contrary, young patients with duodenal ulcers were likely to be treated surgically. Sex, smoking and alcohol were not regarded as risk factors. The correlations between the degree of neurosis and the amounts of smoking and alcohol were analyzed in the cases with duodenal ulcers. In the cases treated surgically, the degree of neurosis showed no or little correlations. In the cases treated conservatively, however, the degree of neurosis showed positive correlation with the amounts of smoking and alcohol.

Divergent pharmacological effects of three calmodulin antagonists, N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7), chlorpromazine and calmidazolium, on isometric tension development and myosin light chain phosphorylation in intact bovine tracheal smooth muscle.

To investigate the usefulness of calmodulin antagonists in intact cell systems, effects of three calmodulin antagonists, N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7), chlorpromazine (CPZ) and calmidazolium on isometric tension development and myosin light chain (P-LC) phosphorylation in bovine tracheal smooth muscle strips were compared to inhibition of purified myosin light chain kinase activity. These antagonists inhibited the Ca++-calmodulin-induced activation of myosin light chain kinase in a concentration-dependent manner, with IC50 values of 1.0 (calmidazolium), 25 (W-7) and 65 microM (CPZ), respectively. Inhibitory effects of these antagonists were abolished with increasing concentrations of calmodulin. However, when these antagonists were used in intact smooth muscle strips, the gradation of potencies did not parallel the anticalmodulin activities. W-7 (100 microM) exhibited a similar extent of antagonism between the contractile responses to carbachol and KCl. The increase in P-LC phosphate content in response to 1-min stimulation with 10(-5) M carbachol was inhibited by W-7. CPZ exhibited an unexpectedly potent antagonism on carbachol-induced isometric tension development and P-LC phosphorylation. Atropine showed an antagonism similar to CPZ. CPZ and verapamil had similar antagonistic effects on KCI-induced contractions. Calmidazolium (50 microM) produced no significant inhibition on carbachol-induced isometric tension development and P-LC phosphorylation in intact smooth muscle strips. It may be concluded that 1) W-7 antagonizes the smooth muscle contraction through the inhibition of the initial increase in the P-LC phosphorylation; 2) CPZ produces effects other than calmodulin antagonism; and 3) calmidazolium is not effective in intact smooth muscle strips.(ABSTRACT TRUNCATED AT 250 WORDS)

Male sterility of transgenic mice carrying exogenous mouse interferon-beta gene under the control of the metallothionein enhancer-promoter.

As an approach to elucidating the roles of interferon (IFN) in the normal physiology and diseases of animals, transgenic mice carrying extra mouse IFN-beta genes under the control of a mouse metallothionein I enhancer-promoter were constructed. Upon induction with Cd2+, IFN activity (15-430 IU/ml) was detected in the sera of six out of ten transgenic mouse lines so far obtained. Synthesis of mRNA of the transgene was observed in the liver, the testis and less abundantly in the brain. Interestingly, IFN mRNA was constitutively synthesized in the testis where substantial levels of IFN accumulated without heavy metal induction, whereas synthesis in the liver was mostly dependent on induction by CD2+. Since IFN activity in the serum also depended on heavy metal induction, the IFN in the serum may be produced mainly in the liver. All males expressing the IFN gene in the testis were found to be sterile. Testes were involuted and contained few mature sperm, and degeneration of spermatocytes and spermatids was observed. These findings suggest that high levels of IFN are harmful to spermatogenesis and can cause male sterility.

Multiple functioning paraganglioma associated with polycythemia.

Multiple retroperitoneal functioning paraganglioma complicated by polycythemia is reported in the case of a girl who, at the age of 13 years complained of headache, palpitation, and faintness preoperatively. In the retroperitoneal cavity, there were 21 paragangliomas larger than 1 cm in diameter. All were removed surgically in two sections, the first in 1979 and the second in 1983. Bilateral adrenals were macroscopically normal, and her symptoms disappeared. Now the recurrence of retroperitoneal paraganglioma is likely, although there have been no symptoms for 8 years, since the first operation. It is difficult to conclude that the tumors were benign or malignant in this case. Though complication of polycythemia had been diagnosed before the operation, the symptom continued even with the serum erythropoietin level kept in the normal range. Association of polycythemia with paraganglioma is extremely rare, and it is also rare to find more than 10 paragangliomas. Only a limited number of reports have been made on each case in the literature.

Rapid quantitative analysis of paraquat by electron spin resonance spectroscopy.

A very simple and sensitive method for quantitative measurement of paraquat in plasma, urine and some drinks is described. The paraquat radical formed by its reduction can be detected by electron spin resonance spectroscopy (ESR) at room temperature without clean-up or concentration process of samples. The sensitivity limit is 0.1 microgram paraquat/ml and the required amount of sample is 100 microliter. The time needed for the measurement is within 10 min.

Roles of the respiratory Na+ pump in bioenergetics of Vibrio alginolyticus.

Bioenergetic characteristics of Na+ pump-defective mutants of a marine bacterium Vibrio alginolyticus were compared with those of the wild type and revertant. Generation of membrane potential and motility at pH 8.5 in the mutants were completely inhibited by a proton conductor, carbonylcyanide m-chlorophenylhydrazone, whereas those in the wild type or revertant were resistant to the inhibitor. Motility and amino acid transport were driven by the electrochemical potential of Na+ not only in the wild type or revertant but also in the mutants. In the absence of the proton conductor, motility and amino acid transport of the mutants did not significantly differ from those of the wild type or revertant even at pH 8.5, where the Na+ pump has maximum activity. Therefore, the electrochemical potential of Na+ in the mutants seemed to be maintained at a normal level by a respiration-dependent H+ pump and Na+/H+ antiporter. On the other hand, growth of the mutants became defective as the medium pH increased, especially on minimal medium. These results indicate that the Na+ pump is an important energy-generating mechanism when nutrients are limited at alkaline pH.

Antagonism of alpha 1-adrenoceptor-mediated vascular contraction by urapidil in isolated arterial strips of spontaneously hypertensive rats.

Antagonism of alpha 1-adrenoceptor-mediated vascular contraction by urapidil was examined in helical strips of femoral and mesenteric arteries isolated from 13-week-old Aoki spontaneously hypertensive rats (SHR) and age-matched normotensive Wistar-Kyoto (WKY) rats, since this agent has antihypertensive effect through antagonizing peripheral alpha-adrenoceptors. Schild plot analyses clearly demonstrated the existence of only alpha 1-adrenoceptors in these arteries from both strains. Therefore, it is possible to demonstrate alpha 1-adrenoceptor blocking effects of nonselective alpha-adrenoceptor antagonists as well as selective alpha 1-adrenoceptor antagonists. Urapidil antagonized the alpha 1-adrenoceptor-mediated vascular contraction in a competitive fashion. The pA2 value for urapidil against alpha 1-adrenoceptors was not significantly different between SHR and WKY rats. The addition of 10(-5) M norepinephrine (NE) produced a sustained contraction in a SHR femoral artery, whereas in a WKY rat femoral artery this agonist produced a transient contraction followed by a sustained relaxation. Urapidil elicited a dose-dependent relaxation with a IC50 value of 6.50 in the SHR femoral artery precontracted with NE. In the presence of 3 x 10(-7) M timolol, a beta-adrenoceptor antagonist, femoral arteries from both strains exhibited similar magnitude of contraction in response to the stimulation with 10(-5) M NE. Under these conditions, urapidil elicited a similar extent of relaxation between SHR and WKY rats. On the other hand, the addition of 10(-5) M NE produced a sustained contraction in mesenteric arteries from both SHR and WKY rats. The contraction expressed as a ratio to the maximum developed by KCl depolarization was significantly greater in SHR than in WKY rats. In these arteries, the relaxing effect of urapidil was more evident in SHR than in WKY rats. Contractile responses to NE and relaxing effects of urapidil were not affected by timolol. These results suggest that urapidil effectively antagonized enhanced alpha 1-adrenoceptor responses seen in SHR arteries.

Interferon production under the control of heterologous inducible enhancers and promoters.

In a search for a useful enhancer to control expression of interferon (IFN) gene in mammalian cells, mouse IFN-beta cDNA ligated 3' downstream to the enhancer-promoter of either mouse metallothionein-I (MT-I) or Drosophila heat shock protein (HSP) was introduced into various cultured cells by calcium-phosphate precipitation method, and the level of IFN transiently produced was compared. In the case of the MT-I enhancer-promoter, low levels of IFN were produced without induction (0-21 IU/ml) and the level increased 5-50 times by heavy metals. In contrast, the basal level of expression of the HSP enhancer-promoter was very low and its expression was increased several hundred to thousand times by heat shock or arsenite. Thus the HSP enhancer-promoter appears to be a potent inducible element with very low basal level and high inducibility.

Evidence for reduced beta-adrenoceptor coupling to adenylate cyclase in femoral arteries from spontaneously hypertensive rats.

1. Arterial relaxant responses via beta-adrenoceptors have been demonstrated to be decreased in spontaneously hypertensive rats (SHR) when compared with normotensive Wistar-Kyoto rats (WKY). To determine which process of the beta-adrenoceptor.adenylate cyclase (AC) system is involved in the decreased responsiveness to beta-adrenoceptor stimulation, relaxant responses to forskolin and dibutyryl cyclic AMP (db cyclic AMP) were compared strips of femoral and mesenteric arteries isolated from 13 week-old SHR and age-matched WKY. 2. The relaxant response to either forskolin, an activator of AC, or db cyclic AMP was not significantly different between the SHR and WKY, when the strips of both arteries from both strains were contracted with K+ to an equivalent magnitude (85% of the maximum). 3. Under the same conditions, however, the relaxant response to noradrenaline (NA) via beta-adrenoceptors was significantly decreased in the SHR arteries. 4. When the strips of femoral arteries were contracted with the same concentration of K+, there was a precontraction of greater magnitude in response to the K+ and a decreased relaxation in response to forskolin, db cyclic AMP or NA in the SHR. On the other hand, when the strips of mesenteric arteries were contracted with the same concentration of K+, the precontraction was smaller in magnitude and there was an increased relaxation in the SHR. 5. The relationship between the relaxant responses and the K+-induced precontractions clearly showed that the ability of forskolin and NA to relax the K+-contracted strips depends on the magnitude of precontraction. Therefore, a difference in magnitude of precontraction between the two groups may produce a meaningless difference. 6. The relaxant responses to forskolin and NA were significantly potentiated by the addition of isobutyl methylxanthine (IBMX), an inhibitor of cyclic AMP phosphodiesterase. Even in the presence of IBMX, relaxant responses to forskolin were the same for the two strains. The difference in the pD2 value for NA-induced relaxation between the two strains was the same in the presence and absence of IBMX. 7. The relaxant effect of either nitroprusside or nifedipine, agents which are independent of this system, was not significantly different between the strips from SHR and WKY. These relaxations were not potentiated by IBMX. 8. From these results, it is concluded that the reduced beta-adrenoceptor coupling to AC is mainly involved in the decreased responsiveness to beta-adrenoceptor stimulation. Furthermore, for an accurate comparison to be made, it is necessary to minimize the influence of variations in the magnitude of precontraction on the relaxant responses.

Role of stimulatory GTP-binding protein (Gs) in reduced beta-adrenoceptor coupling in the femoral artery of spontaneously hypertensive rats.

1. Arterial relaxant responses to beta-adrenoceptor agonists are decreased in spontaneously hypertensive rats (SHR) when compared with normotensive Wistar-Kyoto rats (WKY). To establish which component of the beta-adrenoceptor.adenylate cyclase (AC) system is impaired in the SHR arteries, effects of two activators of AC--cholera toxin (CTX) and forskolin--and of dibutyryl cyclic AMP (db cyclic AMP) were compared between strips of femoral arteries isolated from 13 week-old SHR and age-matched WKY. 2. In the absence of timolol, a beta-adrenoceptor antagonist, contractile responses of the strips to noradrenaline (NA) were significantly greater in the SHR than in the WKY. Timolol augmented the contractile responses to NA to a smaller extent in the SHR than in the WKY. 3. After blockade by timolol of beta-adrenoceptors, contractile responses of the strips to NA through the activation of alpha-adrenoceptors were not significantly different between the two strains. 4. Pre-treatment of the strips with CTX, an activator of the stimulatory GTP-binding protein (Gs), produced a slow-onset and long-lived antagonism of the alpha-adrenoceptor-mediated contractions. The antagonism was much smaller in the SHR than in the WKY. 5. The dose-response curves of the strips from both strains for alpha-adrenoceptor stimulation with NA determined after pretreatment with CTX were comparable to those determined in the absence of timolol. 6. Forskolin, an activator of the catalytic subunit of AC, and DB cyclic AMP also antagonized the alpha-adrenoceptor-mediated contractions. However, these antagonisms were not significantly different between the two strains. 7. Isobutyl methylxanthine (IBMX), an inhibitor of cyclic AMP phosphodiesterase, produced a similar antagonism of the alpha-adrenoceptor-mediated contractions between the two strains. 8. These results suggest that a reduced function of Gs is the main factor responsible for the decreased responsiveness to beta-adrenoceptor stimulation in the SHR femoral artery.

Immunoglobulin subclasses in chronic tonsillitis.

The distribution and proportion of immunoglobulin-producing cells in palatine tonsil, including IgG and IgA subclasses, have been examined in chronic tonsillitis using an immunofluorescence method. Results obtained indicate that the percentage ratios of IgG1:IgG2:IgG3:IgG4 were 53.1:35.9:4.7:6.3. Higher percentages of IgG1- and lower percentages of IgG2-producing cells were found among 3 types of tonsillitis. Proportional ratios of IgA1:IgA2 were approximately 80:20, and a slight elevation of IgA2-producing cells was observed in chronic tonsillitis.

A modified rabbit ear chamber and an example of its application for intravital-microscopic study on acute effects of topical thermal stimulation.

A pre-existing plastic rabbit ear chamber was modified to be applicable for intravital-microscopic observations in pharmacological as well as pathophysiological studies on inflammation. The chamber consists of a base disk, a mica (or glass) cover-slip and a holder ring. The base disk has a round-table with a central protrusion in which a heat conductor of platinum wire is so implanted near the protrusion that thermal stimulation can be given to the regenerated tissue between the table and the coverslip.
