[Studies on the half-life of C-terminal parathyroid hormone in chronic renal failure].

Subtotal parathyroidectomy (PTX) was performed for patients with hyperparathyroidism associated with chronic renal failure, and from sequential determination of PTH after PTX, the half-life of 2 different C-PTH was calculated. The half-life (T 1/2) of 46 approximately 84 PTH and 65 approximately 84 PTH was 18.1 +/- 3.9 and 14.0 +/- 2.2 hours (Mean +/- SD) respectively. Half-life of both C-PTH was markedly prolonged compared with that (30 approximately 60 minutes) of healthy subjects. It is considered that the longer half-life of 46 approximately 84 PTH than 65 approximately 84 PTH might be attributed to the difference of molecular weight, degrading enzymes and operative procedures. Determined value of C-PTH in chronic renal failure should be evaluated in consideration of prolonged metabolic clearance of C-PTH.

Tuberous sclerosis: aberrant metabolism of ornithine, proline and glutamate in cultured fibroblasts.

To investigate aberrant metabolism of proline (Pro) and its precursors in tuberous sclerosis (TS), 6, 7 and 5 strains of control, TS (normal skin) and TS (tumor) fibroblasts, respectively, were cultured in Eagle's MEM containing dialyzed fetal bovine serum with or without 0.1 mM ornithine (Orn). Ornithine aminotransferase (OAT) activity was decreased in TS, especially in TS (tumor) after mild sonication treatment. The yield of the OAT protein was inhibited in TS (tumor) when cultured without Orn. Free glutamate (Glu) in the medium was significantly increased in TS (tumor). Free proline (Pro) in cells was significantly decreased in TS (tumor) when cultured with Orn, but protein-bound Pro was not. The relative concentration of free Glu to glutamine (Gln) in the medium and that of free Glu to Pro in cells cultured with Orn were increased in TS (tumor). These results suggest that the requirement for Orn, increased turnover of Pro to Glu and increased elimination of Glu into the medium occur in TS (tumor). Aberrant regulation or turnover of Pro and Glu metabolism may occur in TS, especially in tumor cells.

Clinical effects of MND-19 (Inosiplex) on subacute sclerosing panencephalitis--a multi-institutional collaborative study--The Inosiplex-SSPE Research Committee.

A total of 151 cases of subacute sclerosing panencephalitis (SSPE), comprising 89 cases treated with MND-19 (Inosiplex) and 62 untreated cases, were retrospectively investigated as to background characteristics, survival rate and clinical course in order to compare the findings in the 2 groups of cases. The survival rate for the cases treated with MND-19 (MND-19-treated group) was significantly higher than that for the untreated cases (control group), which was also true on stratified analysis or on smoothing of the background factors by means of Cox's multiple regression model. Investigation of the clinical course revealed that progression through the disease stages was significantly slow in the MND-19-treated group, compared with in the control group. Global rating of the clinical course showed that a prolonged remission was obtained in more MND-19-treated cases than control cases. The measles virus antibody titer was in no way affected in the former group. Side effects of MND-19 were observed in 17 of the 89 treated cases (19.1%).

Cell culture study on neurofibromatosis.

Cells obtained on culture from normal appearing skin (NFns) and from neurofibromas of neurofibromatosis patients (NFnf), and from normal skin of normal donors (Normal) were studied. We measured the growth rate, maximal cell density, radiation sensitivity and resistance to 3-nitrotyrosine of each of the three groups of strains. The growth rate and maximal cell density of fibroblasts derived from neurofibromas of the neurofibromatosis patients were significantly lower than those in normal donors. The neurofibromatosis cells showed no X-ray hypersensitivity, as compared to normal controls. We also did not observe resistance to 3-nitrotyrosine of the cells derived from normal appearing skin of the neurofibromatosis patients. However, cells derived from neurofibromas of the neurofibromatosis patients showed resistance to 3-nitrotyrosine. This phenomenon may explain the slow growth rate of the latter cells.

Effects of maternal caffeine ingestion on the perinatal cerebrum.

The cerebra of fetal rats from dams given 0.04 or 0.02% caffeine in drinking water ad libitum before and/or during pregnancy were examined on gestational day 21. A low placental weight was induced by caffeine ingestion for a long time throughout premating and pregnancy. A greater reduction in the fetal weight of the cerebrum than that of the body was observed with caffeine ingestion during pregnancy of levels of 1.5-3.0 micrograms caffeine/ml or g wet weight in dams and fetuses. In the cerebra of offspring, the levels of caffeine and theophylline did not change for 4 h after birth, and theophylline was not detected at all after intraperitoneal injection of caffeine. Thus, maternal caffeine should be warned against the fetal cerebral function.

Factors modifying the prognosis of Wilson's disease in childhood.

The prognosis of Wilson's disease was investigated in 96 patients, in whom the disease had presented before 15 years of age and had begun between 1965 and 1983 (when D-penicillamine was widely available in Japan). In the activities of daily living, the prognosis was poor in those patients presenting with neurological symptoms. Interruption of D-penicillamine treatment was seen in one third of the patients, and it worsened the prognosis. Toxic side effects were seen in about half of the patients, being more frequent in the patients with initial neurological symptoms. A disappointing 17% of patients with slight or no side effects discontinued the drug. Death occurred in eight patients of whom seven had had initial hepatic symptoms. Not only early diagnosis and treatment before the appearance of hepatic failure or neurological symptoms, but also treatment throughout life without interruption is important for improving the prognosis of Wilson's disease.

Cockayne syndrome with delayed recovery of RNA synthesis after ultraviolet irradiation but normal ultraviolet survival.

We report a girl with Cockayne syndrome (CS) with atypical cellular features. We studied the ultraviolet (UV)-sensitivity of cultured fibroblast cells derived from this case and male CS siblings as positive controls. Cells from this female with CS displayed normal unscheduled DNA synthesis and repair replication capacity. However, the cells also displayed a less depressed level of RNA synthesis after UV irradiation, compared to control CS cells, and showed normal UV survival. This CS case with early onset of abnormalities had more serious clinical manifestations than the control CS siblings. These cytological results suggest that there is considerable clinical and cellular heterogeneity in CS and that cellular sensitivity to UV might not be as essential for the diagnosis of CS as previously thought.

Effect of prenatal X irradiation on chemical components of DNA and DNA-protein crosslinks in rat cerebrum in the perinatal periods.

Wistar rats were X-irradiated in utero with 100 or 200 R on Day 13 of gestation. X Irradiation resulted in decreases not only in cerebral weight up to 15 days old but also in DNA content from Day 19 of gestation to 5 days old, and in a tendency to increase the ratio of protein to DNA in the perinatal period. The DNA contents of the homogenate, isolated nuclei, and chromatin of the cerebrum in the irradiated group were significantly lower than those in the control group. The ratio of protein to DNA at the nuclei, chromatin, and isolated DNA steps increased on irradiation. The total nucleoside content of isolated DNA determined by high-performance liquid chromatography was higher in the irradiated group than that in the control group on Day 21 of gestation but not on Day 19 of gestation. No new peaks were observed and no change in the guanine-cytosine content was seen on irradiation. X Irradiation resulted in decreases in the cytosine and deoxycytidine contents and an increase in the deoxyadenosine content. The formation of DNA-protein crosslinks in the cerebral chromatin as determined by a filter binding assay tended to increase in the irradiated groups.

A clinical trial of interferon therapy in a case of rapidly progressive SSPE.

A 6-year-old Japanese boy with rapidly progressive SSPE was reported, who received interferon therapy and recovered from stage III-A to stage II-B according to the criteria of Freeman. This is the first report of the beneficial therapeutic effect of interferon therapy on rapidly progressive SSPE, which is lethal and poorly responded to any kind of previous therapeutic trials.
