[Diagnosis and therapy of anorectal injuries].

Causes and management of anorectal injuries were examined in 16 patients. The outcome depends on the extent of local defects, on concomitant injuries and on the patient's age. We also stress the importance of immediate diagnosis and adequate treatment, which will result in a reduction of complications and mortality, ranging from 12 to 15% in patients with larger injuries of the colon and rectum. Management of lesions will include primary reconstruction of the sphincter muscle, suture or resection of colonic injury and total diversional colostomy.

[Restoration of work capacity after major vascular surgical interventions. Expectations and reality].

130 patients who had been working pre-operatively were examined and interviewed at least one year after an aorto-femoral bypass operation; 40.8% had continued smoking. Although 83.8% of patients were satisfied with the result of the operation, 73.9% could not be fully rehabilitated despite a successful operation. The majority of patients in whom work capacity had not been restored was in the group which was economically best protected (civil servants and employees). Apparently it is not the operation but the standard of socio-economic protection which is decisive in restoring work capacity.

[Reconstruction surgical therapy of esophageal cancer. Personal experiences and results].

Within the last nine years 125 patients underwent surgery for carcinoma of the oesophagus. In our cases oesophagogastrostomy has proved to be the most successful operative technique. The disappointing results of the first years could be improved significantly by standardized perioperative therapeutic measures. Because of the expected terrible end these patients have to face, indication for surgery was not restricted. In order to improve the results of operative treatment, efforts have to be made to increase the rate of early detection which is the basis for curative measures.

[Transient reduced adrenal gland function following large vascular surgical interventions].

In 48 patients with aorto-femoral bypass-operations pre- and postoperative measurements of total serumcorticoids were performed. All serum levels were then compared to a control group undergoing large abdominal operations. In contrast to the control group twelve patients with aorto-femoral bypass-operations showed values that were for a time clearly below preoperative levels. In six patients with bypass-operations values could be found which were for a short time within the limits of relative adrenal insufficiency. ACTH-stimulation test performed in these patients showed significant lower values than those preoperatively measured. Due to systemic overdose heparinization we believe haemorrhage within the adrenal glands to be responsible for these changes.

[Protection of sutures of the injured esophagus using polyglactin 910 (Vicryl) mesh].

Dehiscence of suture after operative treatment of esophageal rupture is a quite frequent and dangerous complication. Several methods of reinforcement of suture with autologus material have been suggested of which the use of a pleura flap was the most successful one. After experimental investigations in the Department of Surgery of the University Hospital of Münster the securing of suture was performed by using Polyglactin 910 (Vicryl) meshgrafts. In 8 patients where the mesh grafts were used, a secondary leakage could be avoided. Considering our experiences, securing of suture by the use of Polyglactin 910 mesh-grafts is a recommendable method.

[An ischemic-septic disease picture following acute occlusion of the superior mesenteric artery].

The pattern of symptoms which evidences acute mesenterial infarction has been investigated on more than 40 pigs by applying diverse non-surgical therapeutic approaches during transarterial balloon occlusion of the A. mesenterica superior. All animals died within 38 hours, much earlier after central occlusion than after blockage of the peripheral branches of the artery. A convincing delay could only be observed in the group treated by antibiotics. In two groups of clinical patients, 20 before and 28 after the experimental study, we found a significantly better prognosis in the latter group. About 46% of them got an antibiotic treatment immediately after being admitted to hospital. However, recently the time elapsing from the vascular accident to laparotomy has been shorter than in the last two decades. This contributes to better chance of survival in mesenterial ischaemia. Thus, antibiotic protection and early surgical intervention can be considered to be crucial for survival after mesenterial infarction.

[Pancreatic duct occlusion following duodeno-pancreatectomy in cancer surgery].

In 19 patients with a carcinoma of the pancreas an intraoperative occlusion of the pancreatic duct was performed with a partial duodenopancreatectomy by instillation of Ethibloc. In 13 cases a carcinoma of the papilla and in 6 cases a carcinoma of the head of pancreas were found. In two cases the duct was primarily closed by suture, which was followed by a pancreatic fistula that disappeared after 2 weeks. Therefore, a pancreaticojejunal anastomosis was performed in the other patients. One patient died owing to a total necrosis of the remaining pancreas, but no other local complications were seen at the time of pancreatic anastomosis. Lethality was approximately 5% and was therefore significantly lower than in other reports.

[Replacement of the common bile duct by an autologous vein (author's transl)].

3 cm of the common bile duct was removed in 19 dogs and the defect was bridged by an autologous vein. The interposition of a vein graft was performed with a double end-to-end anastomosis. In four cases a T-drain was additionally placed into the transplant. GOT, GPT, LDH, AP and bilirubin were tested in regular intervals. The patency of the transplant was proved by radiography with dogs which survived longer than 6 weeks. 5 dogs died within the first 4 weeks, 4 were killed after 2 months, because of incrustation of the T-drain for a regular percutaneous rinsing of the drainage was not possible. 10 dogs survived for 6 to 12 months. These animals showed increasing laboratory results up to two months, then the tests normalized. The function of the transplanted vein was without complication and the liver histology did not show any pathological alteration. After two months the transplanted vein was bridged by bile duct epithelium.

[Cutaneous allergen test in children: frequency of conversion of prick-test reactions, IgE concentrations in serum and course of the disease in young asthmatics during or without treatment with "Dinatrium cromoglicicum" inhalation (author's transl)].

Among 62 children with asthma or/and hay fever 32 were treated symptomatically. 30 were treated with DNCG inhalations. 22 asthmatics with proved inhalation-allergy responded well or very well to DNCG inhalations, 3 others only fairly. Of 5 patients without proved allergy 4 showed a fair response, 1 none. In group I (without DNCG) 4 out of 20 negative prick-tests became definitely positive at a follow-up examination, in group II (with DNCG) 9 out of 14. The follow-up examinations tended to be done during a time when symptoms were reduced or absent. In group II there were 7 of 9 children with good effect of DNCG. IgE assays in serum were clearly lower in "non-allergics" than in sensitized patients. The IgE was raised only in children where the follow-up showed a positive reaction. One might assume that when the clinical symptoms improve, IgE or mediator substances in the tissues, particularly in mast cells, are accumulated so that cutaneous skin-reactions now become clearly positive. IgE concentration in serum did not show correlated changes. Assessment of the clinical course of the illness apart, conversion of prick testing might indicate protective action of DNCG inhalations in bronchial asthma due to inhalation allergy.

[Significance of arterial circulation disorders for the planning of hip prosthesis interventions].

Between 1980 and 1990 a total of 17 patients who had undergone or had been selected for total hip replacement were treated in the Department of Surgery at Münster University. Angiographic studies performed because of persistent symptoms revealed circulatory disorders of the pelvic floor. Vascular surgery was necessary to eliminate symptoms in these patients. Postoperative complications in 3 patients following total hip replacement were also only eliminated by vascular surgery. In the present authors' view, failure to recognize preoperatively existing arterial circulation disorders can lead to misdiagnosis and severe postoperative complications. Every hip replacement operation should be preceded by a thorough clinical and--if doubt persists--an angiological examination. Documented arterial circulation disorders should be eliminated by vascular surgery prior to hip surgery.

Genetic mapping of a wide spectrum nematode resistance gene (Hero) against Globodera rostochiensis in tomato.

The Hero gene confers resistance to a wide spectrum of pathotypes of the potato cyst nematode Globodera rostochiensis. This gene has been introgressed from the wild tomato species Lycopersicon pimpinellifolium into the cultivated tomato. We have used RFLP and RAPD analysis for the targeted search of the L. pimpinellifolium into the cultivated tomato. We have used RFLP and RAPD analysis for the targeted search of the L. pimpinellifolium segment. The resistant line LA 1792 contains a single introgressed segment on chromosome 4, which is characterized by three RFLP markers from the high-density RFLP map of tomato. The map position of the Hero gene in large populations, four additional markers were identified in the introgressed region. After analyzing more than 800 gametes for recombination, we found that one marker is only 0.4 cM away from the Hero gene. YAC clones isolated from a region near the Hero gene indicate that in this area of the genome, the kb/cM ratio is relatively low (<450 kb/cM) and chromosome walking should be feasible in order to isolate this gene.
