[The value of echography in scrotal contusions. Personal experience apropos of 20 cases].

We report a serial case study of 20 patients presenting scrotal contusions and for whom an emergency scrotal ultrasonography was performed. This investigation is not a luxury. It was a useful confirmation diagnostic tool for hematocele in 1/3 of cases (7/20) as well as for the detection of associated epididymal and or testicular lesions. In 2/3 of cases (13/20) it detected lesions unsuspected by clinical examination so that a therapeutic decision was taken promptly. Scrotal echography is therefore a useful emergency tool not for major trauma where the surgical decision is evident, but for apparently benign trauma where the clinical examination is currently deficient and needs provided a trained ultrasonography operator is available.

Pharmacokinetics of methotrexate and its 7-OH metabolite in cancer patients treated with different high-methotrexate dosage regimens.

The pharmacokinetics of methotrexate (MX) and 7-OH methotrexate (MXOH) was studied in 18 cancer patients treated with 6-hr intravenous (i.v.) infusion of 100 mg/kg (A), 80 mg/kg preceded by 20 mg/kg i.v. loading dose (B), and 70 mg/kg preceded by 30 mg/kg i.v. loading dose (C). Simultaneous analysis of MX and MXOH was performed by high-performance liquid chromatography. The data for MX conformed to a 2-compartment model with overall mean +/- SD for beta, k12, k21, and k13 of 0.225 +/- 0.196, 1.33 +/- 1.44, 0.954 +/- 1.06, and 0.994 +/- 1.28 hr-1, respectively. The total body clearance, Vc, and V-beta were 0.123 +/- 0.037 l/hr.kg, 0.15 +/- 0.122 l/kg, and 0.965 +/- 0.875 l/kg, respectively. No significant differences (p greater than 0.05) in these parameters, attributable to the difference in regimens, were observed. With regimens A, B and C, the maximum observed concentrations of MXOH occurred at 9.2, 11.5, and 9.2 hr and the mean +/- SD values of these concentrations were 15.02 +/- 14, 18.96 +/- 13.63, 14.91 +/- 9.95 microM, respectively. With regimens A and C, maximum observed concentrations of MX equal to 231 +/- 67.1 and 204 +/- 69.5 microM occurred at 0.5 and 6 hr, respectively. Only with regimen B was a steady-state MX concentration of 179 microM achieved throughout infusion; this regimen is therefore highly advantageous for high-dose MX treatment.

Kaposi's sarcoma and its management by radiotherapy.

Thirteen patients with Kaposi's sarcoma were treated by radiotherapy between 1975 and 1984. Five patients were kidney transplant recipients receiving immunosuppressive drugs, while eight patients had spontaneous Kaposi's sarcoma. Eleven patients were followed up for periods from two to 63 months (mean, 27 months). All patients had complete response throughout the period of follow-up except one patient who developed recurrence one year after completion of radiotherapy. There was no difference in the response between transplant recipients and patients with spontaneous disease. This response was also unrelated to the dose or type of radiation used. Radiotherapy has been found effective in the local control of Kaposi's sarcoma with complete relief of symptoms and minimal morbidity.

Anti-androgens in treatment of acne vulgaris.

Seventy-five women suffering from acne vulgaris classified into three groups according to the grade of the disease. Anti-androgenic tablets cyproterone acetate (CPA), were given in three cycles. The total number of patients whose condition improved was 52% after the first cycle, 55% after the second cycle, and 81.3% after the third cycle. No serious side effects were encountered. CPA is a suitable therapeutic modility for AV in women who use contraceptives.

Experience with the classification, diagnosis, and therapy of nonejaculatory intercourse.

Twenty-one consecutive patients complaining of nonejaculatory intercourse were studied. Fifteen patients had anejaculation of various types, and sex therapy succeeded in 4 of 7 cases. Three patients had retrograde ejaculation that could be corrected by ephedrine sulfate in two cases having a neurologic cause. In one of three patients having nonemission, ejaculation was restored after stopping the drug thioridazine. A juvenile diabetic patient was discovered to have nonemission and not retrograde ejaculation. A classification of the disorders leading to nonejaculatory intercourse based on pathogenetic and clinical data is presented, including anejaculation, retrograde ejaculation, nonemission, and aspermia.

Serum zinc in acne vulgaris.

Serum zinc levels in acne patients and in matched healthy controls of both sexes and within the same age group (16-20 years age) were estimated. Women, whether diseased or healthy, have shown a significantly lower mean serum zinc level than the corresponding men. In advanced grades of acne, both men and women have a significantly lower level than the corresponding control groups. Within the same sex group, those with advanced grades have revealed a significantly lower level than those with slight grades.

Treatment of scabies: preliminary report.

Five topical treatments for scabies compared for efficacy in 85 infants with scabetic infestation. Preliminary results during the six-month study period showed lindane to be the most effective, with furacin the least effective.

Carcinoma of prostate metastatic to breast.

Clinically diagnosed breast metastasis from prostatic carcinoma is rare. Primary breast carcinoma in patients with prostatic primary is also uncommon. Four patients who presented with breast malignancies in the course of their prostatic carcinoma are described. All but one of them had diffuse metastatic disease. Three of them were on estrogens at the time breast malignancy was diagnosed. Difficulties always arise in differentiating primary lesions from metastasis clinically and histopathologically. The development of histochemical methods for acid phosphatase, and the newest indirect immunofluorescent antibody technique, used in one of our patients, helped in making the differentiation between primary lesion and metastatic disease. Diagnosis of prostatic carcinoma metastatic to breast carries a poor prognosis, and may be an indication for aggressive therapy.

Potential protective role of angiotensin-converting enzyme inhibitors captopril and enalapril against adriamycin-induced acute cardiac and hepatic toxicity in rats.

Captopril and enalapril-angiotensin-converting enzyme (ACE) inhibitors-were evaluated for their antioxidative protective action against adriamycin-induced cardiac and hepatic toxicity. Rats were treated with either captopril (10 mg kg(-1)) or enalapril (2 mg kg(-1)) intragastrically (i.g.) daily for 7 days before single intraperitoneal (i.p.) injection with adriamycin (15 mg kg(-1)). The animals were killed 30 h after adriamycin administration. Adriamycin produced significant elevation in thiobarbituric acid reactive substances (TBARS), which is an indicator of lipid peroxidation, and significantly inhibited the activity of superoxide dismutase (SOD) in heart and liver tissues, with a significant rise in the serum levels of glutamic pyruvic transaminase (GPT), glutamic oxaloacetic transaminase (GOT), creatine kinase isoenzyme (CK-MB) and lactic dehydrogenase (LDH), indicating acute cardiac toxicity. A single injection of adriamycin did not affect the cardiac or hepatic glutathione (GSH) content or cardiac catalase (CAT) activity, but hepatic CAT activity was elevated. Pretreatment with ACE inhibitors significantly reduced the TBARS concentration in both heart and liver and ameliorated the inhibition of cardiac and hepatic SOD activity. In addition, the ACE inhibitors significantly improved the serum levels of GOT, GPT, CK-MB and LDH in adriamycin-treated rats. Thus, these results suggest that captopril and enalapril possess antioxidative potential that may protect the heart against adriamycin-induced acute oxidative toxicity. This protective effect might be mediated, at least in part, by the limitation of culprit free radicals and the amelioration of oxidative stress.

Larva currens and systemic disease.

Of 26 patients infested with Strongyloides stercoralis 10 (38.5%) were asymptomatic without systemic or cutaneous signs. Nine patients (34.6%) presented with systemic complaints only and seven patients (26.9%) had systemic and cutaneous manifestations. Further observations of the skin lesions on four of those with systemic and cutaneous manifestations revealed linear urticarial bands, extending to several centimeters within 1 hour and persisting up to many days, waiting and waning. Blood examination showed eosinophilia in all patients. These findings confirm the concept that larva currens even alone should be considered a cutaneous sign of systemic disease.
