Correlation of the diagnostic skin test with the immunoperoxidase assay in ragweed hypersensitivity.

The results of the diagnostic skin test are correlated with those obtained with an immunoperoxidase (IP) assay in 17 patients with positive histories for ragweed hay fever. The IP assay, a version of the ELISA technique, employs polystyrene microtiter plates as the solid-phase, horseradish peroxidase as the enzymatic label and O-phenylene diamine as the enzyme substrate. Concordance between the results of these two diagnostic techniques was observed in 66 patients (86%). Among the 11 patients with discordant results the modified RAST test was in agreement with the IP assay in seven and with the diagnostic skin test in four patients. In these latter four patients ragweed-specific IgE antibodies were detected by the IP or the modified RAST assays in low titers. The coefficients of variation for the IP assay ranged from 8% to 14.3%. The pros and cons of the use in vitro tests (the IP and the RAST test) and the diagnostic skin test in clinical practice of allergy are discussed.

Cronkhite-Canada syndrome: report of a case with bacteriologic, immunologic, and electron microscopic studies.

A case of Cronkhite-Canada syndrome is presented. The patient developed protein-losing enteropathy, generalized gastrointestional polyposis, skin pigmentation, onychodystrophy, and alopecia. This patient also exhibited certain unusual features such as: fluctuating dermal manifestations, sclerodermalike skin changes, elevated levels of carcinoembryonic antigen, retinal detachment, cataracts, and cerebellar atrophy. Bacteriologic studies of small intestinal fluid, chromosomal analysis, immunologic investigations, and morphologic studies failed to provide any etiologic clues.

Localization of IgE in adenoids and tonsils: an immunoperoxidase study.

An immunoperoxidase technique was applied to the localization of IgE in formaldehyde-solution-fixed, paraffin-embedded tissue. The content and distribution of IgE in tonsil and adenoid tissues from ten patients with histories of inhalant allergies and elevated serum levels of total and allergen-specific IgE were investigated. Compared with tissues from five nonatopic subjects, the tissues from atopic individuals were observed to harbor a much larger population of IgE-formating plasma cells. The profusion of IgE plasma cells in nasopharyngeal lymphoid tissue, observed in this study, furnishes strong anatomic evidence for a pathogenetic role of IgE in inhalant allergies. For cellular and tissue localization of IgE, the immunoperoxidase technique offers clear advantages over previously used immunoflorescence procedures.

Interference by sulfinpyrazone and salicylate of aspirin inhibition of platelet cyclooxygenase activity.

The irreversible effect of acetylsalicylic acid on platelet cyclooxygenase activity is inhibited by salicylate and by sulfinpyrazone. Since acetylsalicylate is rapidly hydrolysed in plasma to salicylate which has a relatively prolonged half life in plasma, it is possible that persistant elevation of plasma salicylate may interfere with the efficacy of subsequent doses of aspirin. Sulfinpyrazone is sometimes used in combination with aspirin, and could interfere with the efficacy of aspirin on platelets. This theoretical possibility would be more likely to occur at lower doses of aspirin.

Intrarenal vascular alterations in hemodialysis patients. A semiquantitative light microscopic study.

The intrarenal arterial and arteriolar changes in the kidneys of 80 patients with chronic renal insufficiency maintained on hemodialysis were studied semiquantitatively by light microscopy and histochemistry. Intimal proliferation was common and accounted for thickening of vessel walls and luminal narrowing. Fibrocollagenous and fibroelastotic intimal changes were located predominantly in the interlobular and arcuate arteries, whereas the fibromucinous intimal lesion was found mainly in the interlobar arteries. Thickening of the media was encountered in 25 per cent, and adventitial fibrosis in 20 per cent of the kidneys. Necrotizing arterial of arteriolar lesions were not seen. The significance of these alterations as a possible cause of renal ischemia and their role in the perpetuation of hypertension are discussed.

Fibromatosis of the breast.

Two cases of fibromatosis of the female mammary gland treated by local excision and with a follow-up of 11 and 3 years, respectively, are described. Fibromatosis confined to the breast and not involving the underlying fibroaponeurotic fascia is a rare lesion. Among the handful of cases reported in the literature, diagnostic errors are known to have led to unnecessary mastectomy. In the breast, fibromatosis behaves in the same fashion as in the soft tissues: an aggressive infiltrative lesion with a proclivity for local recurrence following inadequate excision but without potential for distant metastasis. Clinically, it may simulate carcinoma; when suspected histologically during frozen section study, surgical resection should be limited to wide local excision and a definitive plan of therapy deferred until the lesion can be examined carefully with permanent sections.

Humidity and the anesthetized patient.

Damage to the ciliated cells of the tracheobronchial tree and incidence of postoperative pulmonary complications were measured by point-scoring systems in 202 patients who breathed dry and humidified anesthetic gases for 225 +/- 78 min. The incidence of postoperative pulmonary complications decreased as the humidity of administered anesthetic gases increased from 0 to 32.5 mg H2O/l. A similar relationship was found between the amount of inhaled moisture and the damage to the ciliated epithelium of the tracheobronchial tree. These results appear to indicate that a high inspired humidity is beneficial for operations on normothermic patients, and that cellular damage caused by dryness is a possible contributory factor in the production of the pulmonary atelectasis that follows stoppage of the mucociliary transport system in the immmediate postoperative period.

Relative recovery of anaerobes on different isolation media.

The recovery of clinical anaerobic isolates on selective and nonselective agar media, as well as the time required to detect the isolates, was examined. Of a total of 235 isolates, 77, 46, and 40% were detected on Schaedler blood agar, colistinnalidixic acid blood agar, and kanamycin-vancomycin-lysed blood agar, respectively, and 94% were detected on the combination of Schaedler blood agar with kanamycin-vancomycin-lysed blood agar. A total of 19% of the anaerobes were detected after incubation for 1 day, and 70% were detected after 2 days.

Effects of hypertonic mannitol on cardiac lymph in nonischemic myocardium.

This investigation was designed to elucidate the effects of hypertonic mannitol on the nonischemic myocardium. In anesthetized open-chest dogs, we collected cardiac lymph for 2 h before and for 4 h during the infusion of hypertonic mannitol or normal saline. We studied the changes in the volume of lymph flow and the release of lysosomal enzyme acid phosphatase and protein into the cardiac lymph. In group 1 (n = 7) and group 3 (n = 8), the effects of 1-h and 4-h infusions of mannitol were studied, respectively. In these dogs, mannitol caused significant rises in lymph flow and release of total acid phosphatase into the cardiac lymph. In group 3, protein efflux increased significantly. In group 2 (n = 5) and group 4 (n = 4), infusion of normal saline for 1 and 4 h did not significantly change the release of total acid phosphatase and proteins into the cardiac lymph. Thus, infusion of hypertonic mannitol for 1--4 h in the nonischemic myocardium may have some deleterious effects because of its apparent ability to impair lysosomal membranes and to increase capillary permeability.

Metastatic carcinoma simulating linitis plastica of the colon. A case report.

Reported herein is the case of a 70-year old woman with metastatic breast carcinoma presenting as linitis plastica of the ascending colon. The breast tumor was diagnosed nine years previously. The clinical, roentgenographic and pathologic features of colonic linitis plastica and the preoperative diagnostic difficulties are discussed. There is no difference in the morphologic features of primary and secondary linitis plastica. The latter may result from anaplastic carcinoma of the stomach, gallbladder or breast.

Localization of IgE in tissues by an immunoperoxidase technique.

An indirect immunoperoxidase technique was used to study the content and distribution of IgE in formaldehyde solution-fixed, paraffin-embedded adenoid and nasal tissues of atopic and nonatopic persons. Adenoid tissue from 15 patients, and nasal tissue from five patients with symptoms of inhalant allergies and augmented serum levels of total and allergen-specific IgE were examined with this method. Adenoid tissues from five patients without allergic symptoms were studied for comparison. A rich content of IgE, largely within the cytoplasm of the plasma cells, was observed in tissues of atopic subjects. The sections from nonallergic persons contained few weakly staining IgE-positive cells. These observations provide anatomic support for the role of IgE in hypersensitivity reaction of immediate type. This immunoperoxidase technique, circumventing the principal shortcomings of the immunofluorescence procedures, affords a highly sensitive and practical approach to cellular and tissue localization of IgE.

The pathology of end-stage renal disease in hemodialysis patients.

The pattern of the renal disease and the pathological changes were studied in the kidneys of 80 autopsied patients who were maintained on hemodialysis for periods of up to seven years. Chronic pyelonephritis was most frequently encountered (25%); next in frequency were chronic glomerulonephritis (17.5%) and nephrosclerosis (17.5%). Moderate to severe intrarenal vascular changes were seen; intimal changes were most prominent. Statistically significant differences were observed in the distribution of the initimal lesions in intrarenal vessels of different calibres. Deposits of oxalate crystals, usually in the renal tubules, were encountered in all kidneys except four. Twenty-four patients (30%) showed acquired cystic kidney disease; renal calcification was observed in 61 others. The pathogenesis of these lesions in hemodialysis kidneys and their clinical significance are discussed.
