[A case of pulmonary sarcoidosis with circulatory insufficiency of segmental and subsegmental branches of pulmonary arteries].

A 50-year-old woman was admitted to our hospital on April 4, 1992, because of progressive worsening of dry cough and exertional dyspnea. Moderate hypoxemia and obstructive ventilatory impairment were present. Her chest roentgenogram and CT films showed thickened bronchovascular bundles in the absence of significant parenchymal fibrosis. Transbronchial lung biopsy revealed the formation of noncaseous epithelioid cell granulomas. Pulmonary perfusion scintigraphy showed multiple perfusion defects predominantly in the upper lung fields. Pulmonary ventilation scintigraphy showed a normal pattern. After administration of prednisolone for 3 months, her chest roentgenogram and CT films demonstrated marked decrease of infiltrates, but there was no improvement of hypoxemia and perfusion defects on pulmonary perfusion scintigraphy. Pulmonary angiography revealed multiple stenoses and occlusions of segmental and subsegmental branches of pulmonary arteries. Long-term steroid treatment will be necessary for this rare form of pulmonary sarcoidosis to prevent the development of pulmonary hypertension.

[Recent epidemiological features of sarcoidosis in Japan].

The 8th nation-wide epidemiological survey on sarcoidosis in Japan was carried out in 1991. Total number of sarcoidosis patients was increased two times more than that of the 7th survey which was carried out in 1984. Moreover, the epidemiological features of the disease were different, at least in part, from those of the previous survey, especially on the age distribution and the frequency of cases with symptoms. These differences might be depend on the changes of the diagnostic methods including the reduction of systemic mass radiography.

[Specificity of surface antigen and function of BAL T cells].

To elucidate the pathogenesis of sarcoidosis, we mainly investigated the surface molecules including adhesion molecules on BAL T cells and the functions of BAL T cells such cytokine production, [Ca2+] mobilization, PKC expression. Results are following. Adhesion molecules were up-regulated on BAL T cells compared with peripheral T cells, but cytokine production was decreased via TcR stimulation through [Ca2+] mobilization was occurred. PKC isozyme alpha expression was decreased compared with peripheral blood T cells.

Total hip replacement using femoral neck bone to graft the dysplastic acetabulum. Follow-up study of 18 patients with old congenital dislocation of the hip.

We performed total hip replacement on 25 congenitally dislocated hips using corticocancellous bone from the femoral neck as a bone graft to reconstruct the deficient acetabulum. Two patients (two hips) died less than five years after surgery and three hips developed deep infection. We reviewed the remaining 20 hips at an average interval of 8.4 years (5.2 to 12.9). Their functional scores (modified Merle d'Aubigné and Postel) averaged 5.7 for pain, 5.1 for walking ability, and 4.2 for range of motion. Radiographs showed union and remodelling of the grafted bone in every case. There were radiological signs of aseptic loosening in three sockets (15%) without collapse of the grafted bone. None of the femoral stems was loose. No revision operation has been performed. Our results confirm the usefulness in the long term of femoral autografts for severe acetabular deficiency.

[Dielectric behavior of the rabbit cornea as a measure of the healing process in injured epithelium].

To correlate changes in the passive electrical properties of the rabbit cornea with quantitative grading of corneal injuries induced by topical application of 70% ethanol, we measured ocular tissue impedances using a surface electrode over the range of 10(4)-10(8) Hz and followed their temporal changes for up to 17 days. Dielectric measurements on control eyes yielded a broad dispersion curve, which, in loss tangent terms, could be decomposed into two components: dispersion 1 on the low-frequency side and dispersion 2 on the high-frequency side. By defining the peak value of the total dispersion as Pt and those of subdispersions 1 and 2 as P1 and P2, the ratios, P1/Pt and P2/Pt, were found to serve as useful indices. Upon appearance of corneal erosion due to ethanol, P1/Pt markedly increased and returned to the control level with re-epithelialization of the cornea, and the time course of P2/Pt showed a mirror image to that of P1/Pt. Both ratios correlated well with the erosion area determined photographically. These results indicate that dielectric spectroscopy is applicable to the assessment of the extent and severity of corneal injury.

[A case of paradoxical cerebral embolism through a patent foramen ovale diagnosed by necropsy].

A 62-year-old man was admitted to our hospital for disturbed consciousness and left-side motor sensory deficit. His blood pressure was 142/100 mmHg and an electrocardiogram was normal. CT of the brain revealed a low density area in the distribution of the right middle cerebral artery (MCA). Cerebral angiography demonstrated occlusion of the right internal carotid artery and an embolus. We diagnosed cerebral embolism, but we could not detect source of embolism. His condition deteriorated rapidly, and he died four days later. At necropsy, an elongated piece of thrombus 10 cm in length was lodged in a patent foramen ovale lying in each atrium. No other thrombus was seen in the heart, the myocardium and coronary arteries were normal. Both main pulmonary arteries contained thrombus. The lungs showed edema and congestion. There was no thrombus of both common iliac veins. The right internal carotid artery was occluded at its bifurcation by embolus, distal to which the cavernous segment was 7 cm filled with clot. We reported a rare paradoxical cerebral embolism through a patent foramen ovale diagnosed by necropsy. If no left-sided circulatory source can be demonstrated, the possibility of a paradoxical embolism should be considered.

[New regulation of tap water quality and exposure assessment].

Recently, social concerns regarding tap water quality have increased, mainly because of the possible reduction in safety due to contamination of tap water by various chemicals and more frequent occurrence of the strange odors and tastes in as a result of resource water eutrophication. Consequently, the Ministry of Health and Welfare conducted a detailed two-year investigation of the Water Quality Standards of the Water Works Law by summoning an expert committee, and as a result, totally revised the Standards in December 1992. This was the first overall revision since 1958 when the Water Quality Standards were established, and an additional 21 items, including mainly hazardous chemicals including pesticides and chlorinated by-products, were newly added. Values and testing methods are now listed for 46 items, and the Law obliges every water supply to conduct periodical water testing for the necessary items almost every month with ad-hoc testing, as required of hydrants. Simultaneously, 26 monitoring items are listed in hazardous contaminants guidelines which should be checked, when necessary. The new Water Quality Standards and the guidelines were brought into force in December 1993, and the quality of all parts of the water supply is now tested in accordance. Risk assessment is a scientific process that includes some form of measurement as one of its central elements. In many cases, the measured parameter is the level of exposure to a hazard. Also, measurements are essential in establishing the quantitative relationship between exposure and response, and in determining natural baseline conditions in the environment. Exposure assessment is the process of measuring or estimating the intensity, frequency, and duration of human or other population exposures to risk agents. Exposures may occur in a variety of ways, such as through ingestion, dermal contact, or inhalation. For many risk assessments, exposure assessment is the most difficult task. The reason for this is that exposure assessment often depends on factors that are hard to estimate and for which there are few data. Critical information on the conditions of exposure is often lacking. To be comprehensive, an exposure assessment must describe the levels of exposure and all conditions that might be needed to assess the effects of such exposures, including their magnitude, duration, schedule, and route. This report presents the various problems covered in exposure assessment relevant to monitoring, testing, and methodology.

[Phrenic nerve injury following coronary artery bypass surgery: its clinical implication and management].

The hospital records of 80 patients undergoing coronary artery bypass surgery, including 19 patients using the "isolation pad", were reviewed to examine the prevalence of the phrenic nerve injury (PNI). All patients received ice slush for topical cooling during operation. The degree of the elevated diaphragm was evaluated objectively by "Diaphragmatic Elevation Index, DEI" calculated from the plain chest roentgenogram taken before and after the operation. Sixty-one patients without using the isolation pad were subdivided into 3 groups, mild (35), moderate (15), and severe (11) according to the degree of the elevated diaphragm, and compared along with those using the isolation pad. All groups could not be distinguished by clinical course. The degree of left diaphragmatic elevation was significantly high compared with that of right, but recovered significantly at discharge except 2 patients. Pulmonary complication was highly associated with the "severe" groups. Because PNI is often transient and of little clinical significance, it is sometimes neglected. But considering the high frequency of the development of pulmonary complication, proper protective measure should be undertaken. Utilization of the isolation pad was effective means in terms of protection of phrenic nerve.

Abnormal excretion of urinary phospholipids and sulfatide in patients with mitochondrial encephalomyopathies.

We found that patients with mitochondrial encephalomyopathies excreted urinary phosphatidylethanolamine, cardiolipin, and phosphatidylserine most likely derived from mitochondria and sulfatide which is specific to myelin or the kidney. It is of interest that four patients with myoclonus epilepsy with ragged-red fibers and one patient with chronic progressive external ophthalmoplegia all showed qualitatively similar abnormal excretion of such urinary lipids. It is conceivable that the urinary acidic phospholipids reflect abnormalities in the mitochondrial phospholipids, which are very important for mitochondrial enzymatic activities.
