[Irregular behavior of complement in coeliac disease (author's transl)].

Ninety-six sera from coeliac children were studied, 55 were taken during a normal diet and 41 with a gluten-free diet. C'3, C'4, C'5, C'3, PA, C'1 INH and C'3b INA were measured. No significant differences were found between coeliac patients and normal controls. Results were also similar for gluten-free diet coeliac childrens and for active patients. A challenge was performed with 20 g. of gluten in 14 active patients. The levels of C'3, C'3 PA had a large variability, increasing and decreasing, but no similar and constant pattern was found. Activation products of C'3 appeared, specially between 4 and 24 hours, and disappeared about 48 hours. These results support an alteration of complement in coeliac patients but there is not a constant and steady pattern. Our hypothesis is that complement activation is not a pathogenic mechanism of coeliac disease. Maybe is just a consequence or epiphenomenon.

Gallium-67 radionuclide imaging in acute pyelonephritis.

The symptoms and clinical course of patients with acute pyelonephritis are variable; likewise, urinalysis, blood cultures, and excretory urography may be normal or equivocal. The ability of gallium-67 to accumulate in areas of active inflammation was useful in the diagnosis of acute pyelonephritis in 12 cases. A multiplane tomographic scanner was used for imaging four of these patients. Initial experience with this scanner is also discussed.

[Functional evaluation of pancreas cystic fibrosis. Correlations and value of the echocardiogram versus other methods (author's transl)].

The results of a correlation of various parameters used in the functional evaluation of 12 cases of pancreas cystic fibrosis (PCF) are reported. Three clinical scores were measured by four separate observers and average score was taken. Electrocardiogram, phono, vectorcardiogram, pH, blood gases, radiology, spirometry, erythropoietin, blood counts and echocardiogram were also recorded. Each of these were correlated. In the echocardiogram (EC), right and left systolic time intervals, wall and cavity of the right ventricle and the left ventricular function were measured. Some correlations were found to have a statistical significance: the data attained from the EC, clinical score and the spirometry. However, blood count, blood gases, erythropoietin and pH were found to have no statistical significance. The EC was found to be the most accurate and sensitive method of recognising the initial stages of pulmonary hypertension in PCF; detecting its presence at as early an age as six years (on average). These results present a strong argument for the use of the EC as a vital part of each exam of PCF.

Antiserum to murine leukemia virus recognizes novel cell surface molecules associated with growth control and transformation.

Antiserum directed against murine leukemia virus also reacts with several external proteins present in rat cells transformed by a temperature-sensitive Rous sarcoma virus. Reaction of iodinated cell extracts with anti-MLV (murine leukemia virus) serum revealed the presence of a 200,000 dalton iodinated component detectable also by metabolic labelling with glucosamine only in serum-starved cultures restricted in the expression of transformation. A similar assay with iodinated cells that express the transformed phenotype revealed the preferential recognition of two components with an approximate molecular weight of 100,00 daltons as well as an additional 65,000-dalton external component. Growth of the transformed non-producer NT3-KR cells in the presence of inducers of C-type viruses leads to an increased synthesis of a 100,000-dalton glycoprotein (gp100) recognized by the anti-MLV serum which is also recognized by the antiserum in NRK-MSV-MLV transformed producer cells, in addition to a virus-like glycoprotein of 71,000 dalton (gp71). Absorption of the anti-MLV serum with monolayers of NT3-KR cells eliminated the ability of the serum to recognize the gp100 but not the gp71 from NRK-MSV-MLV-transformed producer cells. The mediation of post-translational changes in growth control is suggested by the transformation-dependent alteration in the molecular weight of the non-virion surface proteins recognized by anti-MLV serum in the rat cells used in this study.

[Investigation of the hypoosmolal syndrome in carcinomas of the lung (author's transl)].

A study was performed on 30 patients diagnosed antomopathologically for malignant neoplasia of the lung (epidermoid carcinomas, adenocarcinomas, oat-cell carcinomas, and neoplasias which could not be definitely classified). The following parameters for blood and urine were determined: osmolality, sodium, potassium, urea, and creatinine. Osmotic free water, creatinine, sodium, and potassium clearances were also calculated, as well as the plasma osmolality/urinary osmolality ratio. The basic aim of our study was to investigate for the presence of disturbances in the metabolism of water and alterations in plasmatic and urinary osmolality in this type of tumor. These could appear as complete inadequate ADH secretion syndromes as discovered by Bartter and Schwartz or as incomplete syndromes (hypoosmolality and/or hyponatremia). Among the more significant results was the tendency toward oliguira seen in 44% of the patients and the high incidence of plasmatic hypoosmolality (31%). In three patients plasmatic hypoosmolality and hyponatremia were concommitant in repeated observations. A complete inadequate ADH secretion syndrome was discovered in another patient with an oat-cell carcinoma. He presented plasmatic hypoosmolality, hyponatremia, relative urinary hypertonia, and oliguria but not renal, suprarenal, or hepatic pathology.

Adverse hemodynamic and ultrastructural changes in dog hearts subjected to protein-calorie malnutrition.

In the absence of thiamine deficiency, the specific effects of protein-calorie malnutrition on left ventricular (L.V.) function are unknown. Mature beagle dogs of both sexes were subjected to a hypocaloric, nitrogen-poor diet which resulted in a weight loss of approximately 40% after seven weeks. Following preparation of this nutritional model, myocardial contractility was assessed acutely by obtaining isovolumetric L.V. contractions on cardiopulmonary bypass at constant heart rate, mean aortic pressure, and at a wide range of end-diastolic volumes. These changes were compared to a matched group of animals which were normally fed. There were consistent decreases in L.V. compliance in malnourished animals compared with normals; indices of ventricular contractility per se (L.V. dp/dt, force-velocity relations, peak developed L.V. pressure) were also diminished in the experimental animals. Myocardial concentration of glycogen was diminished in malnourished compared to control animals. Light and electron microscopic examination confirmed the presence of myofibrillar atrophy in the presence of interstitial edema. These results suggest that protein-calorie malnutrition seriously interferes with normal L.V. function in the experimental animal by reducing compliance as a result of "starvation edema," and by reducing myocardial contractility associated with atrophy of the myofibers.

Echocardiographic sign of right-sided hypertrophic obstructive cardiomyopathy.

The echocardiographic and haemodynamic findings with a patient with hypertrophic obstructive cardiomyopathy (HOCM) and dynamic subpulmonary stenosis, without left ventricular gradient, are described. The echocardiogram shows not only asymmetric septal hypertrophy, systolic anterior movement of the anterior mitral leaflet, and midsystolic collapse of the aortic valve, but also increase in the right ventricular wall echoes and systolic collapse of the pulmonary valve. We believe that in the absence of pulmonary hypertension these signs, particularly the systolic closing movement of the pulmonary valve cusp, may suggest right ventricular obstruction in HOCM.

[Laryngeal tuberculosis: study of 11 cases].

We report 11 patients with laryngeal tuberculosis seen in our hospital, January 1990 to July 2000. Eight were men and all cases presented with dysphonia and/or disphagia. In 8 pulmonary tuberculosis was associated. Mycobacterium tuberculosis was isolated from the sputum in 7 patients. Granulomatous laryngitis was demonstrated in the eight patients with laryngeal biopsy. The evolution with medical treatment was favourable in all patients.
