The relative abundance of ethane to acetylene in the Jovian stratosphere.

The observed ratio of C2H6 to C2H2 in the Jovian stratosphere increases from approximately 55 at 2 mbar to approximately 277 at 12 mbar. In current photochemical models this ratio typically increases between 2 and 12 mbar by a factor of < or = 3. Recent laboratory kinetics studies on the reaction between C2H3 and H2 to form C2H4 suggest an efficient chemical mechanism for hydrogenation of C2H2 to C2H6. Inclusion of this scheme as part of a comprehensive updated model for hydrocarbon photochemistry in the atmosphere of Jupiter provides an explanation of the altitude variation of the C2H6/C2H2 ratio. The sensitivity of these results to uncertainties in the key rate constants at low temperatures is illustrated, identifying needs for additional laboratory measurements. Since the key reaction rate constants decrease with decreasing temperature, the hydrogenation of C2H2 as proposed predicts a qualitatively decreasing trend in the C2H6/C2H2 value with decreasing distance from the Sun. The observed variation between Jupiter and Saturn is consistent with this prediction.

Hydrocarbon nucleation and aerosol formation in Neptune's atmosphere.

Photodissociation of methane at high altitude levels in Neptune's atmosphere leads to the production of complex hydrocarbon species such as acetylene (C2H2), ethane (C2H6), methylacetylene (CH3C2H), propane (C3H8), diacetylene (C4H2), and butane (C4H8). These gases diffuse to the lower stratosphere where temperatures are low enough to initiate condensation. Particle formation may not occur readily, however, as the vapor species become supersaturated. We present a theoretical analysis of particle formation mechanisms at conditions relevant to Neptune's troposphere and stratosphere and show that hydrocarbon nucleation is very inefficient under Neptunian conditions: saturation ratios much greater than unity are required for aerosol formation by either homogeneous, heterogeneous, or ion-induced nucleation. Homogeneous nucleation will not be important for any of the hydrocarbon species considered; however, both heterogeneous and ion-induced nucleation should be possible on Neptune for most of the above species. The relative effectiveness of heterogeneous and ion-induced nucleation depends on the physical and thermodynamic properties of the particular species, the abundance of the condensable species, the temperature at which the vapor becomes supersaturated, and the number and type of condensation nuclei or ions available. Typical saturation ratios required for observable particle formation rates on Neptune range from approximately 3 for heterogeneous nucleation of methane in the upper troposphere to greater than 1000 for heterogeneous nucleation of methylacetylene, diacetylene, and butane in the lower stratosphere. Thus, methane clouds may form slightly above, and stratospheric hazes far below, their saturation levels. When used in conjunction with the results of detailed models of atmospheric photochemistry, our nucleation models place realistic constraints on the altitude levels at which we expect hydrocarbon hazes or clouds to form on Neptune.

Hemodynamic effects of oral amiodarone on left ventricular function before and after global ischemia.

We evaluated the hemodynamic effects of amiodarone on left ventricular (LV) function after global ischemia. Amiodarone was administered orally at 15 mg/kg/day for an average of 28 days to a group of 10 dogs. The concentration of amiodarone in serum and LV myocardium was 0.40 +/- 0.24 micrograms/ml and 44 +/- 27.0 micrograms/g, respectively. This experimental group and a control group of 10 dogs each underwent 120 min of aortic cross-clamping with cold crystalloid cardioplegia resulting in an average myocardial temperature of 15 degrees C. LV function measurements by pulse-transit sonomicrometry and Millar solid-state micromanometers were processed by a minicomputer. Compared to the nontreated control group, oral amiodarone for 28 days produced depression of LV contractility which was reflected by lower slope of the end-systolic pressure-volume relation (Evmax), percentage shortening of segment length, left ventricular pressure-segment length loop area, and slope of the end-systolic pressure-segment length relation (Esmax). After ischemia, percentage recovery of LV global function (Evmax and mean velocity of circumferential fiber shortening) and regional function (Esmax) was significantly better in the amiodarone group than in the control group. We conclude that oral amiodarone for 28 days results in a depression of LV contractility but the combination of amiodarone and ischemia does not act synergistically to further depress postischemic LV function.

Local tumor recurrence following "radical prostatectomy".

The resurgence of radical retropubic prostatectomy as a commonly employed treatment for organ-confined prostate cancer is largely a result of the reduction in morbidity related to the procedure. One aspect of this reduced morbidity is the potential to preserve potency. The technical points required to perform this procedure must be followed precisely or there is the potential for performing an inadequate cancer operation if these steps are not faithfully followed. We recently encountered a patient with local tumor recurrence because the initial urologist failed to adhere to these principles. A "salvage" procedure was performed.

Serum magnesium levels in asthmatic patients during acute exacerbations of asthma.

The purpose of this study was to determine whether serum magnesium levels in asthmatic patients during acute exacerbations differ from those of a control population. Twenty-three known asthmatics presenting to the emergency department in acute exacerbation (cases) and 15 nonasthmatic patients (controls) matched for age, sex, race, and socioeconomic status had serum magnesium assays drawn. Admission criteria were: age 18 to 50 years with no history of alcoholism, heart disease, renal disease, or diuretic use. Patients giving a history of pregnancy were excluded. Serum magnesium levels were not significantly different in the two study populations, nor did they correlate with the severity of asthma (mean values: cases, 2.04 +/- 0.159 versus controls, 2.03 +/- 0.134 mg/dL; SD of the difference of the means = .048). An analysis for beta-error demonstrated the true difference of the means to be less than .1 (95% confidence) or less than .13 (99% confidence). In conclusion, serum magnesium levels in asthmatics are not significantly different from those of a control nonasthmatic population. They are not clinically useful for predicting the severity of disease.

Sex differences in self-disclosure: a meta-analysis.

A meta-analysis of 205 studies involving 23,702 Ss was conducted to determine whether there are sex differences in self-disclosure. Across these studies, women disclosed slightly more than men (d = .18). This effect size was not homogeneous across studies. Several moderator variables were found. Sex of target and the interaction effect of relationship to target and measure of self-disclosure moderated the effect of sex on self-disclosure. Sex differences in self-disclosure were significantly greater to female and same-sex partners than to opposite-sex or male partners. When the target had a relationship with the discloser (i.e., friend, parent, or spouse), women disclosed more than men regardless of whether self-disclosure was measured by self-report or observation. When the target was a stranger, men reported that they disclosed similarly to women; however, studies using observational measures of self-disclosure found that women disclosed more than men.

Minisatellite variant repeat (MVR) mapping: analysis of 'null' repeat units at D1S8.

Minisatellite variant repeat mapping by PCR (MVR-PCR) is a new approach to studying variation in human DNA which analyses interspersion patterns of variant repeats within minisatellite arrays. MVR-PCR has been applied to the hypervariable human minisatellite D1S8 which contains two major classes of variant 29bp repeat units designated a-type and t-type. The MVR-PCR assay uses a- or t-type specific primers, together with an amplimer at a fixed site in the DNA flanking the minisatellite, to reveal the interspersion patterns of variant repeats along an allele. Extreme levels of variation are seen both in the internal structures of individual alleles and in the digital code generated from the two superimposed alleles in total genomic DNA. However, occasional repeat units fail to amplify in MVR-PCR, signifying the existence of further repeat sequence variants termed 'null' or O-type repeats. Although not significant in individual identification, correct genotyping of null repeats is important when using MVR digital codes in parentage analysis. We have therefore characterised these null repeats and show that most null repeats share a common variant repeat sequence. We discuss the possible origins of null repeats and their application to paternity testing and the analysis of minisatellite evolution.

Effectiveness of a preoperative teaching programme for cataract patients.

Shortened hospital stays have necessitated examination of alternative approaches for pre and postoperative teaching. An investigation of one alternative became a study in which a home care preoperative teaching programme for older adults undergoing cataract extractions with lens implants was implemented and evaluated. The findings indicated that the teaching programme was effective and resulted in positive benefits.

Ethnic variation in postnatal assessments of gestational age: a reappraisal.

While the possibility of an ethnic bias in postnatal assessments of gestational age has been suggested by several investigators, others have reported that postnatal assessments do not provide biased estimates in non-White ethnic groups. In the light of this ongoing controversy, this study examines the validity of the Ballard postnatal assessment of gestational age by ethnicity, using a relatively large hospital data base that allows for the inspection of ethnic variations in the agreement between the Ballard assessment and last menstrual period (LMP). The results indicate that there is a greater over-estimation of the LMP interval by the Ballard method in Blacks compared with Whites and suggest that systematic differences exist by ethnicity of mother in the agreement between the Ballard postnatal assessment and the LMP interval. After taking maternal characteristics and pregnancy complications into account, for a given gestational age interval, Blacks have on average a greater level of maturity as measured by Ballard. One interpretation of these findings is that postnatal assessments may provide biased over-estimates of the LMP gestational age interval in certain ethnic groups. An alternative interpretation of these data is that the gestational age interval based on LMP is not a valid indicator of fetal maturity, readiness for birth and infant risk status across all ethnic groups.

Esophageal function and gastroesophageal reflux during sleep and waking in patients with chronic obstructive pulmonary disease.

This study was conducted to assess esophageal function and pulmonary resistance changes with esophageal acidification in patients with COPD. Twelve patients with COPD and a mean age of 55.6 years were studied. Each patient underwent standard esophageal manometry, 24-h ambulatory pH testing, esophageal acid clearance evaluation during sleep, and an assessment of pulmonary resistance with and without esophageal acid perfusion. Neither airway resistance nor conductance was altered by the esophageal acid infusion; LES pressures were normal and esophageal contractile pressures were mildly decreased. Acid exposure in the upright and supine positions was within normal limits. Acid clearance during sleep was similar to that in normal subjects; however, acid clearance during waking appeared to be somewhat prolonged in the COPD patients. We conclude that patients with COPD do not have a bronchoconstrictive reflex to distal esophageal acidification, and esophageal function in these patients appears to be relatively normal.

Behavioral effects of benzodiazepine ligands in non-dependent, diazepam-dependent and diazepam-withdrawn baboons.

Acute i.m. injections of benzodiazepine receptor ligands were administered to baboons before 1-h observational sessions. The agonist midazolam produced sedative effects, the antagonist flumazenil produced no behavioral effects, the inverse agonist FG7142 produced tremor and the inverse agonist 3-carboethoxy-beta-carboline hydrochloride (beta CCE) produced tremor, vomiting, jerks and seizures. Co-administration of these drugs (midazolam + beta CCE, midazolam + flumazenil or flumazenil + beta CCE) produced a mutual antagonism of these effects. Compared to the non-dependent condition, in the diazepam-dependent condition (baboons maintained on 20 mg/kg per day diazepam) and withdrawn condition (dependent baboons tested during withdrawal), midazolam produced decreased sedative effects, flumazenil produced increased effects (i.e., tremor, vomiting and jerks), and beta CCE produced increased frequency of seizures. Taken together, these data suggest that (1) benzodiazepine receptor ligands lie on a continuum of behavioral activity, and (2) chronic diazepam administration alters the behavioral effects of these benzodiazepine ligands, producing a shift in the direction of the inverse agonist.

Alterations in brain high-energy phosphate and membrane phospholipid metabolism in first-episode, drug-naive schizophrenics. A pilot study of the dorsal prefrontal cortex by in vivo phosphorus 31 nuclear magnetic resonance spectroscopy.

In this pilot study, membrane phospholipid and high-energy phosphate metabolism were studied in the dorsal prefrontal cortex of 11 drug-naive, first-episode schizophrenic patients and compared with those of 10 healthy control volunteers comparable in age, education, and parental education. The schizophrenic patients had significantly reduced levels of phosphomonoesters and inorganic orthophosphate and significantly increased levels of phosphodiesters and adenosine triphosphate compared with the controls. The levels of phosphocreatine and adenosine diphosphate did not differ in the two subject groups. The adenosine triphosphate and inorganic orthophosphate findings suggest functional hypoactivity of the dorsal prefrontal cortex. The phosphomonoester and phosphodiester findings are compatible with either premature aging or an exaggeration of normal programmed regressive events occurring in the neural systems sampled.

Catheter balloon valvuloplasty of stenotic porcine bioprosthetic valves: Part I: Anatomic considerations.

During the last several years, dilating balloons have been applied in the treatment of stenotic cardiac valves. This interest has recently been extended to stenotic porcine bioprosthetic valves. Part I of this review discusses the pathologic processes responsible for degeneration of porcine prostheses: intrinsic calcification (mineralization), cuspal tears and perforations, and cuspal thrombosis (non-infective). These changes form the basis of applied balloon valvuloplasty techniques.

Catheter balloon valvuloplasty of stenotic porcine bioprosthetic valves: Part II: Mechanisms, complications, and recommendations for clinical use.

During the last several years dilating balloons have been applied in the treatment of stenotic cardiac valves. This interest has been extended to stenotic porcine bioprosthetic valves. Part I of this review discusses the pathologic changes producing stenotic porcine prosthetic valves. Part II of this review describes an in vitro study of porcine prosthetic valve valvuloplasty defining the mechanisms, complications, and clinical applications. Results of this study indicate a limited and cautious role in balloon dilation of stenotic bioprosthetic valves.

Anterior sternal retraction for reoperative median sternotomy.

The incidence of reoperative median sternotomy for repeat cardiac surgery is increasing. Reoperative median sternotomy is associated with a higher morbidity and mortality than first-time cardiac surgery. A portion of this morbidity and mortality may be due to direct injury to the heart and great vessels in the process of reopening the sternum. We report a new technique utilizing anterior sternal retraction that allows division of adhesions between the undersurface of the sternum and the heart and great vessels under direct vision. This technique enables the surgeon to minimize the risk of serious injury to these underlying structures during reoperative cardiac surgery.

Troposphere-stratosphere interactions in a one-dimensional model of Jovian photochemistry.

A simple one-dimensional model of the Jovian atmosphere including the coupling between a rapidly mixed troposphere and a stagnant stratosphere is presented. We treat analytically the case of a chemically unreactive species flowing downward through the stratosphere and troposphere with a constant flux. The calculated concentration profile has a maximum value approximately one atmospheric scale height above the tropopause. The corresponding mixing ratio rapidly decreases in the lower stratosphere. The contrast between the peak stratosphere and tropopause concentrations reflects the variation between the lower stratosphere and upper troposphere eddy diffusion coefficients. Numerical simulations of unreactive CO and C2H6, considering high-altitude photochemical sources (yielding downward fluxes to the troposphere) and deep troposphere thermochemical sources, demonstrate that upper troposphere abundances may have a large photochemical contribution. Recent observations of CO near 5 bar can be reproduced by a model using any one of three different tropospheric eddy diffusion profiles: a constant value of approximately 10(8) cm2 sec-1, a constant value of < or approximately 10(4) cm2 sec-1, or a two-layer model with a rapidly mixed (10(8) cm2 sec-1) layer below 20 bar and a slower mixing layer (10(4) cm2 sec-1) between 100 mbar and 20 bar. In the latter two scenarios, the photochemical source is an important and/or dominant source of upper tropospheric CO. However, the upper tropospheric C2H6 abundances are distinctly different among the three cases. These calculations suggest objectives for higher spectral resolution observations, critical vertical scales for planning experiments on future missions to Jupiter, and predictions that can be tested with the Galileo probe mass spectrometer.
