Treatment of progressive posthemorrhagic hydrocephalus with temporary external ventricular drainage. Preliminary results.

Peri-intraventricular hemorrhage (PIVH) is a major problem of preterm neonates: over 40% of infants with birth weight less than 1500 g have been found to experience this disorder. The posthemorrhagic hydrocephalus (PHH) is generally believed to occur secondary to obliterative posterior fossa arachnoiditis. Its management in critically ill premature infants with multiple medical problems has thus become quite significant. This paper reports the results of the placement of a temporary external ventricular drainage in three patients in whom progressive ventricular dilatation began within two weeks after severe neonatal PIVH. In all patients the hydrocephalus and increased intracranial pressure were controlled within three weeks of treatment. One infant died from other problems associated with his prematurity. Early demonstration of progressive ventricular dilatation by ultrasound and prompt external drainage has been found to be a safe initial method to treat PHH in preterm infants.

Fetal ventriculomegaly, agenesis of the corpus callosum and chromosomal translocation--case report.

Real-time ultrasound was used to evaluate intracranial anatomy and ventricular size in a child with fetal ventriculomegaly. It was associated with agenesis of the corpus callosum and chromosomal translocation. Serial measurements of cerebral ventricular dilatation at various gestational age may improve perinatal management of the newborn with fetal ventricular enlargement.

[Optimization of the thoracic radiograph. Experimental and clinical study on the use of various film/screen combinations using rare earths].

The optimization of chest radiography is a still unresolved problem, as it must answer to various clinical requirements. Even if sometimes the choice is not difficult, it becomes hard when the best film-screen combination is needed. In order to assess the optimization image quality and exposition, different experimental and clinical conditions have been investigated referring to the various combination of screen, film and tension. The Trimax system has been chosen for our investigations, performed in two different phases: the former on an anthropomorphous chest phantom, the latter on 25 patients affected by lung interstitial diseases, mainly nodular. The results are discussed.

Obstructive ventriculomegaly.

Intracranial Hemorrhage (ICH) occurs primarily in premature infants, and post-hemorrhagic ventricular dilatation is a well-known complication in this age-group. Blood clots within the ventricle are diagnostic of ICH but not usually seen. In our case Real-Time Ultrasonography has proved to be very helpful to identify intraventricular hematomas and to control obstructive ventriculomegaly without therapeutic intervention.

[Transcutaneous bilirubin determination: correlation in white premature infants weighing less than 1500 gm].

The transcutaneous bilirubinometer (TcB) was evaluated in 24 preterm infants of less than 34 weeks gestation and weight less than 1500 g. A close correlation between the TcB index and serum bilirubin values was obtained with 50 measurements at sternum (R = 0.858, p less than 0.001). From these resultant ratios it is concluded that the TcB-meter is a valuable tool in screening healthy preterm infants less than 34 weeks gestation for hyperbilirubinemia.

The interaction of the two isomers of the opioid benzodiazepine tifluadom with mu-, delta-, and kappa-binding sites and their analgesic and intestinal effects in rats.

The (+) and (-) isomers of tifluadom were assessed in rats for their opioid activities. In vitro (+)-tifluadom was almost equipotent at mu- and kappa- sites and about 10 times less potent at delta-sites: (-)-tifluadom had the same binding spectrum but was 10-20 times less potent. In vivo (+)-tifluadom delayed the hot-plate reaction time; this effect was antagonized by naloxone, but not by Ro 15-1788. (-)-Tifluadom up to 20 mg/kg had no antinociceptive effect. In the intestinal transit test analgesic doses of (+)-tifluadom did not delay the intestinal transit of a charcoal meal in rats and had weak antagonist activity against morphine-induced inhibition of intestinal transit, whereas (-)-tifluadom had neither agonist nor antagonist effect. It thus appears that (+)-and (-)-tifluadom are not selective in vitro and in vivo for one type of opioid binding site/receptor.

[Incidence of occipital ventriculomegaly in premature infants with peri-intraventricular hemorrhage].

Ventricular dilatation after periventricular/intraventricular hemorrhage (PIVH) is a more common complication than was assumed before the advent of ultrasound. In this prospective study based on 264 ultrasonic head scans, the true incidence of ventriculomegaly was investigated in two groups of preterm infants with PIVH. The major incidence of PIVH in babies with less than 34 weeks of gestation (p less than 0.05) was confirmed. Statistical analysis of the follow-up examinations showed that the degree of ventriculomegaly correlated well with the extent of the cerebral-intraventricular hemorrhage (X2 test, p less than 0.002).

Prognostic value of serum creatine kinase brain isoenzyme in term babies with perinatal hypoxic injuries.

Creatine kinase brain isoenzyme (CK-BB) may be released into the serum after brain injury. Its concentration was measured serially in a group of 80 term infants at risk of perinatal hypoxic damage. Their neurodevelopmental outcome was furthermore assessed at 12 months of age. Our results are consistent with the hypothesis that infants with high CK-BB concentration, measured in serum with a new and very sensitive method for enzyme analysis, have more frequently poor short-term outcome (p less than 0.02). These data suggest that early CK-BB measurement may be clinically useful to identify newborns at high risk for neurologic or developmental sequelae.

Interruption of breast-feeding versus phototherapy as treatment of hyperbilirubinemia in full-term infants.

Since 1963 there are reports in the literature about the syndrome of breast milk jaundice. The factors that may contribute to this phenomenon are still unclear. In our study the effect of interruption of breast-feeding in 25 jaundiced full-term infants was compared to the effect of phototherapy in 25 jaundiced otherwise healthy full-term infants. The results show that this treatment has the same therapeutic effect as phototherapy (p greater than 0.05) without its stressful complications and can be recommended as alternative treatment of this type of hyperbilirubinemia.

Double direction phototherapy and light-induced genetic abnormalities in human lymphocytes.

The irradiation of jaundiced infants with fluorescent lights is currently the most common method of treating neonatal hyperbilirubinemia. To investigate whether phototherapy is capable of damaging the genetic material, peripheral lymphocytes of ten jaundiced newborns were studied. Chromosomal analyses were carried out before and after 24 h of treatment with double blue light phototherapy at wavelengths from 420 to 500 nm. Using the BrdU technique for chromosomal culture, no detectable aberrations on genetic material of circulating lymphocytes were observed. This study doesn't, however, exclude potential deleterious effects of phototherapy on other tissues such as skin fibroblasts or immature ovaries in female infants.

[C-reactive protein in the serum of newborn infants].

Bacterial infections are more frequent and severe in the first few days of life than at any time thereafter. It is also well recognized that in newborn infants clinical signs of infection may be minimal: accordingly, laboratory tests are of greatest importance for diagnosis in this period of life. Because of the difficulties in interpreting the meaning of granulocyte changes in preterm infants, we have examined the possible clinical usefulness of serum c-reactive protein, determined by a new, sensitive electrophoretic method (ZIA), for detection of bacterial infection in infants of this age. This study suggests that c-reactive protein testing is a helpful adjunct in diagnosing bacterial infection in the first month of life.

Abdominal wall endometrioma: CT findings.

Endometriosis of the superficial soft tissues is an uncommon lesion that may occur in the absence of other evidence of pelvic endometriosis. An example of an endometrioma arising in the rectus sheath is reported here. To our knowledge, this is the first CT description of an anterior abdominal wall endometrioma.
