Identification and characterization of protein products of the cot oncogene with serine kinase activity.

The products of the cot gene, a novel oncogene isolated by DNA transfection assay using the hamster cell line SHOK, were identified as 46 kDa and 52 kDa proteins by using anti-peptide antibodies. The 46 kDa and 52 kDa proteins both showed autophosphorylation activity at serine residues. The two forms of the Cot protein were suggested to differ in their amino-terminal structures as a result of alternative initiation of translation. Subcellular fractionation revealed that the 46 kDa and 52 kDa proteins are both predominantly localized in the cytosol. These Cot proteins are the fourth oncogene products with serine kinase activity identified.

[A case of tuberculous osteomyelitis resembling metastatic cancer on bone scintigraphy].

A 48-year-old male was referred to our division, on the suspicion of lung cancer with bone metastasis. Chest radiography showed a mass shadow and bone scintigram demonstrated multiple scattered regions of increased uptake. However bronchial and bone biopsy specimens revealed granuloma with epithelial cells and giant cells. A diagnosis of tuberculous osteomyelitis was made, and antituberculous therapy was begun. Chest radiography and bone scintigrams one year later showed marked improvement.

[Hemodynamic changes and ventricular function before and after Fontan procedure for univentricular heart].

Ventricular function and hemodynamic parameters before and after Fontan operation were studied in patients with univentricular heart (UVH) of left ventricular (LV) type (9 cases, ages: 6-66, mean 13 years) and right ventricular (RV) type (13 cases, ages: 6-17, mean 9 years) without atrioventricular valve regurgitation. The preoperative ejection fraction (EF) was poor and only 44% of LV type and 23% of RV type met the Choussat's criterion (EF greater than or equal to 60%). There were 2 hospital deaths each in LV type and in RV type, but their causes could not be attributed to preoperative parameters of ventricular function. In survivors, the EF reduced from 62.9 +/- 7.0% before Fontan procedure to 43.4 +/- 12.6% after the procedure in LV type (p less than .05) and from 54.5 +/- 7.0% to 47.0 +/- 9.0% in RV type (p less than .01), although the ventricular end-diastolic pressure (EDP) also reduced from 12.7 +/- 2.7 mmHg before to 6.6 +/- 1.9 mmHg after the operation in LV type (p less than .05) and 10.4 +/- 2.6 mmHg to 5.5 +/- 2.4 mmHg in RV type (p less than .05). the ventricular end-diastolic volume (EDV) also reduced from 185 +/- 35% to 126 +/- 58% of expected normal volume in LV type (p less than .05) and 173 +/- 28% to 99 +/- 18% of expected normal volume in RV type (p less than .01).(ABSTRACT TRUNCATED AT 250 WORDS)

Descending axonal projections from the medial parabrachial and Kölliker-Fuse nuclear complex to the nucleus raphe magnus in cats.

In 9 Nembutal-anesthetized and vagotomized cats, a total of 42 units, including 2 respiratory units, recorded from the medial parabrachial (NPBM) and Kölliker-Fuse (KF) nuclear complex were found to be antidromically activated by electrical stimulation of the nucleus raphe magnus (NRM). The latencies ranged from 0.4 to 2.5 ms (mean 1.1 ms). In 5 cats, following injection of WGA-HRP (wheat germ agglutinin-conjugated horseradish peroxidase) into the NRM, a number of retrogradely labelled neurons were observed in the rostral pons, mainly in the NPBM, KF and nearby pontine area. These results demonstrate that mainly non-respiratory neurons in the rostral pons, especially in the NPBM and KF nucleus, send monosynaptic axonal projections to the NRM.

The effects of halothane on the contents of putative transmitter amino acids in whole rat brain.

The effects of halothane on brain amino acid contents were investigated in rat by using a gas chromatography-mass spectrometry system. The contents of glycine and glutamate did not show any consistent changes in 1% or 2% halothane, but increased in 3% or 4% halothane. The levels of gamma-aminobutyric acid were not altered in any concentrations of halothane. The levels of aspartate increased in a dose-dependent manner up to 3% halothane, but returned to the control level in 4% halothane. These findings suggest that halothane brings the alteration of brain amino acid contents by affecting their synthesis and catabolism.

Phylogeny of Nannizzia incurvata, N. gypsea, N. fulva and N. otae by restriction enzyme analysis of mitochondrial DNA.

Sequence divergence among Nannizzia incurvata, N. gypsea, N. fulva and N. otae was studied genetically using digestion profiles of mitochondrial DNA with restriction enzymes, Hae III, Hha I, Hind III and Xba I. Only N. fulva was subdivided into two types on restriction profiles. Phylogenetic relationships suggest that 1) N. gypsea is more closely related to N. fulva than N. incurvata, 2) the phylogenetic distance between N. otae and the other three species is larger than the distances between the other three species.

Multiple free (aorta-coronary) gastroepiploic artery grafting.

The use of in situ gastroepiploic artery grafts has been reported recently. In the present case, the in situ gastroepiploic artery was too short to graft both the diagonal and the obtuse marginal branches of the coronary arteries in a patient with diffuse coronary artery disease. The free (aorta-coronary) gastroepiploic artery graft was anastomosed to both arteries to form a Y graft.

Exercise-induced silent myocardial ischemia in patients with vasospastic angina.

UNLABELLED: To clarify the incidence and clinical characteristics of exercise-induced myocardial ischemia in patients with vasospastic angina, we performed exercise thallium computed tomography in 25 patients who had no significant coronary artery stenosis greater than 70%. Coronary artery spasm was documented by coronary angiography in all patients. Eleven patients (44%) developed exercise-induced perfusion defects, but only four of them had anginal pain (36%). Diltiazem (90 mg, administered orally) prevented the development of exercise-induced perfusion defects in all patients. Multivessel coronary spasm was documented by coronary angiography in 11 patients, and nine of them (82%) showed exercise-induced perfusion defects (p less than 0.05). CONCLUSION: (1) Exercise-induced myocardial ischemia was demonstrated in 44% of patients who had vasospastic angina without fixed coronary stenosis, and 64% of them were asymptomatic. (2) Patients with multivessel spasm had a greater prevalence of exercise-induced myocardial ischemia than those with single-vessel spasm.

[Protective effect of eptazocine, a novel analgesic, against cerebral hypoxia-anoxia in mice].

Cerebral protective effect of eptazocine, a mu-antagonist-kappa-agonist, was investigated using mice subjected to hypoxia-anoxia. Eptazocine (1 to 10 mg/kg) prolonged the survival time of mice subjected to KCN (3 mg/kg, i.v.) injection in a dose-dependent manner, and this effect was completely inhibited by naloxone (5 mg/kg). EKC, U50,488H, opioid kappa-agonists, also had such an effect, but were weaker than eptazocine. In mice exposed to hypobaric hypoxia (190 mmHg), eptazocine (3, 10 mg/kg) and EKC (10 mg/kg) prolonged the survival time, but morphine (5 mg/kg) and pentazocine (10 mg/kg) shortened the time. The eptazocine effect was attenuated by either naloxone (5 mg/kg) or atropine (0.5 mg/kg), different from what was seen in the case of physostigmine and diazepam, and the combination of eptazocine (1 mg/kg) and physostigmine (0.075 mg/kg) had a potentiating effect. MR-2266, a selective kappa-receptor antagonist, inhibited the eptazocine effect more potently than naloxone. These results suggest that eptazocine elicited its cerebral protective effect via its binding with opioid kappa-receptors and probably an activation of the central cholinergic system.

Effects of eptazocine, a novel analgesic, on KCN-induced changes in the cerebral contents of glycolytic metabolites and high-energy phosphates in mice.

Effects of eptazocine on cerebral metabolic changes due to a sublethal dose of KCN were investigated in mice. KCN (2 mg/kg, i.v.) induced a temporary loss of consciousness being moderated by eptazocine (1-10 mg/kg) in a dose-dependent manner. The KCN injection decreased the contents of phosphocreatine (PCr), ATP and glucose and increased the contents of AMP and lactate, resulting in a 34% decrease in energy charge potential (ECP) and an increase in lactate/pyruvate (L/P) ratio. Such changes were improved by eptazocine (10 mg/kg) and EKC (3 mg/kg), but not by pentazocine (10 mg/kg) and morphine (3 mg/kg), and the improving effect of eptazocine was completely inhibited by MR-2266 (3 mg/kg), a relatively selective opioid kappa-receptor antagonist. On the other hand, eptazocine (3, 10 mg/kg) was found to increase the glucose content in normal mice, but not to give significant changes in the contents of glycolytic metabolites and high-energy phosphates. These results suggest that eptazocine may improve anoxic changes in cerebral energy metabolism.

[Analyses of factors contributing to the erectile dysfunction with aging by nocturnal penile tumescence, penile blood pressure index and papaverine test].

To evaluate the factors contributing to the decline in erectile function with age, we performed measurement of nocturnal penile tumescence (NPT) and penile vascular examinations in 407 men. The following results were obtained. 1) The penile circumferential increment during NPT measured by an erectometer was gradually decreased with age, especially markedly after 60 years of age. The percentage of subjects in whom the NPT was less than 10 mm increased with age: 2.7% in the 5th decade, 7.1% in the 6th decade, 16.7% in the 7th decade and 29.1% in the 8th decade. These findings indicate that the incidence of organic erectile dysfunction increased with age. 2) To evaluate the penile vascular status, measurement of penile blood pressure index (PBPI) and papaverine test were performed. The mean PBPI values were significantly decreased in the subjects over 70 years of age. The percentage of subjects having good response to papaverine injection obviously decreased with age: 66.7% between 50 to 59 years, 48.6% between 60 to 69 years, and 15.8% of over 70 years. All subjects in whom the NPT was less than 10 mm showed either a low PBPI level of less than 0.6 or an incomplete response to papaverine injection. These results suggest that penile vascular impairment plays a major role in the age-associated decline in erectile function. 3) We investigated the relationship between cigarette smoking and penile vascular impairment. Smokers showed not only lower PBPI values but higher incidence of the incomplete response to papaverine injection than nonsmokers. Therefore, it is considered that cigarette smoking is a significant risk factor in the erectile dysfunction that occurred in the aged.

[Clinical results of sequential and "Y" grafting with the internal mammary artery].

One hundred and sixty-five patients (125 men, 40 women; age ranged from 40 to 81 years), underwent coronary artery bypass grafting in the 3 years period from 1986 to 1988. The internal mammary artery (IMA) was applied to 125 patients (75.8%). The sequential left IMA grafting to the left anterior descending coronary artery (LAD) and the diagonal branch (Dg) was performed in 9 out of 125 patients. In 3 of 9 patients, the right IMA was simultaneously grafted to the obtuse marginal branch (OM). In situ bilateral IMAs grafting to the LAD and Dg or LAD and OM were used in 13 patients. In three patients, "Y" grafting (the free right IMA was anastomosed to the side of the in situ left IMA) was performed for LAD and Dg. Eighty-one of 82 IMA grafts (99%) were patent about 1 month after operation. Three patients (1.8%) died during hospitalization, one from Low Output Syndrome, two from cerebral infarction. Perioperative complication included myocardial infarction in 5 (3.0%), cerebral infarction in 10 (6.0%). We concluded that the IMA could be applied in multivessel grafting as well as single vessel grafting in severe diffuse coronary artery disease in the Japanese, and the early patency was good even in the sequential and "Y" grafting.

Evaluation of a bronchial anastomosis by laser Doppler velocimetry.

Healing after bronchoplasty was evaluated by assessing the bronchial mucosal blood flow by laser Doppler velocimetry in dogs. Bronchoplastic surgery at the right main bronchus was performed and bronchial mucosal blood flow was determined by laser Doppler velocimetry at proximal and distal sides of the anastomosis before and after operation. Four experimental groups were established. After operation the blood flow was adequately preserved, and healing of the anastomosis site was satisfactory in the minimum detachment group and steroid-treated group. Mucosal blood flow was markedly reduced in both proximal and distal sides in the extensive detachment group. The extent of the reduction in the blood flow was smaller in the omentum dressing group than in the extensive detachment group. The state of healing of the anastomosis site was closely related to the bronchial mucosal blood flow.

[Surgical treatment of small cell lung cancer].

Thirty four small cell lung cancer (SCLC) patients underwent operation in Kyoto University from 1976. The mean age was 62 years old; 25 male cases, 9 female, respectively. All case were limited disease (LD) except 2 patients with rib metastasis. Seventeen patients had a performance status (PS) of 0 and 17 PS 1, twenty eight patients were treated with chemotherapy. Of the remaining 5 patients, 2 were treated by only oral 5-FU derivatives, one died within 30 days of operation, one could not be treated owing to old-age, and one refused chemotherapy. Except one operational death 33 cases were able to be evaluated. Out of 28 patients, 16 patients were treated by only postoperative chemotherapy. Concerning operational radicality, 21 cases out of 31 were operated radically (68%). The survival rates of these 33 patients were 78% 37% at 1, 3 and 5 years respectively. The MST was 26 months. The neoadjuvant group head better results than the non-adjuvant group.

[A case of anomalous right pulmonary venous drainage to high segment of superior vena cava with intact atrial septum--surgical repair by cavoatrioplasty avoiding injury to the sinoatrial node and its feeding artery].

Surgical repair of anomalous right pulmonary venous drainage to the high segment of the superior vena cava (SVC) with intact atrial septum in a 5-year-old girl by cavoatrioplasty avoiding injury to the sinoatrial (SA) node and its feeding artery is described. The procedure was composed of #1: an oblique incision at the anterior wall of SVC above the anomalous right pulmonary veins (ARPVs) and closure of the lower end of the SVC, making the lower segment of SVC the ARPV channel, #2: a fan-shaped incision at the base of the right atrial (RA) appendage and suturing the cephalad flap over a created atrial septal defect, diverting the ARPV flow through the SVC into the left atrium, and #3: reconstruction of continuity between the cephalad end of the SVC and the RA by suturing the RA appendage flap over the lower segment of SVC. Pulmonary valvotomy was also done for concomitant pulmonary stenosis. Postoperative course of the patient was excellent, with constant normal sinus rhythm. Angiography one month after operation showed no stenosis of the SVC and pulmonary venous pathways. The method reported here was considered to have following advantages: It employs simple and short incisions, avoids injury to the SA node its feeding artery, and achieves wide and nonobstructive pathways of both SVC and pulmonary venous return without using foreign material.

[A case of successfully managed acute asthmatic attack by cervical epidural block during mechanical ventilation].

A 47-year-old male suffering from an acute asthmatic attack was admitted in the morning. He was arrested on arrival. Following successful resuscitation, he was treated by using controlled ventilation with a Bennett 7200 respirator. Before starting mechanical ventilation, he was physically exhausted and had carbon dioxide retention (PaCO2 76.2 mmHg). The patient received dexamethasone 40 mg/day and theophylline 400 mg/day during the first 4 days. Despite the decrease in PaCO2 following mechanical ventilation, cycling pressure remained high (up to 50-70 cmH2O) even after 24 hours. Because of the risk of barotrauma due to high cycling pressure, we conducted cervical epidural block to reduce airway hyperreactivity to the endotracheal tube and endotracheal suctioning. Six to 10 ml of lidocaine with 1:200,000 epinephrine was administered every 1 or 2 hours. Within 4 hours after epidural block, airway pressure decreased to 30-40 cmH2O. Since then, requirements of sedative (diazepam) have decreased greatly to 10 mg/day. The patient was successfully extubated 4 days after admission. Finally, epidural block was continued for 6 days without any complication. We do not know how cervical epidural block works to reduce airway pressure. It might be beneficial to manage patients artificially ventilated during asthmatic attack.

[Prosthetic valve implantation without removal of tricuspid apparatus for an old case of corrected transposition of the great arteries].

Prosthetic valve implantation was performed for the systemic atrioventricular regurgitation in an old patient with corrected transposition of great arteries. SJM27M prosthesis was implanted without removing posterior and septal leaflet, chordae and papillary muscles with respect to preserve the function of the morphological right ventricle. It is important to preserve the function of the morphological right ventricle because it must continuously play a role of systemic ventricle even after surgical procedure.

[A comparative study of myocardial scintigraphy with 99mTc-SQ-30,217 and 201Tl in cases with myocardial infarction].

A comparative study of myocardial scintigraphy with 99mTc-SQ-30217 (SQ) and 201Tl was performed in 6 patients having ischemic heart diseases which were confirmed by the coronary angiography. The SQ study was done 1 week after the 201Tl study. Both studies were performed in exercise and resting state. Degree of perfusion defect was compared between SQ and 201Tl SPECT by means of scoring 0-2 (0: no defect, 1: equivocal, 2: definite defect) by five doctors (two physicians and three radiologists). Ability of this agent to detect the ischemic lesions was similar to that of 201Tl except for two regions. Effect of the liver image on the evaluation of the inferior wall of the left ventricle was small on this study, although the hepatic accumulation of the tracer was prominent. This agent is a promising tracer for the evaluation of myocardial perfusion in the cases with myocardial infarction.
