This paper describes the development and preliminary testing of a functional upper-limb orthosis for people that have limited strength in their arms. This is symptomatic of conditions such as muscular dystrophy (MD), spinal muscular atrophy (SMA), and partial spinal cord injury. The exoskeletal orthosis is wheelchair mounted, has two links and four degrees of freedom. It uses linear elastic elements to balance out the effects of gravity in three dimensions. Preliminary results on testing with ten subjects will be presented.

Twenty-three patients with oat cell cancer of the lung were randomized to receive chemotherapy with POCC (procarbazine, 100 mg/m2/day orally, Days 1-14; vincristine, 2 mg iv, Days 1 and 8; cyclophosphamide, 600 mg/m2 iv, Days 1 and 8; and CCNU, 60 mg/m2 orally, Day 1) or COM (cyclophosphamide, 2000 mg/m2 iv, Day 1; vincristine, 2 mg iv, Day 1; and methotrexate, 30 mg/m2 iv, Day 15). After two to three cycles of chemotherapy, all patients were to receive radiotherapy to initially involved sites and then have chemotherapy continued. Patients treated with POCC had a median survival of 14 months vs 10 months for those treated with COM (P = 0.055). Eight of 15 first sites of relapse were intrathoracic and five of these eight had received radiotherapy to that site. Six central nervous system (CNS) failures were evenly divided between the two chemotherapy programs. Thus, the CNS penetration of procarbazine and CCNU in the POCC combination did not prevent CNS relapse. All future patients will receive prophylactic brain radiotherapy.

Despite evidence indicating the effectiveness of behavioral techniques in treating anxiety symptoms and disorders, treatment of anxiety in the medical setting has tended to be largely pharmacological. This research project investigates the attitudes of a sample of practicing physicians toward utilization within their practices of non-pharmacological approaches in the treatment of anxiety.

Under the circumstances of this experiment, the following conclusions may be drawn: The fine-cut alloy system (Velvalloy) has a higher resistance to fracture than the spherical alloy system (Spheraloy) or the dispersed-phase alloy system (Dispersalloy). The class II preparation for posterior teeth should contain a round axiopulpal line angle. Further studies to evaluate other modifications of preparation design are justified.

This article discusses the use of clips of popular movies on videotape to educate family practice residents in the psychosocial aspects of medical care. Video clips anchor residents' insights about patients from clinical practice and illustrate family life cycle issues and Diagnostic and Statistical Manual of Mental Disorders-Revised diagnoses. Movies capture learners' attention, expose residents to diverse life-styles, engage the humanistic side of physicians, and imprint powerful pictorial images in memory. Teaching with film clips is time efficient and provides emotionally engaging experiences for faculty and residents. Selected films are reviewed for suitable clips and then incorporated into 1-hour teaching conferences. Successful "cinemeducation" requires appropriate films on videocassette, a VCR with a real-time counter, a television screen in clear view of all class members, audio speakers with sufficient volume to hear dialogue without distortion, and a willingness to be open to the emotional impact of movies. A detailed list identifies movie scenes that can be readily incorporated into the psychosocial teaching program of any medical education curriculum.

The authors of an evaluation of the new programmes of nurse and midwife education in Scotland summarise their findings and methods. Students were found to be enjoying rich educational experiences. However, they did identify a number of problems in initial programme development, achievement of student roles and in assessment of learning. The authors offer recommendations for continued curriculum development and for further research. The evaluation was commissioned by the National Board for Scotland.
