Posterior descending coronary artery flow reserve assessment by Doppler echocardiography in children with and without congenital heart defect: comparison with invasive technique.

To evaluate whether transthoracic Doppler echocardiography can reliably measure coronary flow velocity and coronary flow velocity reserve (CFVR) in the posterior descending coronary artery (PD) in children, we examined 32 patients who had congenital heart disease (ventricular septal defect in 10, tetralogy of Fallot in 6, tricuspid atresia in 3, double-outlet right ventricle in 2, patent ductus arteriosus in 2, and aortic valve stenosis in 2) and 7 patients who had a history of Kawasaki disease without stenosis or aneurysm formation of the coronary artery. Average peak flow velocity (APV) in the PD was measured by transthoracic Doppler echocardiography at the time of intracoronary Doppler study. CFVR was defined as the ratio of hyperemic to basal APV. Clear envelopes of basal and hyperemic APV in the PD were obtained in 23 of 32 patients by transthoracic Doppler echocardiography. APV obtained from transthoracic Doppler echocardiography correlated highly with that from the Doppler guidewire method (r=0.91). The mean difference between transthoracic Doppler echocardiography and the Doppler guidewire method was 0.1+/-2.9. There was an excellent correlation between transthoracic Doppler echocardiography and the Doppler guidewire method for the measurements of CFVR (r=0.84). The mean difference between transthoracic Doppler echocardiography and Doppler guidewire was -0.016+/-0.198. Noninvasive measurement of coronary flow velocity and CFVR in the PD using transthoracic Doppler echocardiography accurately reflects invasive measurement of coronary flow velocity and CFVR by the Doppler guidewire method in pediatric patients with various heart diseases.

Forensic application of rapid analysis of carboxyhemoglobin in blood using an oxygen electrode.

Blood carboxyhemoglobin (HbCO) levels in forensic victims were measured by a new and rapid oxygen-electrode method for successive determinations of HHb, HbO2 and HbCO. It was found that the method is applicable to old denatured blood when combined with the cyanmethemoglobin method. Methemoglobin (Met-Hb) in blood was also estimated by subtracting the sum of HHb, HbO2 and HbCO from total hemoglobin (total Hb.) Blood Met-Hb levels in six victims of city gas poisoning were practically negligible, while those in five victims of fire were 13.5-26.3%.

Liver damage in rats during acute carbon monoxide poisoning.

Plasma leucine aminopeptidase (LAP) levels and respiration rates of isolated liver mitochondria were studied in carbon monoxide (CO)-poisoned rats sampled at respiratory arrest. An increase in LAP levels paralleled a decrease in the respiratory control ratio and the ADP/O ratio. The results suggest that the damage to mitochondria closely correlates with the liver damage in rats during acute CO poisoning.

A practical method for the accurate determination of methemoglobin in blood containing carboxyhemoglobin.

Kinetics of the oxidation of carboxyhemoglobin (HbCO) by potassium ferricyanide was studied photometrically in a weakly acid solution. An increase in the absorbance at 630 nm reached a maximum within 10 min when over a 100-fold excess of ferricyanide to hemoglobin iron was used. A slight decrease in the absorbance was observed after completion of the reaction when over a 500-fold excess of the reagent was used. In the presence of 0.4% Sterox SE, the absorbance began to decrease without complete oxidation. From these findings, a simple, rapid and accurate method for the determination of methemoglobin (Met-Hb) in blood was devised. The method was compared with two other methods, using 11 blood samples containing various amounts of HbCO, and proved to be suitable for blood containing elevated HbCO as well as for ordinary blood.

Hyperuricemia in rats during acute carbon monoxide poisoning.

The effect of carbon monoxide (CO) inhalation on plasma levels of uric acid and hypoxanthine in rats was investigated. Exposure to 3% CO caused respiratory arrest within about 2 minutes. The plasma uric acid level of CO-treated rats increased to 157% above that of ether-treated rats. When rats were exposed to 1% or 0.8% CO, the exposure periods until the onset of respiratory arrest were prolonged, and plasma uric acid levels at respiratory arrest were further elevated. Plasma uric acid levels at respiratory arrest increased with prolongation of the exposure periods. Under our experimental conditions, hypoxanthine or xanthine was not detected in plasma of CO-treated rats. These results are discussed in relation to the hyperuricemia in hemorrhagic shock or hypoxemia: CO-induced hyperuricemia can be attributed to the stimulated degradation of adenine nucleotides under tissue anoxia, and thus could be an excellent parameter of tissue anoxia.

A simple spectrophotometric method for the determination of carboxyhemoglobin in blood.

The spectrophotometric method for the determination of carboxyhemoglobin (HbCO) in blood reported by Fretwurst and Meinecke was modified so as to give the same values of percentage HbCO (HbCO%) as those determined by the oxygen electrode method. Values of HbCO% of nine practical samples determined by both the oxygen electrode method and the present method were nearly identical regardless of the presence of methemoglobin (Met-Hb) in blood. The present method is suitable for forensic practice.

Contribution of peripheral input to length discrimination during use of a precision grip.

This study examined the ability of 63 subjects to discriminate a length difference, using a precision grip with the thumb and the index finger. To assess the relative contribution of different peripheral inputs associated with the discriminative ability of precision grip, three tasks were performed under each of three conditions-normal conditions; when vibratory stimuli were delivered to the forearm muscles; and when a local anesthetic was administered to the index finger. Under the first and second conditions, the length difference detected most frequently was 2.0 mm; in the third task, it was 3.0 mm. When a 100-Hz vibratory stimulus was applied to the forearm muscles, no difference in the threshold for length discrimination was observed for three of seven subjects. After injection of lidocaine into the metacarpophalangeal joint of the index finger, the threshold for length discrimination was significantly different for five of seven subjects. The detected length difference ranged from 1.0 to 4.0 mm. The authors question whether a single peripheral input is likely to make a major contribution to the complex perception that is used for discrimination tasks.

Mechanical strength of latissimus dorsi tendon transfer with Teflon felt augmentation.

Tensile properties of Teflon felt augmentation after latissimus dorsi tendon transfer to rotator cuff defect were examined in dog cadaver shoulders. Two experimental groups with latissimus dorsi tendon transfer were designed. In group 1 (right shoulders, n = 7) the tendon ends of latissimus dorsi were sutured to the greater tuberosity. In group 2 (left shoulders, n = 7) the repair was augmented with Teflon felt at the suture site. The ultimate tensile force and stiffness of each specimen in group 2 were individually compared with those in group 1. The average increases in ultimate tensile force and stiffness in group 2 were 53.8% and -3.7%, respectively. The individual value of ultimate tensile force in group 2 was significantly greater than that in group 1. These results demonstrate that augmentation at the repair site is mechanically effective latissimus dorsi tendon transfer. The augmentation procedure may provide protection during the healing period because of this and a surrounding fibrous tissue reaction.

Restoration of tensile properties at tendon insertion to bone by a patellar tendon-tibia autograft: an experimental study with canine infraspinatus.

A free patellar tendon-tibia autograft was performed to reconstruct a tendon insertion to bone of the canine infraspinatus. In this experimental study restoration of mechanical properties at tendon insertion to bone of the patellar tendon-tibia autograft was examined on 0, 42, and 84 postoperative days. Five dogs were used for each time point. Bone union occurred in all grafts by 6 postoperative weeks. The ultimate strength at the tendon insertion to bone recovered from 255.5 N on 0 day to 264.3 N and 439.3 N on 42 and 84 postoperative days, respectively. Stiffness recovered from 141.8 KN/m on 0 day to 201.8 KN/m and 226.3 KN/mN on 42 and 84 postoperative days, respectively. Our results demonstrate that patellar tendon-tibia autograft maintains excellent mechanical properties at tendon insertion to bone in the early healing period. This result suggests that transfer or free graft of tendons with attaching bone plug has a mechanical advantage for reconstruction of the rotator cuff, which may allow the patient to institute early postoperative mobilization.

Fibrous connection to bone after immediate repair of the canine infraspinatus: the most effective bony surface for tendon attachment.

The purpose of this histologic study was to identify the most effective bony surface for fibrous connection to bone after immediate repair of the canine infraspinatus. Light microscopic views were used to evaluate collagen fiber development. The left infraspinatus tendon of 15 dogs was transected and repaired to 3 different bone surfaces: a tendon end adjacent to the tendon insertion (group 1, n = 5), a calcified fibrocartilage layer (group 2, n = 5), and a cancellous surface (group 3, n = 5). Tendon repair to distal tendon ends restored the 4-layered enthesis in the healing period, whereas tendon repair to the calcified fibrocartilage layer considerably delayed fiber development into bone. Fiber connection to cancellous surface developed according to the remodeling of trabecular bone. Secure fiber connection into the thickened trabecular bone developed by 16 postoperative weeks. On the basis of these results, in clinical settings, ruptured tendon ends should be attached to the remaining distal tendon end or to a cancellous surface; they should not be attached to a calcified fibrocartilage layer.

Spontaneous rupture of mediastinal cystic teratoma into the pleural cavity: report of two cases and review of the literature.

The authors report on two female patients aged 12 and 14 years, who spontaneously developed a rupture of benign mediastinal cystic teratoma into the right pleural cavity. They presented with acute onset of severe chest pain and respiratory distress. The tumors were completely resected by thoracotomy. The serum and pleural fluid levels of carcinoembryonic antigens, CA-125 and CA19-9 were invariably elevated, then decreased to normal range after the surgical resection. Rapid diagnosis of this extremely rare complication is important because it may progress to a life-threatening condition.

Infantile hemangiopericytoma--case report and literature review.

Infantile hemangiopericytoma is a rare soft tissue neoplasm of pericytic origin and is almost always benign, despite its worrisome pathologic features. We describe a 2-month-old male infant with a soft tissue mass on his right thigh. Histologically, the lesion showed a characteristic hemangiopericytoma-like vascular pattern, multilobulation, and moderate mitotic activity. These morphologic features were prediagnosed as infantile hemangiopericytoma. However, immunohistochemical and ultrastructural studies revealed a heterogeneous cellular composition, primarily pericytes and endothelial cells, similar to that observed in infantile myofibromatosis.

Effects of tumor cell-derived interleukin 1 alpha on invasiveness of metastatic clones of murine RCT sarcoma through endothelial cells.

Interleukin 1 alpha (IL-1alpha) production and invasiveness through mouse lung endothelial cells (MLE) were investigated in high-metastatic RCT+ and low-metastatic RCT- clones established from poorly differentiated murine sarcoma. Apparently, a higher level of IL-1alpha was derived from RCT+ cells than from RCT- cells. In an invasion assay, the number of cells which penetrated the MLE monolayer in RCT+ was significantly greater than that in RCT-. The invasiveness of RCT+ and RCT- cells was stimulated by additional recombinant mouse IL-1alpha (rIL-1alpha) in a dose-dependent manner. Anti-mouse IL-1alpha monoclonal antibody (anti-IL-1alpha mAb) significantly inhibited the invasiveness of RCT+ and RCT- cells through the MLE monolayer. However, in RCT+ cells these effects were higher than in RCT- cells. In an attachment assay, the ability of RCT+ cells to attach to the MLE monolayer was significantly higher than that of RCT- cells. The attachment ability of RCT+ and RCT- cells to the MLE monolayer was significantly increased by the pretreatment with rIL-1alpha in a dose-dependent manner. In a retraction assay, conditioned medium of RCT+ stimulated the retraction of the MLE monolayer more markedly in comparison with conditioned medium of RCT-. The retraction of the MLE monolayer was stimulated by additional rIL-1alpha in a dose-dependent manner. The increased retraction of the MLE monolayer was closely associated with the enhancement in tumor cell invasiveness. These findings suggest that IL-1alpha derived from RCT+ and RCT- cells might contribute to the enhancement of tumor cell invasion by stimulating the attachment to the MLE monolayer and retraction of the MLE monolayer.

Effects of vascular endothelial growth factor and E-selectin on angiogenesis in the murine metastatic RCT sarcoma.

The relationship between vascular endothelial growth factor (VEGF) and induction of angiogenesis in high-metastatic RCT(+) or low-metastatic RCT(-) clones of the poorly differentiated murine RCT sarcoma was investigated. The association with E-selectin in VEGF-induced angiogenesis was also evaluated. RCT(+) cells produced significantly larger amounts of VEGF than RCT(-) cells. In a tube formation assay with murine lung microvascular endothelial (MLE) cells, conditioned medium from RCT(+) cells showed a significantly greater effect on tube formation than that from RCT(-) cells. Induction of tube formation was suppressed by anti-mouse VEGF monoclonal antibody. Furthermore, anti-mouse E-selectin monoclonal antibody suppressed the tube formation induced by recombinant mouse VEGF. In a flow-cytometric analysis, the expression of E-selectin on MLE cells was upregulated after pretreatment with conditioned medium from RCT(+) and RCT(-) cells. Conditioned medium from RCT(+) cells induced a higher expression of E-selectin compared to medium from RCT(-) cells. Anti-VEGF monoclonal antibody prevented the upregulation of E-selectin by the RCT cell-conditioned medium. These findings suggest that E-selectin plays an important role in the angiogenesis induced by VEGF. VEGF derived from tumor cells may enhance angiogenesis by upregulating the expression of E-selectin on vascular endothelial cells.

Directional excitability of the utricle: a study by thermoconvective stimulation.

The frog utricular macula was stimulated by thermoconvective flow in four different directions. The evoked utricular nerve action potentials were recorded via a glass suction electrode. The maximum spikes and the time constants of the potentials were counted. The spike counts and the time constants were not significantly different among stimuli. This suggests that the utricle has the same physiological excitability to any directional stimulus.
