Abnormal cortical excitability in Rett syndrome.

Visual and somatosensory evoked potentials (VEPs and SEPs) were studied in 9 patients with Rett syndrome and compared with those in 6 patients with photosensitive progressive myoclonus epilepsy (PPME). In Rett syndrome, a giant III-IV amplitude of VEPs was present in 8 patients, although none exhibited giant II-III amplitudes. Four of 6 patients with Rett syndrome who demonstrated giant SEPs did not have a C reflex. Conversely, the patients with PPME demonstrated giant II-III and III-IV amplitudes in VEPs, and giant SEPs with concurrent positive C reflexes. It is concluded that the mechanism of altered cortical excitability in Rett syndrome is different from that in PPME.

Auditory evoked responses in Krabbe disease.

Serial auditory evoked responses were investigated in 3 children with Krabbe disease. Auditory brainstem responses revealed prolongation of each wave component and interpeak latency with decreased amplitudes in later components which finally disappeared except for wave I. Long-latency auditory responses (LLRs) persisted in the advanced stage when all wave components of middle-latency auditory responses (MLRs) had disappeared. The results of auditory brainstem responses and MLRs are compatible with magnetic resonance imaging findings and a review of pathologic findings in Krabbe disease, including extensive involvement of brainstem and subcortical structures. It is suggested that the source of LLR waves is different from that of MLR because of the persistent existence of LLR waves. It is speculated that the cerebral cortex and/or subcortical U fibers, which are spared in Krabbe disease, have an important role in generating LLR wave components.

Detection of serum p53 antibodies in patients with esophageal squamous cell carcinoma: correlation with clinicopathologic features and tumor markers.

The significance of serum p53-Abs in patients with esophageal squamous cell carcinoma was determined. Examination of clinicopathological features and assessment of tumor marker sensitivities of carcinoembryonic antigen (CEA), squamous cell carcinoma antigen (SCC-Ag) and CYFRA21-1 were performed. Thirty-three (58%) of 57 patients were positive for serum p53-Abs, however, no relation with cancer progression existed. Fourteen of the 33 sero-positive patients revealed normal levels of all tumor markers tested. Thus, serum p53-Abs appears to be a useful marker for the detection of esophageal squamous cell carcinoma.

Lymph node metastasis with adenocarcinoma of the gastric cardia: clinicopathological analysis and indication for D1 dissection.

PURPOSE: To establish the indications for proximal resection with dissection of perigastric lymph nodes in order to treat adenocarcinoma of the gastric cardia. METHODS: We analyzed the clinicopathological features of 110 resected adenocarcinomas of the gastric cardia with respect to the appropriate operative method and extent of lymphadenectomy for treatment. RESULTS: Of the 110 patients, 93 underwent curative resection. The D2 group (total gastrectomy with dissection of extended regional lymph nodes) revealed higher morbidity and mortality rates compared to the D1 group (proximal resection with dissection of perigastric lymph nodes). The risk of lymph node metastasis was determined by depth of invasion, size of tumors, and gross findings of tumors. CONCLUSION: The appropriate operative method for adenocarcinoma of the gastric cardia requires: (i) tumor size less than 4 cm; and (ii) gross findings indicating a superficial type of lesion, which are recommended for proximal resection with dissection of perigastric lymph nodes.

Sensitization primes platelet-activating factor (PAF)-induced accumulation of eosinophils in mouse skin lesions: contribution of cytokines to the response.

We have examined the hypothesis that cytokines mediate the enhanced responsiveness of eosinophils to PAF in sensitized mouse skin. PAF (10 ng per site) resulted in a considerable degree of eosinophil accumulation in ovalbumin (OA)-sensitized mice but not in non-sensitized mice. Intradermal preadministration of cytokines (IL-5, IL-3 and GM-CSF) also significantly enhanced PAF-induced migration of eosinophils in a dose-dependent manner. The relative potency with which these cytokines primed cell migration was IL-5 greater than IL-3 greater than GM-CSF, however, each cytokine alone showed no direct effect. We conclude that the sensitization or the exogenous application of cytokines is capable of augmenting PAF-induced eosinophil migration in mice in vivo, and the cytokines thus elicited by sensitization may contribute to the extensive recruitment of inflammatory cells in allergic diseases.
