Prosthetic testicle: appearance at MR imaging.

The appearance of testicular prostheses on magnetic resonance (MR) images was studied in six patients with seven silicone implants: two with a fluid consistency and five that were solid. On T1-weighted images, both types of prosthesis demonstrated homogeneous low signal intensity; on T2-weighted images, the fluid implants had a uniformly low signal intensity, and the solid implants had a uniformly high signal intensity. Both prostheses created a chemical shift artifact on all imaging sequences owing to the difference in chemical composition of the prostheses and the surrounding tissue.

Human lung deposition of particles suspended in air or in helium/oxygen mixture.

Deposition in mouth and throat and the fraction of alveolarly deposited particles in the lung of 3.6- to 3.8-microns Teflon particles labeled with 99mTc were estimated in nine healthy subjects. The particles were inhaled in air or helium/oxygen mixture with a flow of 0.5 l/s by subjects with or without induced bronchoconstriction. The bronchoconstriction (two- to threefold increase in airway resistance) was induced by an aerosol of methacholine bromide. As the Reynolds number is three times lower for the helium/oxygen mixture than for air, and the sedimentation rate of the particles is about the same in both, a different regional deposition between particles suspended in air and helium/oxygen mixture should be due to turbulence. Deposition in mouth and throat did not differ significantly between air and the helium/oxygen mixture. The alveolarly deposited fraction tended to be larger for unconstricted airways and was significantly larger for constricted airways for inhalations in the helium/oxygen mixture compared to air. In real life, air pollutants and therapeutic aerosols may be inhaled with larger flow rates and broncho-constriction may be more pronounced in patients, so that deposition of particles due to turbulence can be important.

Properties of verapamil-hypersensitive multidrug-resistant Chinese hamster ovary cells.

Two vincristine-resistant Chinese hamster ovary cell lines have been shown previously to be hypersensitive to the calcium channel blocker, verapamil. They are now shown to be hypersensitive to the membrane-active agent quinidine sulfate and to the calcium channel blockers diltiazem and nicardipine. Hypersensitivity to quinidine sulfate implies that calcium channels are not the primary target for these drug effects on these cell lines and is consistent with our previous observation that their calcium accumulation is normal in the presence and absence of verapamil. The two cell lines have elevated levels of membrane P-glycoprotein and of two cytosolic proteins, Mr 27,000 and pI 6.0 and 6.4. Revertants have normal levels of these cytosolic proteins, suggesting that these proteins may play a role in conferring resistance. [3H]Verapamil accumulation by the two cell lines is lower than in controls. One of the cell lines has been hybridized to normal cells and the vincristine resistance and verapamil sensitivity of three hybrid clones has been determined. Vincristine resistance is semidominant but verapamil hypersensitivity is completely recessive.

Methadone maintenance patients in general medical practice. A preliminary report.

Medical maintenance is the treatment by primary care physicians of rehabilitated methadone maintenance patients who are stable, employed, not abusing drugs, and not in need of supportive services. In this research project, physicians with experience in drug abuse treatment provided both the pharmacologic treatment of addiction as well as therapy for other medical problems, as needed. Decisions regarding treatment were based on the individual needs of the patient and on currently accepted medical practice rather than on explicit regulations. We studied the first 40 former heroin addicts who were transferred to this program from more conventional methadone clinics. At a follow-up visit at 12 to 55 months, 33 (82.5%) of 40 patients had remained in treatment; five (12.5%) had been discharged because of cocaine abuse and two (5%) had been voluntarily discharged. Personal benefits of medical maintenance include the dignity of a standard professional atmosphere and a more flexible reporting schedule. This program has the potential for improving treatment of selected methadone maintenance patients.

Suppression of experimental tractional retinal detachment by low-dose radiation therapy.

We used a standardized model of traction retinal detachment (TRD) created by cellular membranes in the rabbit to test the effects of low-dose radiation therapy in suppressing TRD. The vitreous and lens were removed from pigmented rabbits, and homologous conjunctival fibroblasts were grown in cell culture. After resolution of postoperative inflammation, 50,000 fibroblasts in 0.1 mL of culture fluid were injected into the vitreous cavity. Ten eyes were maintained as controls. Nineteen eyes received 6 Gy (600 rad) of x-ray irradiation one to three hours after cellular injection. Eyes were monitored weekly for three weeks with indirect ophthalmoscopy. Seven (70%) of ten control eyes developed TRD at one week; no additional TRDs were noted at weeks 2 and 3. Significantly smaller numbers of irradiated eyes developed TRD: at week 1, two (11%) of 19; at week 2, five (28%) of 18; and at week 3, five (29%) of 17.

A method for the manufacture of single barrel liquid ion-selective microelectrodes: an in situ study of ant venom pH.

A method for the manufacture of single barrel ion-sensitive microelectrodes, employing liquid ion-selective sensors, is described in detail. This method had proven reliable and repeatable for the manufacture of pH-sensitive microelectrodes using a liquid proton carrier. It is believed that this method has advantages over other published methods since it allows direct visualization and control of major steps during electrode fabrication and overcomes several problems often encountered using more conventional methods. Microelectrodes fabricated using this technique have been used to measure the in situ pH of venom from two myrmicine ants, Tetramorium caespitum (L.) and Myrmica ruginodis (N.).

AIDS in ICUs: outcome.

The admission of patients with acquired immune deficiency syndrome (AIDS) to intensive care units in the United Kingdom was surveyed in January 1986. Ninety-three intensive care units completed the questionnaire. Thirty-two patients had been admitted to 12 units up to that time. Twenty-five (78%) of these patients had received artificial ventilation of the lungs. The commonest cause of respiratory failure on admission was Pneumocystis carinii pneumonia; this occurred in 27 patients, seven of whom also had pulmonary cytomegalovirus infection. Four patients had Kaposi's sarcoma and three of these patients also had Pneumocystis carinii pneumonia. The overall mortality was 72%; twenty (80%) of the patients who required artificial ventilation, died. Ten patients survived to leave the intensive care unit, but one patient died of respiratory failure prior to discharge home.

A cluster of three cases of aplastic anaemia in children.

During a 2-week period three unrelated children presented with severe aplastic anaemia at a general hospital serving a population of 25,000 children aged 0-15 years. The probability of this occurring by chance alone was 0.00009 (exact probability cluster analysis). Serology for common viral infections including hepatitis A & B and infectious mononucleosis was negative. It was not possible to demonstrate IgM antibodies to human parvovirus (HPV) by radioimmunoprecipitation or HPV virions by DNA hybridization in the patients or any members of their families. Epidemiological investigation failed to demonstrate a common environmental toxin. It did reveal, however, that all three patients had spent time, within the preceding 3 months in a swimming pool and its surrounding area in a region of Cardiff where none of them resided. Pool maintenance was satisfactory and water analysis showed no abnormality. The possibility must remain that the cluster was caused by an undisclosed environmental toxin.

Cellular polypeptides overexpressed after herpes simplex infection permit virus subtyping and may help diagnose cervical cancer.

The synthesis of cellular macromolecules is inhibited after infection with herpes simplex viruses (HSV) although certain host proteins accumulate to high concentrations as identified by monoclonal antibody TG7A. By western blotting, a polypeptide with a relative molecular weight of 90 kilodaltons was identified in cells infected with type 2 viruses and a polypeptide of 40 kilodaltons relative molecular weight in type 1 infected cells, and virus typing was confirmed by restriction enzyme analysis of viral DNA. Thirty seven clinical isolates from the genital region were subtyped as HSV type 2 and 18 from the orofacial region as type 1 by the different intracellular location of the 90 kilodalton and 40 kilodalton proteins seen on immunofluorescent staining of cells infected with HSV. Expression of these proteins has been associated with cellular transformation due to gene products of HSV or other viruses. Overexpression of the cellular proteins identified by TG7A reactivity was shown to be a marker for cells in cervical smears from patients with CIN III that appeared to be dyskaryotic. Little or no reaction was observed in squamous epithelial cells found in normal or abnormal smears.

Premenstrual syndrome research: using the NIMH guidelines.

The 1983 National Institute of Mental Health (NIMH) guidelines for a premenstrual syndrome (PMS) diagnosis were applied to Daily Rating Form (DRF) data in a prospective research study. The use of two sets of criteria, each consistent with the NIMH guidelines, resulted in prevalence rates of 44% and 17%. The authors discuss other problems in defining a "syndrome": symptom patterns and the relationship between PMS and the presence of a psychiatric disorder.

Neurofibromatosis hyperkyphosis: a review of 33 patients with kyphosis of 80 degrees or greater.

Thirty-three patients with hyperkyphosis secondary to neurofibromatosis were reviewed as to the natural history of the deformity, complications of the untreated deformity, and results of treatment. The kyphoses ranged from 80 to 180 degrees, averaging 132 degrees. The natural history was not good, six patients presenting with paraparesis and six with respiratory distress. Twenty-seven patients had operative treatment. Laminectomy was useless for the treatment of paraparesis due to angulated spine deformity. Combined anterior and posterior spine fusion using abundant amounts of autogenous bone for both, coupled with prolonged rigid immobilization, gave the best results. Halo traction and halo casts were invaluable for the more severe deformities. Instrumentation was often impossible due to the severe deformity. All patients operated on by the authors eventually achieved solid union.

Fonda-Anderson reading chart for normal and low vision.

The purpose of our chart is to show all scientific and practical demands for near-vision testing. Comparable notations of measurement of visual acuity are explained, and the method of conversion from different notations is presented. One side of the chart has continuous, informative text in eight sizes progressing from 4 point to 24 point. The reverse side of the chart shows eight methods for testing visual acuity, namely: samples of the telephone directory, music, words, numbers, symbols, and a reduced distant chart. This side of the chart is designed especially for testing the vision of the partially sighted patient. The type size is designated using five methods of measurements.

The use of indomethacin, sulindac, and tamoxifen for the treatment of desmoid tumors associated with familial polyposis.

Seven familial polyposis patients with desmoid tumors were treated with indomethacin, sulindac, or tamoxifen either as single agents or in combination. Serial computed tomographic (CT) scan examination was employed for objective measurement of tumor size since physical examination alone was an inaccurate means to evaluate intraabdominal and retroperitoneal desmoids. Only one patient with minimal tumor burden demonstrated a favorable response with complete resolution of an abdominal wall desmoid.

Use of Karpas HIV cell test to detect antibodies to HIV-2.

Commercial enzyme-linked immunosorbent assays for HIV-1 infection often fail to detect antibodies to HIV-2. A simplified version of the HIV cell test detects antibodies to this new virus and enables typing of HIV infections. Since sera from 3 macaque monkeys infected with simian immunodeficiency virus gave weakly positive reactions, this test holds promise for discovering any further strains of HIV that may exist or evolve.

Double-blind controlled trial of single-dose treatment with bovine surfactant in severe hyaline membrane disease.

In a double-blind clinical trial the effects of a single dose of reconstituted bovine surfactant ('Surfactant TA') were assessed in 30 premature infants (birthweight 751-1750 g) with severe hyaline membrane disease. 17 infants had a sonicated saline suspension of 100 mg/kg surfactant phospholipid instilled into the trachea at 5.0 (SD 0.7) hours of age and 13 infants received saline by the same route at 4.3 (1.1) hours of age. In the surfactant-treated group there was early improvement in oxygenation and ventilation. Haemodynamically significant patent ductus arteriosus occurred more often in the surfactant group; pneumothorax and pulmonary interstitial emphysema occurred less often. The combined incidence of death and severe bronchopulmonary dysplasia was significantly lower in the surfactant group (3/17) than in the placebo group (9/13).
