Terminal deletion of the short arm of chromosome 3.

A boy with growth and mental retardation, flat occiput, high and broad forehead, blepharoptosis, narrow palpebral fissures, low set, malformed ears, short neck, anal atresia, deep sacral dimple is reported. High-resolution banding analysis showed terminal deletion of the short arm of chromosome 3 (46,XY,del(3)(p25.3)). Deletions of the short arm of chromosome 3 are relatively rare. The clinical features of the patient are compared with those of 19 previously reported cases.

Diagnostic patterns of social phobia. Comparison in Tokyo and Hawaii.

A comparison was made of the diagnosis of social phobia by Japanese psychiatrists in Tokyo and American psychiatrists in Hawaii. A brief segment of videotaped interviews and written case histories of four Japanese patients from Tokyo and two Japanese-American patients from Hawaii, who were clinically diagnosed with social phobia, were presented to the clinicians for their diagnosis. Japanese psychiatrists tended to diagnose social phobia congruently for the Japanese cases but not for the Japanese-American cases. American psychiatrists tended to diagnose various categories including anxiety disorder and avoidant personality disorder, in addition to social phobia, disregarding the ethnic background of the patients. This illustrates that the diagnostic pattern for social phobia varied considerably between psychiatrists of these two countries. The reasons considered are the patient's cardinal symptom manifestation, style of problems presentation, as well as the clinician's professional orientation and familiarity to this particular disorder.

[Bone mineral content in premenopausal women treated with gonadotropin-releasing hormone analogue].

To assess bone metabolism during treatment with gonadotropin-releasing hormone analogue (GnRHa), serum osteocalcin (BGP), alkaline phosphatase (ALP), parathyroid hormone (PTH), calcitonin (CT), calcium (Ca) and phosphorus (P) were determined before and after 6 months of GnRHa treatment in 15 premenopausal women with clinically diagnosed endometriosis. The bone mineral content (BMC) of the lumbar spine (L3) was measured by single energy quantitative computed tomography in 9 women, and in 6 of these 9 women microdensitometry was performed simultaneously during the treatment. BMC decreased significantly to 92.5 +/- 6.8% (mean +/- SD) of the pretreatment value after 6 months of treatment. On the other hand, microdensitometry revealed no significant change during treatment. Serum BGP and ALP were significantly higher after 6 months of treatment than before treatment, indicating an increase in bone formation. These data indicate that the GnRHa treatment induces an increase in bone turnover and a significant bone loss.

IR-Met and IR-leu-enkephalin content in the perioesophageal ganglia of Helix aspersa seasonal variations.

1. In order to determine the presence of opioid peptides and their possible variation throughout the year in the different ganglia of the perioesophageal ring of the terrestrial snail Helix aspersa, radioimmunoassays with highly specific antisera risen for Met-enkephalin (ME) and Leu-enkephalin (LE) were carried out monthly. 2. Clear monthly variation in both IR-enkephalins' concentrations were found, following a seasonal trend. IR-LE concentrations were higher than IR-ME throughout the year in all the regions, however, no clear regional differences were found for each. 3. IR-LE/IR-ME ratios presented defined seasonal peaks for each region.

Cisplatin suppository: preparation, release characteristics and clinical evaluation.

Cisplatin (CDDP) has attracted attention as a chemotherapeutic agent for the treatment of uterine endometrial carcinoma but causes serious side effects, including renal toxicity. CDDP suppositories containing NaCl at different concentrations were prepared to enhance the efficacy and to reduce the side effects of CDDP. The release characteristics, melting point and viscosity of the suppositories were first studied. The rate of CDDP release increased as the NaCl concentration increased: it was 12% 12 h after administration of suppositories containing no NaCl, but 32% with 0.2% NaCl. The melting point was raised by addition of NaCl: 35.5 degrees C without NaCl and 36.5 degrees C with 0.2% NaCl. Addition of 0.2% NaCl doubled the viscosity. Clinically, the suppository containing 0.06% NaCl was given to 3 patients with endometrial carcinoma twice a week for 3 weeks to examine serum CDDP levels and endometrial absorption. Patients with endometrial carcinoma showed different peak plasma platinum (Pt) levels which were as low as 0.12, 0.06 and 0.22 micrograms Pt/ml with similar patterns of change in the level. Radiographic analysis revealed many Pt particles in sections of necrosed endometria after 21d of the treatment. No side effects of CDDP were found in biochemical testing or subjective symptoms.

Breathing patterns during sleep in stable asthmatic children.

Breathing patterns during sleep at night were studied in 15 asymptomatic asthmatic children and 11 nonasthmatic controls using impedance pneumography. Inspiratory time (TI), expiratory time (TE) and expiratory time/inspiratory time ratio (TE/TI) were used as the indices of breathing patterns. In the daytime, TI, TE and TE/TI, showed no significant differences between asthmatics and controls. During nocturnal sleep, TE and TE/TI increased significantly in the asthmatics compared with the value during daytime, while these values showed only a small variation overnight in the controls. TI showed no significant changes through the night in either the asthmatics and the controls. The results of this study indicate that the stable asthmatic children had abnormal breathing patterns during nocturnal sleep.

Dominant mice show much lower concentrations of methionine-enkephalin in brain tissue than subordinates: cause or effect?

Possible relationships between dominance rank and brain methionine-enkephalin content were studied by using a method of consecutive rank determination in groups of mice reassembled according to their initial ordinal dominance rank. Thus, 10 groups of 3 Balb/c AnN mice of identical ordinal rank were formed with the consecutive dominants of 3 groups of 10 mice. Dominant, subdominant, subordinate and non-aggressive mice were recognized in these groups. Rank order was strongly correlated with brain Met-enkephalin content. Dominant mice contain less than a fourth Met-enkephalin immunologic reactivity in the brainstem and about half the forebrain concentration than subordinates. Brainstem Met-enkephalin correlated with the final ordinal rank and, accordingly, it was found to decrease exponentially after mice acquired alpha-dominance status. Different concentrations in the forebrain correlated with the initial but not the final ordinal rank and, therefore, probably preceded dominance status.

Human papillomavirus DNA in squamous cell carcinoma of the upper aerodigestive tract.

Nineteen samples of DNA from squamous cell carcinoma of the upper aerodigestive tract were analyzed by Southern blot hybridization for the presence of human papillomavirus (HPV) DNA. Two samples of DNA contained HPV 16 DNA or its homologous sequence. In one maxillary carcinoma, the sequences homologous to HPV 16 were detected. In one tonsillar carcinoma, HPV 16 sequence was also shown to be present. The patients positive for HPV DNA were female and had neither smoking nor drinking habits. These results indicate that HPV infections may play a role in the development of some types of squamous cell carcinomas of the upper aerodigestive tract.

Analysis of the nucleation and crystal growth kinetics of lysozyme by a theory of self-assembly.

Concentration changes in supersaturated solutions during the nucleation and growth of the orthorhombic form of hen egg-white lysozyme crystals have been observed for 121 d at 35 degrees C and pH 4.6, and with 3% NaCl. The effect of a variation in the initial protein concentration on the rate of approach to solubility in equilibrium is analyzed, by applying a model, originally developed for the understanding of protein self-assembly. It is shown that the observed kinetics can be explained fairly well by this model, whose basic assumptions are that (a) the nucleation is induced by aggregation of i0 molecules into particular geometry, and (b) the growth proceeds via attachment of a monomer. The i0 value for this process is four, which agrees with the number of molecules in a unit cell. Similarity and dissimilarity of the observed crystal growth to that of low molecular weight substances are discussed.

Difference between visual feedback and visual suppression upon the stabilization of body sway in normal subjects.

We developed a system to feedback the center of foot pressure by vision using a microcomputer. The subjects were instructed to adjust the center of foot pressure (displayed on the computer display at real time) to the point of intersection of the two stable (horizontal and vertical) lines on the computer display. The peak of the power spectra of body sway shifted significantly to the higher frequency range in the Y component (the lateral direction) in the visual feedback condition. Thus, we conclude that the visual feedback of body sway depends mainly on the error signals between the fovea and the image in the lateral direction to stabilize body sway.

Multi-cultural study of minor psychiatric disorders in Asia: symptom manifestations.

Patients with minor psychiatric disorders, including neuroses, situational adjustment reaction or acute emotional reaction, were investigated using symptom questionnaires at five research sites in Asia including: Chiang-Mai, Thailand; Bali, Indonesia; Kao-Hsiung, Taiwan, China; Shanghai, China; and Tokyo, Japan. The results revealed that the symptom profiles differ significantly among groups of different settings indicating that sociocultural background does contribute to the manifestation of neurotic symptomatology. It was also found that numerous and various subtypes of somatic scales were identified through factor analysis of symptoms for these Asian populations. It demonstrates that the spectrum of neurotic symptoms has a different focus for subjects in different sociocultural settings.

[Antianaphylactic components of Asarum forbsii Maxin].

From the ethyl acetate extract of Asarum forbesii Maxin, four new constituents, asarumin A(I), B(II), C(III) and D(IV), were isolated along with elemicin (V), trans-asarone(VI) and linoleic acid(VII). The structures of the new compounds were elucidated as methyl 3 S-benzoyloxy-2 S-hydroxy-2-isopropylbutyrate for I, methyl 2 R-benzoyloxyisopentanoate for II, methyl 2 R-trans-cinnamoyloxyisopentanoate for III and methyl 2 R-piperonyloyloxyisopentanoate for IV. Compounds I, II, III and VII showed weak inhibition of PCA in rats, but the other compounds were inactive.

[Lumbar bone density and serum osteocalcin in pre- and postmenopausal women].

The bone mineral density of the lumbar spine (L3) and serum osteocalcin (OC) were measured in 31 pre- and 25 postmenopausal women. Bone density was measured by single energy quantitative computed tomography (QCT value). A significant inverse correlation between the QCT value and age was demonstrated in postmenopausal women but no correlation was found in premenopausal women. Mean QCT value was significantly lower in postmenopausal women than in premenopausal women. Serum OC increases with advancing age and mean serum OC was significantly higher in postmenopausal women than in premenopausal women. A significant inverse correlation between the QCT value and serum OC was demonstrated. These data suggest that there is an association between menopause and accelerated loss of lumbar bone density and that the serum OC level may be useful in evaluating the bone mass.

[A case of malignant mesenchymoma of the maxilla].

The case of a 48-year-old male with a malignant mesenchymoma of the maxilla is reported. The patient had complained of bloody rhinorrhea and cheek pain, and a total maxillectomy was performed. The tumor consisted of a rhabdomyosarcoma, an osteosarcoma and a chondrosarcoma, which were relatively differentiated. Later, recurrence of a tumor and a lung metastasis occurred, however the tumor revealed undifferentiated mesenchymal cells. This led us to suspect the origin of the malignant mesenchymoma had been a primitive mesenchymal cell. To our a knowledge, the present case is the first reported case of a malignant mesenchymoma with a maxillary origin in the world.

Chondrosarcoma

[Effects of maternal exercise using bicycle ergometer on maternal heart rate, blood pressure, oxygen consumption and fetal heart rate].

The effects of maternal exercise on pregnant women and fetal well-being are largely unknown. Forty-eight pregnant women between 16 and 39 weeks' gestation were exercised on a bicycle ergometer. We studied the oxygen consumption, blood pressure, maternal and fetal heart rate (FHR) at rest, during and after the exercise. The mean maternal heart rate and blood pressure were increased to 166.1 +/- 12.2/min (mean +/- S.D., n = 48) and 161.1 +/- 20.1/82.7 +/- 15.2 mmHg, respectively, at maximal exercise. The absolute oxygen consumption (1/min) was increased with advancing pregnancy at rest and maximal exercise, but the functional oxygen consumption (ml/kg/min) was not changed during pregnancy. The mean FHR was increased about 4 and 9 bpm in the 2nd and 3rd trimesters, respectively. Abnormal FHR patterns after the exercise were observed in 8 cases (16.7%), mild tachycardia: 6 cases, deceleration: 2 cases. Increasing the maternal heart rate at maximal exercise, increased the frequency of the abnormal FHR pattern. When the maternal heart rate was below 160/min, there was no abnormal FHR pattern. These results suggest that several medical checks should be done not only for the mother but also for her fetus during exercise and the maternal heart rate should not exceed 160/min.
